currently a resident of 49E AGUILAR STREET SAN FRANCISCO DEL
MONTE, QUEZON CITY, do hereby name, constitute and appoint ELISA
T. CHUA, of legal age and with residence address at 49E AGUILAR
STREET SAN FRANCISCO DEL MONTE, QUEZON CITY, to be my true
and lawful attorney-in-fact, for me and in my name, place and stead to do
the following acts and deed, to wit:

To claim and receive in my behalf any due amounts or cheque

relative to the PAG-IBIG Provident Retirement benefits from the
HOME DEVELOPMENT MUTUAL FUND OFFICE, QUEZON
AVE. BRANCH, bearing my identification documents in the form
of Social Security System ID and PAGIBIG ID and further to do
and enter the following:

a.) The possibility of an amicable settlement of submission to

alternative modes of dispute resolution;
b.) The simplification of the issues;
c.) The necessity or desirability of amendments to the
pleadings;
d.) The possibility of obtaining stipulations or admission of
facts and of documents to avoid unnecessary proof;
e.) The limitation of number of witnesses;
f.) The advisability or necessity of suspending the proceeding,
and;
g.) such other matter as may aid in the prompt disposition of
the action.
