Looking for input from students who left school in 2018-2019!


Every year, Indiana must send a special report to the U.S. Department of Education about what
students who had an IEP (Individualized Education Program) do after they leave high school. If
you were a student with an IEP and you left school in the 2018-2019 school year, the Indiana
Department of Education (IDOE) would like you to fill out the Post-School Outcomes Survey. (If
you are a parent or guardian of a student who left school in the 2018-2019 school year, you can
fill out the survey on behalf of your child.)

The survey will ask if former students had a job and earned money, attended college or
university classes, and participated in any type of work training program in the year after leaving
school. The Indiana Department of Education uses this information to improve school
programming and supports for future students with disabilities attending Indiana schools.

important. Please know that information shared in the survey will only be used by IDOE and the
Center on Community Living and Careers (the people distributing the survey) to create the post-
school outcomes report. It will not be shared with other organizations, and no individually
identifying information will be shared in the reporting of the data.

about work, further education, training programs, and other experiences you’ve had in the year
since you’ve left school. The Indiana Department of Education will use information from the
survey for a report to the federal government and to improve school transition programs
throughout the state. Your privacy is important. IDOE will not share your information with
organizations not involved with the survey or the report, and no individually identifying
information will be shared in the reporting of the data.
