1. Introduction
This manual describes solutions for problems that may occur
during operation of the Aquilion ONE system.
Problems are sorted into various categories. Find the section
that applies to your problem, and take the actions described in
that section.
Contact your Toshiba service representative if you are unable to
solve a problem despite taking the actions described in this
manual, or if the problem you have is not covered in this manual.
In some cases, when an error message is displayed, a
combination of the [DETAIL] button and the [XC RESET] or
[OK] button is displayed in the error message window. In such
cases, click the [DETAIL] button first. The contents of the error
are logged in the system and the error details are displayed.
Then, press the [XC RESET] or [OK] button.

2. Procedures for [Reset Scan System] and [Reset System]


The solutions instruct that [Reset Scan System] or [Reset
System] is to be performed in some cases. The procedures for
them are as follows.

(1) Move the mouse cursor to the upper left of the monitor
screen and leave it for 5 seconds or longer.

(2) The following window surrounded by a white border is


displayed in the center part of the monitor.

(3) Hover the mouse cursor (x) over [Reset Scan System] or
[Reset System] and wait for a few seconds. The reset
function should start.

3. Times required for system shutdown, reboot, and reset


The times required for system shutdown, reboot, and reset are
as follows.

Shutdown about 2 minutes


Reboot about 5 minutes and 50 seconds
Reset Scan System about 4 minutes and 40 seconds
(scan monitor side)
about 1 minute and 40 seconds
(image processing monitor side)
Reset System about 4 minutes and 40 seconds
(scan monitor side)
about 2 minutes and 30 seconds
(image processing monitor side)

4. Operating procedures for CT system shutdown


[Reset Scan System] and [Reset System] cannot be performed
in some cases. In such cases, follow the procedures below.

(1) Press the [Ctrl] and [Esc] keys on the keyboard


simultaneously to display the Windows menu.

(2) Select [CT System Shutdown]. The CT system software is


automatically terminated. Log off from Windows.

(3) Shut down Windows.

1. An error message is displayed during start-up of the


system.

The warning message "More than 80% of the image data disk
is filled." is displayed.
Cause
There is insufficient disk space for the
image files. This may cause scanning
and image registration to be interrupted
if there is insufficient disk space.

Solution 1 Click the [OK] button on the error message. Ensure there
is enough disk space for the image files before starting
scanning.

Communication error warning window is displayed.


Cause
A communication error has occurred
between the scanning monitor and the
image processing monitor.
Alternatively, the scanning monitor is
not started (including the case that
image data base recovery is being
executed at the scanning monitor).

Solution 1 Select [YES] on the warning window. Communication


setup is retried.

Solution 2 If due to an emergency you are unable to stop scanning,


select [NO] on the warning window. Operation is enabled
only for the system for the scanning monitor.

The error message "RTM could not run." is displayed.


Cause
The reconstruction system has not started up normally.

Solution 1 Shut down the system and turn the power of the console
OFF. Then reboot it after 1 to 2 minutes have passed.
Start operation from the patient registration after
rebooting.

The error message "Error in xSYS." is displayed.


Cause
A file system error has occurred.

Solution 1 Shut down the system and turn the power of the console
OFF. Then reboot it after 1 to 2 minutes have passed.
Start operation from the patient registration after
rebooting.

2. A problem occurs during scanning.


The scan range cannot be extended.
Cause
The tube current or the maximum tube current for Sure Exposure 3D/Sure Exposure is
extremely high.

Solution 1 Reduce the tube current or the maximum tube current for
Sure Exposure 3D/Sure Exposure.

An additional scan cannot be performed.


Cause
Since Sure Exposure 3D/Sure Exposure is set, scanning cannot be performed for the
range for which scanoscopy has not been performed.

Solution 1 Change the tube current to a constant value or reduce the


scan range.

The error message "Error in xxx movement. Call service." is


displayed.
Cause
The movement of the unit specified in
the error message is not completed
normally.

Solution 1 Click the [DETAIL] button on the error message and click
the [OK] button. A hard reset occurs. Then click the
[Confirm] button and perform scanning again.

Solution 2 If the problem is not solved by solution 1, shut down the


system. Then turn the power of the system OFF and
reboot it. Start operation from the patient registration after
rebooting.

The error message "Couch movement interrupted." is


displayed.
Cause
The couch top does not move to the
end of the target position.

Solution 1 Check the couch top position. If there is no problem,


perform scanning again using "Additional scan".

Solution 2 If the couch top position is obviously abnormal, shut down


the system. Then turn the power of the system OFF and
reboot it. Start operation from the patient registration after
rebooting.

The error message "Error in xxx (Time Out). xxxx" is


displayed.
Cause
The movement is not completed within
the time set by the system.

Solution 1 Click the [DETAIL] button on the error message, and click
the [OK] button. Then click the [Confirm] button, and start
scanning again.

Solution 2 If the problem is not solved by solution 1, shut down the


system. Then turn the power of the system OFF and
reboot it. Start operation from the patient registration after
rebooting.

The error message "Error in xxx condition. Set them again." is


displayed.
Cause
Scanning parameter setting has not
been completed properly.

Solution 1 Click the [DETAIL] button on the error message, and click
the [OK] button. Then click the [Confirm] button, and
perform scanning again.

Solution 2 If the problem is not solved by solution 1, shut down the


system. Then turn the power of the system OFF and
reboot it. Start operation from the patient registration after
rebooting.

The error message "xxx XC xxx" or "Over kV (or mA)." is


displayed.
Cause
An X-ray controller error occurred.

Solution 1 Click the [DETAIL] button on the error message and click
the [XC RESET] button. A hard reset occurs. Then click
the [Confirm] button, and perform scanning again.

Solution 2 If the problem is not solved by solution 1, shut down the


system. Then turn the power of the system OFF and
reboot it. Start operation from the patient registration after
rebooting.

The error message "Error in GTS. Call service." is displayed.


Cause
A communication error between the
console and the gantry, or a
communication system error in the
gantry occurred.

Solution 1 Click the [DETAIL] button on the error message, and click
the [OK] button. Then click the [Confirm] button, and
perform scanning again.

Solution 2 If the problem is not solved by solution 1, shut down the


system. Then turn the power of the system OFF and
reboot it. Start operation from the patient registration after
rebooting.

The error message "Error in offset data Acquisition. Retry


Acquisition." is displayed.
Cause
An error occurred during the offset data
acquisition before scanning.

Solution 1 Click the [DETAIL] button on the error message, and click
the [OK] button. Then click the [Confirm] button, and
perform scanning again.

Solution 2 If the problem is not solved by solution 1, shut down the


system. Then turn the power of the system OFF and
reboot it. Start operation from the patient registration after
rebooting.

The error message "An abnormality has occurred in data


transfer system (*). xxxx" is displayed.
Cause
An error occurred at the data transfer
system in the gantry.

Solution 1 Click the [DETAIL] button on the error message and click
the [ERROR CLEAR] button. A hard reset occurs. Then
click the [OK] button, and perform scanning again.

Solution 2 If the problem is not solved by solution 1, shut down the


system. Then turn the power of the system OFF and
reboot it. Start operation from the patient registration after
rebooting.

The error message "Error in pre-process. xxxx" is displayed.


Cause
An error occurred at the pre-processing
section for the acquisition data in the
console.

Solution 1 Move the cursor to the upper left of the monitor and leave
it there for a few seconds. When the window appears at
the center of the monitor, move the cursor to [Reset Scan
System] and leave it there for a few seconds. The system
is rebooted. Start operation from the patient registration
after rebooting.

Solution 2 If the problem is not solved by solution 1, shut down the


system. Then turn the power of the system OFF and
reboot it. Start operation from the patient registration after
rebooting.

The error message "Error occurred in Scan System. xxxx" is


displayed.
Cause
An error occurred at the scan control
section in the console.

Solution 1 Click the [OK] button on the error message. Then click
the [Confirm] button, and perform scanning again.

Solution 2 Shut down the system, and turn the power OFF. Then
reboot it. Start operation from the patient registration after
rebooting.

The error message "Error in recon-process. xxxx" is


displayed.
Cause
An error occurred at the reconstruction
processing section in the console.

Solution 1 Move the cursor to the upper left of the monitor and leave
it there for a few seconds. When the window appears at
the center of the monitor, move the cursor to [Reset Scan
System] and leave it there for a few seconds. The system
is rebooted. Retry the raw data reconstruction after
rebooting.

The error message "Error in RDD. xxxx" is displayed.


Cause
An error occurred at the control section
of the raw data disk in the console.

Solution 1 Move the cursor to the upper left of the monitor and leave
it there for a few seconds. When the window appears at
the center of the monitor, move the cursor to [Reset Scan
System] and leave it there for a few seconds. The system
is rebooted. Start operation from the patient registration
after rebooting.
In addition, this problem may be caused by an error in the
raw data saving area. Therefore, contact your Toshiba
service representative.

3. An error occurred during the image reconstruction.


The error message "Error in recon-process. xxxx" is
displayed.
Cause
An error occurred in the reconstruction
processing section in the console.

Solution 1 Move the cursor to the upper left of the monitor and leave
it there for a few seconds. When the window appears at
the center of the monitor, move the cursor to [Reset Scan
System] and leave it there for a few seconds. The system
is rebooted. Retry the raw data reconstruction after
rebooting.

The error message "Error in RDD. xxxx" is displayed.


Cause
An error occurred in the control section
of the raw data disk in the console.

Solution 1 Move the cursor to the upper left of the monitor and leave
it there for a few seconds. When the window appears at
the center of the monitor, move the cursor to [Reset Scan
System] and leave it there for a few seconds. The system
is rebooted. Start operation from the patient registration
after rebooting.
In addition, this problem may be caused by an error in the
raw data saving area. Therefore, contact your Toshiba
service representative.

4. An error occurred during operation of the image file.


The error message "Failed to xxx studies. xxxx" is displayed.
Cause
An error occurred in the image file system during operation.

Solution 1 Try the operation again.

Solution 2 If the problem is not solved by solution 1, move the cursor


to the upper left of the monitor and leave it there for a few
seconds. When the window appears at the center of the
monitor, move the cursor to [Reset System] and leave it
there for a few seconds. The system is rebooted.

Solution 3 If the problem is not solved by solution 1 or 2, shut down


the system and turn the power OFF. Then reboot it.

The error message "The registration area of background job is


full." is displayed.
Cause
The background job queue overflows. Additional processing requests are disabled.

Solution 1 This error causes the transfer queue to stop. Click the
image transfer button in the command box, and delete the
job in the image transfer queue. Try the image transfer
again after the image transfer queue has been deleted.

The error message "Failed to xxx Aquilion DVD." is displayed.


Cause
An error occurred during DVD processing.

Solution 1 Check the media and the drive, and try the process again.

Solution 2 If the problem is not solved by solution 1, move the cursor


to the upper left of the monitor and leave it there for a few
seconds. When the window appears at the center of the
monitor, move the cursor to [Reset System] and leave it
there for a few seconds. The system is rebooted.

Solution 3 If the problem is not solved by solution 1 or 2, shut down


the system and turn the power OFF. Then reboot it.

The error message "Error in xSYS." is displayed.


Cause
An error occurred in the file system.

Solution 1 Move the cursor to the upper left of the monitor and leave
it there for a few seconds. When the window appears at
the center of the monitor, move the cursor to [Reset
System] and leave it there for a few seconds. The system
is rebooted.

An image is not displayed.


Cause
Image ID is abnormal.

Solution 1 Retry reconstruction processing of the raw data.


For the detailed procedure, refer to section 7 "Image
Reconstruction" in the Scanning volume.

5. An abnormality occurs during image processing.


The CT numbers calculated using the Measure function are
displayed incorrectly. ("M: ***" is displayed etc.)
Cause
The setting for the target CT number range in the setting window displayed by clicking the
[Pref] button is set to ON.

Solution 1 Set the setting for the target CT number range to OFF.

6. The error occurred during filming.


The error message "Cannot get filed data." or "Generate
image error." is displayed.
Cause
The image data cannot be registered on the virtual film.

Solution 1 Read the data and create the sheet again.

7. Turning the power ON after the system was


terminated abnormally and the power was turned OFF.

The gantry movement is stopped by the emergency stop


button.
Cause
The gantry movement stopped by the emergency stop button without going through the
proper termination process.

Solution 1 Perform the system shut down process and turn the power
of the Navibox OFF. Wait for a few seconds, and then
turn the power of the Navibox ON.

The power of the Navibox is suddenly turned OFF.


Cause
The Navibox is suddenly turned OFF without going through the proper termination
process.

Solution 1 Wait for a few seconds and turn the power ON. Recovery
by the software is performed. Wait for the completion of
recovery.
* Turn ON the power of the system within 72 hours.
Otherwise the image data may be lost.

The switch of the distribution board is turned OFF.


Cause
The switch of the distribution board is suddenly turned OFF without going through the
proper termination process.

Solution 1 The solution differs depending on when it was turned


OFF. Contact your Toshiba service representative.
Be careful of the following when the power is turned ON.
• Confirming the air conditioner
To avoid problems with the system due to a rise in the
temperature of the scanning room, ensure that the air
conditioner is running.
• Warming-up of the X-ray detector
When the power of the system remains OFF for a long
time, a period of approximately 2 hours is required for
stabilization of the X-ray detectors after the power is
turned ON. Do not perform scanning during this period.
Perform scanning for the TOS phantom before
examination and check that the image has no artifacts.
For the test procedure, refer to subsection 3.1 "Pre-
operation check" in the Basic volume.
• Recovery processing time
Recovery processing by the software is performed.
Wait until recovery processing is completed. Recovery
processing may take up to 1 hour. If the system is used
to examine emergency patients, another diagnostic
modality should be available.
• Turn ON the power of the system within 72 hours.
Otherwise the image data may be lost.

The system stopped due to a power interruption.


Cause
The power of the system is shut down without going through the proper termination
process.

Solution 1 There is no backup power supply unit attached to the CT


scan system. Therefore, the situation is the same as "The
switch of the distribution board is turned OFF".
First, turn the power of the console OFF.
Then turn the power of the Navibox ON after the power
supply resumes. Recovery is performed.
Be careful of the following when the power is turned ON.

• Confirming the air conditioner


To avoid problems with the system due to a rise in the
temperature of the scanning room, ensure that the air
conditioner is running.
• Warming-up of the X-ray detector
When the power of the system remains OFF for a long
time, a period of approximately 2 hours is required for
stabilization of the X-ray detectors after the power is
turned ON. Do not perform scanning during this period.
Perform scanning for the TOS phantom before
examination and check that the image has no artifacts.
For the test procedure, refer to subsection 3.1 "Pre-
operation check" in the Basic volume.
• Recovery processing time
Recovery processing by the software is performed.
Wait until recovery processing is completed. Recovery
processing may take up to 1 hour. If the system is used
to examine emergency patients, another diagnostic
modality should be available.
• Turn ON the power of the system within 72 hours.
Otherwise the image data may be lost.

8. Other problems
A faint ring artifact appears on the screen.
Cause
The calibration data needs to be updated.

Solution 1 Perform the calibration data (air calibration data)


acquisition procedures described in the "Power ON
Procedures" in the operation manual.

Liquids enter the inside of the system.


Cause
Parts such as the PWBs are exposed to liquids.

Solution 1 The CT scan system is not waterproof. Perform the


termination process immediately and turn the power of the
system OFF. Then contact your Toshiba service
representative.

Mouse operation is disabled.


Cause
The mouse is not recognized.

Solution 1 Press the [Ctrl] + [Esc] keys at the same time. Select [CT
System Shutdown] from the Windows menu. The shut
down process starts.

Solution 2 If the above method cannot be performed, turn the power


of the Navibox OFF. Wait for a few seconds, and then
turn the power of the Navibox ON. Recovery by the
software is performed. Wait for the recovery to complete.

Keyboard operation is disabled.


Cause
The keyboard is not recognized.

Solution 1 Perform the shut down process.

Solution 2 If the above method cannot be performed, turn the power


of the Navibox OFF. Wait for a few seconds, and then
turn the power of the Navibox ON. Recovery by the
software is performed. Wait for the recovery to complete.

The error message "Error in Keyboard. Call service." or "Error


in communication between Keyboard and SS." is displayed.
Cause
Communication between the keyboard
and the console is not normal.

Solution 1 Perform the shut down process.

Solution 2 If the above method cannot be performed, turn the power


of the Navibox OFF. Wait for a few seconds, and then
turn the power of the Navibox ON. Recovery by the
software is performed. Wait for the recovery to complete.

The gantry external projector does not light.


Cause
The gantry is tilted.

Solution 1 When an attempt is made to return the gantry tilt to 0°


using the footswitch, the gantry may be still tilted to 0.5° if
the footswitch is released prematurely. Confirm that the
gantry is not tilted.

Couch vertical movement stopped temporarily and started


moving only in low speed.
Cause
When an error occurs in the couch sensor system, the couch height display indicates
"----- mm" and the message "The couch height information is abnormal. Contact to your
maintenance person immediately. The couch vertical movement can only be operated
from the gantry control panel in low speed. Remote operation is disabled." is displayed.

Solution 1 Use the following procedure to correct the problem.

1. Move the couch top up until it can be moved into the


gantry dome.
2. Insert the couch top into the gantry dome.
3. Lower the couch until it stops.
4. Check that the height display on the gantry control
panel is [ 000] mm.
5. The error is corrected when [ 000] mm is displayed.
6. If [----] mm is displayed, the error has not been
corrected. Contact your service engineer.

Gantry movement and couch movement are restricted as


follows. However, scanning can be performed normally.

• The couch vertical movement is performed from the


gantry operation panel in low speed.
• The couch vertical movement cannot be performed
from the interactive window.
• Gantry tilting can only be performed in the 0° direction.

Solution 2 If the problem cannot be corrected by performing the


procedure described in solution 1, contact your service
engineer.

Gantry tilt, couch horizontal movement, or couch vertical


movement stopped and the position indicator on the control
panel blinked.
Cause
Movement is disabled due to a movement error (indicated by blinking on the control
panel) or stoppage at the interlock position.

Solution 1 If movement stopped at the interlock, movement in the


reverse direction is still enabled. Move the gantry (or the
couch) in the reverse direction to release the interlock.

Solution 2 If the problem is not solved by solution 1, shut down the


system. Then turn the power of the system OFF and
reboot it. Start operation from the patient registration after
rebooting.

The error message "Error in Image data disk (power source).


Call service." is displayed.
Cause
A power source of the image data disk
drive malfunctioned.

Solution 1 The system can be used in this status. If another power


source of the image data disk drive malfunctions, a
system failure will occur. Contact your Toshiba service
representative.

malfunctioned or is in discharge status.

Solution 1 The system can be used in this status. If another backup


battery for cache memory in the image data disk drive
malfunctions or is in discharge status, the cache memory
will be nonfunctional and a system failure will result.
Contact your Toshiba service representative.

The error message "Error in Image data disk (FAN). Call


service." is displayed.
Cause
A cooling fan in the image data disk
drive malfunctioned.

Solution 1 The system can be used in this status. If another cooling


fan in the image data disk drive malfunctions, the power of
the Image data disk drive is turned OFF after the data on
the cache memory is written to HDD and a system failure
results. Contact your Toshiba service representative.

The error message "Error in raw data storage (RDD)." is


displayed.
Cause
One of the HDDs in the raw data disk
drive malfunctioned.

Solution 1 The system can be used in this status. If another HDD in


the same raw data disk drive malfunctions, raw data
cannot be written or read. Therefore,
scanning/reconstruction cannot be performed and a
system failure will occur. Contact your Toshiba service
representative.
