13 AREAS OF ASSESSMENT

1. SOCIAL STATUS

 Patient X is a 12 year old male. He lives in , Quezon City. He was born on _________ and was
baptized under the Roman Catholic religion. He is a Filipino citizen and speaks Tagalog dialect and
could speak and understand Ilocano language. His province is in Isabela.
He lives with his sisters. He spend his free time hanging out with his friends.

2. MENTAL STATUS

 The patient was conscious and coherent during the interview and assessment. He was also able to tell
us the current date which shows that he is oriented. He is currently in grade 6 and can speak and
comprehend English.

3. EMOTIONAL STATUS

 The patient was very cooperative during the interview and gives us appropriate answers to our
questions. About his health condition, he already accepted that his illness but he is still anxious his
condition and what will happen to him he does not recover from it.

4. SENSORY PERCEPTION

A. Vision
 Patient X was able to read a printed material that has letters in large and small font which was about
3 meters or 7 feet away from him. He was also able to read written words on a paper within a near
distance. The extraoccular movement is positive.

B. Smelling
 Patient X could smell because he got a nose pack because he was currently having epistaxis.

C. Tasting
 He was able to tell the taste of his biscuit,which is sweet. He was also able to tell that his
viand,Sinigang, is Sour.

D. Hearing
 The patient has no difficulty in hearing. He can hear the loud noise produced by all the voices of his
visitors. He can even hear the soft sound of a ticking wrist watch when we placed it near his ear.

E. Touching
 The patient was able to identify which is sharp and which is not between the tip of the pen and the
dull part of the pen. He was able to identify which is the soft(cotton) and which is the hard(pen). He
could also identify a wet cotton and a dry one. All of these were done with the patient's eyes closed.

F. Speech
 The patient can speak clearly and utter words without any difficulty.

5. MOTOR ABILITY

 The patient is ambulatory but cannot do much of the activities of daily living because he was
suppose to rest. When it comes to his Range of Motion(ROM)...

6.BODY TEMPERATURE

 The patient's body temperature during the assessment was....


7. RESPIRATORY STATUS

 During the assessment,the patient's respiratory rate is cpm. He also produce noisy breathing sound
and expiratory wheezing sound.

8. CIRCULATORY STATUS

 His cardiac rate was bpm. He has a blood pressure of mmHg. His capillary refill last from one to
three seconds.

9. NUTRITIONAL STATUS

 The patient has a fair nutritional status. His teeth are complete. The patient has a weight of kg. and a
height of____ and his BMI is____.

10. ELIMINATION STATUS

 Patient X has an irregular bowel movement, having an interval of every.... His stool color is.... His
urine color is yellow and is releasing more or less ....ml of urine. He urinates.... times a day.

11. REPRODUCTIVE STATUS

 According to the patient he was circumcised when he was in grade thre and never had any sexual
activity.

12. STATE OF PHYSICAL REST AND COMFORT

 Patient X always has 8 hours of sleep or more before he was hospitalized. He still gets enough sleep
until now that he is in the hospital.
