• Nieder reportou 3 casos de aumento da corticoterapia e
2 de radionecrose. A taxa de risco de radionecrose foi
estimada em 22% a um ano e 26% a 18 meses.
• Conclusão:
• O papel da dose
• Hudes: utilizou 2 doses por fracção e 3 doses totais em gliomas em
recidiva. O aumento da dose levava a um aumento da taxa de
resposta mas sem impacto na sobrevida. Nenhuma resposta
(24Gy/8 fr.); 79% de resposta com (30 ou 35 Gy/10 fr.)

• Toxicidade:
• O risco de complicações parece ser comandado pela
dose
• Shepherd administrou entre 4 e 10 frações de 5 Gy.
Encontrou que o risco de complicações era multiplicado
por 6,4 para uma dose maior que 40 Gy.
• Numa série de 22 doentes, Laing demonstrou que o
volume era também importante (43 para 22 cm3).

Forty-six patients with recurrent malignant astrocytoma were treated with stereotactic high

activity temporary I implants between September, 1986 and October, 1992. All patients had

been initially treated for malignant asctrocytoma (44) or low-
low-grade astrocytoma (2) with surgery
and external fractionated radiation. The median time between initial diagnosis and recurrence
treated with brachytherapy was 12.5 months. Twenty-

• : All but four patients have died; median survival time following brachytherapy is 46 weeks.
Twelve patients underwent reoperation for radiation necrosis at a median interval of 6.5 months
after treatment (26%). Five patients incurred complications directly due to brachytherapy (11%).
Forty--four patients are evaluable regarding pattern of failure following brachytherapy. Six of

these 44 patients (13.6%) recurred at a distance from the treatment volume (4 in brain and 2 in
spinal subarachnoid space).

recurrent malignant astrocytoma, but complications are significant, reoperation frequently
required, and recurrence outside the treatment volume common.

• To evaluate the impact of tumor volume on survival of patients reirradiated with 192Ir for
recurrent glioblastoma.
• Between 1993 and 1997, 42 patients with recurrent glioblastomas (29 males and 13 females,
age 18–
18–69 years, median age 49) were treated with 192Ir implantation. Previous treatments
included surgery, external beam radiotherapy, and chemotherapy. Maximum diameter of the
recurrent tumor was 1.2–
1.2–10.1 cm (median: 5.7 cm) and tumor volume was 1.6–1.6–122 cm3
(median: 23 cm3). Karnofsky performance status score was 50–
50–100 (median: 80). Brachytherapy
dose was 40–
40–60 Gy.
• Probability of overall survival was 80% at 6 months, 48% at 1 year, and 11% at 2 years.
Median survival was 50 weeks. Univariate analysis showed that both tumor volume (T < or T ≥
30 cm3) and Karnofsky performance status score were significant predictors of survival.
Multivariate analysis showed that smaller tumor volumes were associated with a higher
probability of survival (p < 0.001).
• Tumor volume less than 30 cm3 was associated with a higher probability of, and quality of,
survival than larger lesions for patients reirradiated by brachytherapy for recurrent glioblastoma.

• Purpose: In this study, we assess the efficacy of GliaSite brachytherapy in the treatment of patients with
recurrent glioblastoma multiforme (GBM).
• Methods and Materials: Between 1999 and 2004, 24 patients with recurrent glioblastoma multiforme were treated
with the GliaSite Radiation Therapy System (RTS). The GliaSite is an inflatable balloon catheter that is placed in
the resection cavity at the time of surgical resection. Low-dose-rate radiation is then delivered locally by
temporarily inflating the balloon with an aqueous solution of organically bound 125I (Iotrex [sodium 3-(125I)-iodo-
4-hydroxybenzenesulfonate]). Patients at the Johns Hopkins Hospital with recurrent GBM, who were previously
treated with surgery and external beam radiotherapy, underwent surgical resection followed by GliaSite balloon
implantation. Subsequently, the patients received radiation therapy using the GliaSite to a mean dose of 53.1 Gy.
Ten patients were male, and 14 patients were female. The mean age was 48.1 years. All patients had
pathologically confirmed recurrent GBM. The median Karnofsky performance status (KPS) was 80. Median follow-
up time was 21.8 months.
• Results: At the time of analysis, 18 patients (75%) had died; 6 patients (25%) were alive. Median survival from
diagnosis for all patients was 23.3 months. Median survival after GliaSite brachytherapy was 9.1 months. Patients
with a KPS ≥70 had a median survival of 9.3 months, whereas patients with a KPS <70 had a median survival of
3.1 months (p < 0.003). Survival was not significantly different between patients receiving 45 Gy and patients
receiving a dose greater than 45 Gy. Acute side effects were minor, consisting of mild nausea and/or headache.
One patient developed a wound infection. No incidents of meningitis were observed. Late sequelae were rare, but
2 incidents of symptomatic radiation necrosis were observed. One patient developed transient expressive aphasia.
• Conclusions: GliaSite radiotherapy confers a prolongation of survival in patients with recurrent glioblastoma
multiforme compared to historical controls with recurrent GBM. GliaSite therapy leads to a favorable survival
outcome of 9.3 months in patients with KPS ≥70, but only 3.1 months in patients with KPS <70. Favorable
survival is observed for patients within each recursive partitioning analysis class. Treatment with GliaSite is safe
and generally well tolerated. Additional data are needed to fully assess the therapeutic benefit of GliaSite
brachytherapy for recurrent GBM.

• Conclusão:
• Dum modo geral, a reirradiação encefálica total das
metástases cerebrais é possível e o risco de
complicações é pequeno durante a sobrevida dos
doentes.
• Factores de prognóstico:
• O controle da doença extracraneana foi um
factor de prognóstico em 2 dos estudos
• Características do tumor (ex. RE + em ca.
mama)
• Irradiação bifraccionada em condições
estereotáxicas foi um factor de prognóstico
favorável na sobrevida global comparativamente
à irradiação não fraccionada em condições
estereotáxicas com uma duração de sobrevida
duas vezes superior

• Complicações:
• Conclusão:
• A reirradiçao de doentes em condições estereotaxicas é
possível mesmo com um nº significativo de metástases
ou em episódios repetidos.
• Um bom estado geral do doente, um tempo longo até à
recidiva, um pequeno nº de metástases e uma doença
controlada são critérios que permitem optar por uma
reirradiação.
• Uma irradiação fraccionada parece ser interessante, mas
a ausência de estudos randomizados não permite
conclusões definitivas.
