A. Personal details which includes your Full Name, Date of Birth, Contact Number.
B. Sensitive Information which includes your Gender, Birth Place, Civil Status , Place of Birth,
Date of Graduation.

HOW WE USE YOUR PERSONAL AND

The Sacred Heart Academy of Lupao, Inc. uses the gathered information for the following
purposes.

(A) To provide requested copies of School Records such as SF10 / Form 137, Certificate
of Graduation, Certificate of Good Moral Character, Diploma.

(B) To provide School Forms such as SF1 and SF2 of the enrolled students.

(C) To file Registration Forms of the enrolled students.

SECURITY MEASURES OF YOUR

The privacy of your information is important. That is why we maintain, enforce and implement
physical, electronic, and procedural security measures that ensure the availability, integrity,
confidentiality, and security of your information.

RETENTION PERIOD

Your personal data will be retained for a reasonable period in accordance with our legal and
business purposes prescribed by the Department of Education for retention of School Records.

HOW TO ACCESS YOUR

You may access your personal information, update, and request copies of your records by
securing the following:

(1) If Graduated / Completed, Enrolled Student, a Transaction form must be filled out before
releasing of requested records. Such request may be released after 3 working days.

* The requester is the only authorized to claim the requested document. If not, a copy of
Authorization and valid ID of both the requester and representative must be presented before
releasing of the document/s.
