MODULE 1 LESSON 1 LEARNING TASK

Health Belief Model was one of the behaviour change models to explain human health decision-making
and subsequent behaviour. This theory was modified to help predict whether people will take action to
prevent, screen for, and control illness. On the case of Charlene, she chose traditional birthing on her
first pregnancy which ended up to a stillbirth, considering the fact that this type of birthing is risky to the
both of her and her baby and she actually never had a prenatal consultation. And we can also consider
the fact that probably she has no idea about the risks of this type of birthing since this is her first
pregnancy. Meanwhile, now on her second pregnancy with her 3 month old pregnancy, she and her
husband wants to have a healthy baby, so they chose to have an early consultation. With this, they have
a strong belief of a positive outcome through adopting this kind of medical procedure, and this is the
concept of Health Belief Model. And it states that this model suggests that a person's belief in a personal
threat of an illness or disease together with a person's belief in the effectiveness of the recommended
health behaviour or action will predict the likelihood the person will adopt the behaviour. Their choice
of adopting this medical assistant will help them achieve their expected outcome, and this is the Health
Promotion Model of Nola Pender's theory wherein this model focuses on helping people achieve higher
levels of well-being and encourages clients to choose medical assistants from trained healthcare
providers.

If I were to choose a field and career in community health nursing, I were personally want to engage my
nursing skill in a maternal clinic. Because some rural areas have a significant increase of pregnancy rate
but low number of maternal clinics and nurses. Nurses in this area focuses mainly specifically on working
with new mothers and their infants. Some of their roles will include doing pregnancy tests, running
maternity clinics, coordinating care, providing early intervention for at-risk children, running
immunization programs, supporting teen mothers and promoting breastfeeding. A large part of their
role will focus on health education and referrals. And as a future nurse, it is one of my responsibility to
render my care and alleviate the risk and mortality rate for both mother and infant with the principle of
identifying needs and planning based on the needs of the certain community.

MODULE 1 LESSON 1 ASSESSMENT TASK

Interdisciplinary collaboration is defined as a complex phenomenon that is often formed between two
or more people from various professional fields to achieve common goals (Houldin, Naylor, & Haller,
2004).

Inter-disciplinary collaboration works across authorities and organisations or between different teams
within an organisation. Example of collaboration that a community has, is the collaboration of the
community with health care delivery system. They are the network of healthcare facilities and
healthcare professionals that carries out in rendering healthcare to the people in the community.
Example is the Barangay Health Centers or Rural Health Care Centers in every community. It is a
community-based and patient-directed organizations that deliver comprehensive, culturally competent,
high-quality primary health care services. Together with the Philippine Health Insurance Corporation
(PhilHealth), this social insurance program provides a means for the healthy to pay for the care of the
sick and for those who can afford medical care to subsidize those who cannot.

LESSON 3 LEARNING TASK

Understanding a certain community is crucial to help you to work effectively and efficiently. Failing to
understand the community you are working with will be difficult for you and to the community to
connect especially in communicating and implementing the tasks and intervention you are to render for
them. Before proceeding to the tasks, as a public health nurse, first you must have an assessment to the
community, therefore, start by finding out as much about the community you are working with as you
can, it's physical and geographical characteristics, its culture, its government, and its assumptions. By
combing through existing data, observing, and learning from community members, you can gain an
overview of the community that will serve you well. Recording your findings and your analysis of them in
a community description that you can refer to and update as needed will keep your understanding fresh
and help others in your organization or with whom you collaborate. After gathering all your needed
informations, you can now determine the tasks and interventions you are to give to the community.

It is one of the role of a public health nurses to provide a critical linkage between epidemiological data
and clinical understanding of health and illness as it is experienced in peoples’ lives. And through this, it
is translated into action for the public good. The surveillance and monitoring of disease trends within
the community is the illustration of this role. Emerging patterns that potentially threaten the public’s
health are identified and appropriate interventions planned, coordinated and implemented. This is a
role that public health nurses can do in any setting; however, it occurs mainly in the community public
sector. They are essential in contributing to systems for monitoring crucial health status indicators such
as environmentally caused illnesses, immunization levels, infant mortality rates, and communicable
disease occurrence, in order to identify problems that threaten the public’s health and develop effective
interventions..
