PLACEMENT LEARNING CONTRACT/AGREEMENT

I. PURPOSE OF THE CONTRACT/AGREEMENT

This contract/agreement is entered into by the following parties involved in the Field Instruction Placement
Program of the Bachelor of Science in Social Work of the Dalubhasaan ng Lungsod ng Lucena. This agreement
provides important information and guide in the course of action of field instruction students.

B. Goals and development of specific skills

The Field Instruction subjects (FI 1 and FI 2) of the Dalubhasaan ng Lungsod ng Lucena is geared towards
the development of quality social work students prepared to possess the necessary competencies as generalists in
performing their tasks in working with individuals, groups, families and communities using the primary methods
of casework, groupwork and community organization and supplemented by the secondary methods of research,
administration and supervision. These assignments use theories for social work, social science and indigenous
knowledge in the Filipino context.

During their field practice, BSSW students are provided with the experiences to critically apply the theories
into practice, as they use their knowledge, abilities and skills in problem-solving and decision-making.

II. PERIOD

As stipulated in CMO 39, “Field instruction is an essential feature of the BSS curriculum. It requires
students to spend a minimum of 1,000 hours as required by Social Work Law, RA 4373. The FI is a supervised
field work in a social welfare institution and in a rural or urban community with required documentation of the
placement experience”

The current Field Instruction 1 will cover 500 hours of field placement in agencies and will commence on
July 29, 2019 to December 20, 2019. Affiliates are expected to complete the required hours while on agency and
school placement.

a. City Social Welfare and Development Office, Lucena City
b. Reception and Action Center for Children
c. Dalubhasaan ng Lungsod ng Lucena (Integrated Community Extension and Return Service Program,
Tertiary Education Subsidy Program, Gender and Development Program)

V. COMPENSATION AND BENEFITS

Affiliates will cover their own costs for field placement which are transportation from residence to agency.
However, the agency may provide transportation allowance for affiliates home visitation, attendance in seminars, and
the like.

The agency shall provide the affiliate students with a working space with table and chair, and as much as possible
access to computer.

Affiliates shall agree on the uniform they will wear while on placement/duty. They shall, at all times, wear their
pin or bring their school identification card.

Insurance (group insurance) is covered by the school (Dalubhasaan ng Lungsod ng Lucena).

They will report for duty on Mondays (8:00-12:00; 1:00-5:00) through Thursdays only. On individual cases and
depending on their schedule in the school (referring to those with subjects outside of field instruction being taken
during the semester, or for personal reasons), reporting days may vary accordingly. For affiliate assigned in the agency
they will observe the Public and special holidays as observed, but not school declared holidays. Affiliates assigned in
the school, will observe both public and special holiday and school declared holidays.

The Agreement as well as its terms and conditions herein shall be held in strict confidence and that
no portion or part hereof may be reproduced or be revealed to any personnel or entity other than the
signatories herein or their authorized representative
