I am enclosing herewith my duly filled PF and EPS withdrawal form 19 & 10C along with the cancelled
cheque and copy of bank pass book for withdrawing my deducted accumulation as per the scheme.

I have spent a lot of time in tracing my employer, but with no success i have not found any details of the
whereabouts of the employer. The employer has closed the local office for which i was employed.

I have been provided the PF slips during the employment which also attached herewith. An Notarized
Affidavit stating the discrepancies in name is also attached with withdrawal forms.

Note: Sir, I had earlier applied for the withdrawal but unable to recieve my amont. The withdrawal form
was sent by registered post and had been recieved by your office. The details of dispatch and recieving
are also enclosed for your ready reference.

Therefore i request you to kindly have your kind attention on the matter for the settlement of my
account.
