WHEREAS, the province of Pangasinan, including the municipality of Bayambang, was placed
under Modified General Community Quarantine (MGCQ) in light of the COVID-19 pandemic;
WHEREAS, the National Task Force (NTF) for COVID-19 issued Memorandum No. 2, s. 2020
or “Operational Guidelines on the Application of the Zoning Containment Strategy in the
Localization of the National Action Plan against COVID-19 Response” on June 15, 2020;
WHEREAS, the Zoning Containment Strategy aims to “contain and prevent the spread of
COVID-19; protect the Filipino people; and mitigate the social, economic, and security impacts
of the COVID-19 pandemic crisis”;
WHEREAS, the rapid increase in the number of positive COVID-19 cases in the Municipality
of Bayambang calls for the immediate action of the Local Government Unit and concerned
agencies;
WHEREAS, it was observed that majority of the COVID-19 confirmed cases are Authorized
Persons Outside of Residence (APORs);
WHEREAS, the quarantine pass system allowed the Local Government Unit to effectively
enforce rules during the implementation of the guidelines under the Enhanced Community
Quarantine;
WHEREAS, the Philippine National Police – Pangasinan Provincial Office (PPO) issued a
memorandum on September 16, 2020 regarding the stricter implementation of IATF guidelines
and health protocols;
WHEREAS, there is a need to enforce and abide by policies that are necessary to ensure the
health and safety of the people;
1.1 Printing and distribution. The Local Government Unit of Bayambang, through the Office
of the Local Civil Registrar, is in charge of printing the quarantine passes while the Punong
Barangays and the barangay councils are in charge of distributing them to their constituents. The
names on the QPs shall be based on the list submitted by the Punong Barangays during the ECQ
period.
1.2 Coverage. The QPs shall be presented to any establishment upon entry. Starting on
September 21, 2020 (12nn), all establishments (grocery stores, banks, government offices, etc.)
shall strictly impose the “No Quarantine Pass, No Entry” policy.
a. QP for heads of households – One (1) QP shall be issued for each household. The
holder of the QP shall be allowed to purchase necessities and procure essential goods and
services for their household. These QPs shall be valid until September 30, 2020 or until its
validity is revoked by the Local Government Unit.
1.4 For residents of neighboring towns and municipalities. Residents of other towns shall be
required to present a QP issued by their barangay council or local government.
2.1 Requirements. Returning APORs must secure the following documents from their origin
town/city:
2.3 APORs with incomplete documents shall be required to undergo 14-day quarantine at the
Isolation Facility or return to their place of origin.
SECTION 3. Quarantine Control Points. All APORs, with the exception of drivers of
cargoes/trucks, are required to present a Quarantine Pass and Certificate of Employment or valid
company ID before being allowed to enter or pass through any of the thirteen (13) Quarantine
Control Points.
SECTION 4. Issuance of Travel Pass. Travel Passes shall be issued to ensure that quarantine
protocols are strictly implemented.
4.2 Town-to-town/city travel. Non-APORs traveling to other cities or municipalities within the
province must secure a Travel Pass signed/certified by the Municipal Mayor or the Municipal
Administrator with the specific destination and purpose. The individual must present a Barangay
Clearance and Medical Certificate to obtain a Travel Pass. Travel Passes may be issued for
essential or indispensable purposes only.
5.1 Entry of non-residents. Non-residents with no Quarantine Pass or Travel Pass, except cargo
and indispensable travelers, shall be denied entry.
5.2 Enforcement of public health guidelines. Barangay councils must ensure that the wearing
of face mask and face shield, physical distancing, and other public health and safety guidelines
are followed by their constituents.
5.3 Strictly no loitering. The “No Loitering” policy must be strictly enforced for non-APORs.
5.4 Curfew. The 8PM-5AM curfew for non-APORs must be strictly enforced by the barangay
councils, with the help of PNP-Bayambang and POSO.
c. Those who are suffering from cardio-vascular diseases, cancer survivors, and senior
citizens (60 years old and above), and
6.2 For LGU offices. Employees of LGU Bayambang who fall under the conditions listed on
Section 6.1 must submit a Medical Certificate signed by Local Health Officers of the LGU,
along with a letter duly noted by the department head signifying that the employee will adopt the
work-from-home scheme, to the Human Resources and Management Officer.
6.3 All offices and establishments are mandated to implement the guidelines of the Department
of Trade and Industry and the Department of Labor and Employment with regard to public health
standards.
SECTION 7. Strict enforcement of liquor ban. The Business Processing and Licensing Office,
PNP-Bayambang, and POSO shall conduct inspection on stores and establishments around the
municipality to ensure that Section 3 of Executive Order No. 45, s. 2020 or the liquor ban is
strictly enforced. In case it is proven that a store is not following the law, all liquors on display
shall be confiscated.
8.1 All barangays placed on lockdown are not allowed to organize any kind of social gathering.
SECTION 9. Separability Clause. If any provision of the Executive Order is declared invalid
or unconstitutional, the other provisions not affected thereby shall remain valid and subsisting.
SECTION 10. Repealing Clause. All other orders, rules, regulations and issuances, or parts
thereof, which are inconsistent with this Order are hereby repealed, amended or modified
accordingly.
SECTION 11. Effectivity Clause. This Executive Order shall take effect immediately and shall
remain in effect until September 30, 2020 or until sooner revoked or amended.
Let copies of this Order be furnished to all offices, departments, and barangays for their
information and appropriate action.
