NOTICE OF PARTIAL APPEAL WITH MEMORANDUM OF PARTIAL APPEAL

COME NOW Complainant-Appellant, by undersigned counsel,

respectfully serves notice on this Honorable Office that it is appealing the
Labor Arbiter Decision dated April 7, 2015, copy of which was received on
April 10, 2015, on the ground of error in law and of serious errors in the
findings of facts, which, if not corrected, would cause grave or irreparable
damage or injury to the Respondents-Appellees.

The appeal is partially made insofar as the finding that Complainant-

Appellant was legally dismissed and that he is not entitled to damages and
attorney’s fees. The filing of this Notice is accompanied by a proof of
payment of the appeal fee hereto attached, a certificate of non-forum
shopping and an attached Memorandum of Appeal (Annex “A”).

Prayer

WHEREFORE, it is respectfully prayed of this Honorable Court that

this Notice of Appeal be duly NOTED; and, accordingly, that the Appeal be
GIVEN due course.

Complainant-Appellant prays for other equitable reliefs.
