Although we hope your employment with Saint Paul Public Schools will be a mutually
rewarding experience, we understand that varying circumstances do cause employees
to voluntarily resign employment. Should this time come, you are asked to follow the
guidelines below regarding notification and exit procedures.
Procedures
3. Employees who fail to report to work for three consecutive days without properly
communicating to their immediate supervisor the reasons for their absence will
be deemed to have voluntarily resigned their employment as of the third such
day.
5. Resigning employees will be scheduled for an exit meeting with their immediate
supervisor to ensure that all tools and equipment are returned, and to provide an
opportunity to discuss any questions or concerns related to employment with
Saint Paul Public Schools. Employees who fail to return any company property,
including keys, credit cards, tools, uniforms, cellular phones, pagers and other
equipment, will be deemed ineligible for rehire and may be subject to legal
proceedings on behalf of Saint Paul Public Schools. Upon completion of this exit
meeting, supervisors are expected to complete, sign and return the Supervisor
Checklist for Ending Employment Form to the Human Resource Department.

Please complete the following, sign and return to your immediate Supervisor who will sign and
forward immediately to the Human Resource Department. Please keep a copy for your
records.
I hereby resign from Saint Paul Public Schools, effective at the end of the day on
New forwarding address, effective date and phone number (if applicable)
The following items must be returned to your immediate supervisor or his or her designee by your last
day of work: Keys, Uniforms, Credit Cards, Computer, Cell Phone, Pagers, ID Badge, Resource
Books or Materials, Other equipment or property belonging to Saint Paul Public
Schools (e.g. Palm Pilots), Completed final timecard(s), Voicemail and/or computer
access codes.
Please complete and return an Exit Interview (on-line or by paper) to the Human Resource
Department. Please note that this is voluntary. The on-line form can be located at the following
address. http://hr.spps.org/exitinterview. Your comments are of vital importance in helping us
maintain a safe and respectful working environment.
Supervisor’s Signature
The purpose of this checklist is to assist supervisors with important steps to be completed when a
person’s employment with the District is ended. Please complete this form and return to the Human
Resource Department’s Compensation Unit.
INSTRUCTIONS:
Please ensure the following items are complete before the employee’s last day of employment:
Submit the employee’s written resignation to the Human Resource Department immediately
upon notification using the Resignation Form available on the HR Web site at:
http://hr.spps.org/forms. The employee’s forwarding address should be included on the form.
In the event that you are discharging an employee, please contact the Human Resource Department
to ensure proper procedures are followed.
Cancel or modify access to software applications, network access and email by contacting the
Director of Educational Technology at (651) 603-4925.
Deactivate voicemail and cancel long distance access by contacting Facility Planning at
(651) 767-3400.
Remove the employee from the authorized signature list(s), where applicable.
