Facilitating Factors During the Conduct Mass Deworming- 2nd Round:


 The coordinator, together with the adviser, provided consent to ensure the 100% approval of the
parents.
 The consent was gathered after parents had signed it.
 Students had eatern their breakfast before taking the medicine.
 Instructions and additional information were provided during the whole process

Hindering Factors During the Conduct of the Mass Deworming:


 Some of the students did not participate during the deworming due to their parents disapproval.
 Other students were afraid of taking the medicine

2. Checked and Received deworming tablets


3. Coordinated with the school principal as to the
re orientation to all teacher’s concerned
4. Re-orient all teachers and other school
personnel, PTA, stakeholders on the
implementation of school Mass Deworming
5. Distributed to all teacher advisers the
deworming tablet equal to the number of
pupils enrolled per class
6. Teacher advisers prepared deworming folders
which include all the deworming forms
required (eg. DF-1-CHECKLIST, DF-2
SUMMARY, DF-3 for 4PS’s Beneficiaries;
Parents’ consent and DepEd Memorandum
B. IMPLEMENTATION
The Class Adviser
1. Checked the health status of each
pupil/student prior to deworming
2. Ensured the pupils/ students have eaten
their full meal before deworming
3. Conducted hand washing and tooth
brushing activity
4. Distributed one tablet of Albendazole
/Mebendazole per pupils/students
5. Ensured that all pupils/students observed
the proper intake of the deworming tablet
C. POST IMPLEMENTATION
1. School QATAME coordinator had
answered the monitoring tool
2. Submitted hard or soft copy of mass
deworming report with the signature of
persons concerned on time
