CHECKING OF SY2019-2020 SENIOR HIGH SCHOOL FORMS

1. In relation to DepEd Order 11, s. 2018 on “Guidelines on the Preparation and Checking of
School Forms,” the division shall conduct the checking of forms to all Senior High School
Advisers from March 16, 2020 onwards.

2. SHS advisers must see the following SHS personnel for this activity;

All Public SHS except CCNHS – Main Campus

These personnel are assigned to spearhead the reading of forms by giving priority to SHS
Grade 12 advisers. They are task to come up with their own schedule per adviser. Three (3)
tables are already ready for the reading of forms in the Division SHS Office; however, the
SHS registrars have the option to conduct the reading of forms in their office.

3. All SHS are required to bring the following school forms on their scheduled date.

Public and Private SHS

a. for Grade 12: a soft copy and two (2) hard copies of printed original SF 10 or Form 137
b. for Grade 11: a soft copy and one (1) hard copy of printed original SF 10 or Form 137
(print only the front page)

i. Summary of Grading Sheet (public shs only)
j. School Form Checking Report

4. For Private Senior High School, the following are the added requirements for the issuance of
Special Order (RM QAD No. 10, s. 2018).

a. Letter of Intent
b. master list (soft and hard copy) of Grade 12 learners eligible for graduation
b.1 alphabetically arranged (last name, first name, middle name)
b.2 male or female
b.3 per section, per tract, per strand and per specialization
b.4 month and year of graduation
c. accomplished Form 9
d. certified true copy of the students’ Form 137 (current and previous school if students is
transferee
e. photocopy of the temporary permit to operate (per track, strand and specialization)
f. verified copy of the original birth certificate (PSA/NSO)
g. certification from the school registrar/school principal if the student has taken (or is
taking) summer class

4. Attached is the schedule of the Senior High School for the checking of their forms. It is
understood that reading of forms in the school was already conducted, and if by chance major
errors are noticeable, the SHS personnel in-charge of that school shall declare re-reading of
forms in the school.
