AFFIDAVIT

I, A.K. Singh S/o R.N. Singh, R/o Flat No. 42/1 Old Wellington
Camp, Race Course, New Delhi -110003 do hereby solemnly affirm
and declare as under: -
1. That I am the father of my daughter Ms. Sristi Kumari, minor,
aged about 5 years hence being natural guardian competent to
swear this affidavit.

2. That I have changed the name of my daughter Ms. Sristi

Kumari as ‘Ms. Saubhagya Singh Prakarsh’. Hence, Ms. Sristi
Kumari and Ms. Saubhagya Singh Prakarsh is same person.

3. That as the name of Ms. Sristi Kumari has been changed as

such she would be called as ‘Ms. Saubhagya Singh Prakarsh’
since today for all purposes.

about 5 years hence being natural guardian competent to swear
this affidavit.

2. That I have changed the name of my son Mr. Samya Kumar as

Mr. Aayushman Singh Prakarsh. Hence, Samya Kumar and
Aayushman Singh Prakarsh is same person.

3. That as the name of Samya Kumar has been changed as such

she would be called as “Mr. Aayushman Singh Prakarsh” since
today for all purposes.
