Comound, Malanggam Bulihan, City of Malolos, Bulacan, under oath, depose and say:

1. That I am duly authorized representative being the president of YOOMART

ENTERPRISES INCORPORATED. With place if business address at
Gatchalian Compound, Sitio Malanggam, Brgy. Bulihan, City of Malolos,
Bulacan with TIN: 237-878-449-000.

2. That YOOMART ENTERPRISE INCORPORATED is a corporation duly

registered with the BUREAU OF INTERNAL REVENUE with Certificate of
Registration NO-OCN-4R0000252595 dated May 05, 2005.

3. That on August 06, 2012 Original Certificate of Registration including other

documents were lost/missing due to flood by a monsoon rain (HABAGAT).

4. That, I am requesting your good office yo furnish us a copy of our

CERTIFICATE OF REGISTRATION.

5. That I am executing this affidavit to vouch for the truth of the foregoing and
for all other intents and purposes that it may serve.
