40. ALL PARTIES EXECUTING CLOSING DOCUMENTS MUST SUBMIT PHOTO
IDENTIFICATION AT CLOSING.

41. The appropriate Richmond County cover sheet (if applicable) must be filled out and
attached to each of the closing instruments to be recorded.

42. NOTE: IF THE PREMISES IS AFFECTED BY A NYC WATER METER:


Where no final reading is requested, DEP will hold the new owner responsible for the
entire bill even if the service period preceded his ownership. Since open charges are
liens against the property, rather than personal liens, the charges will remain on that
property's account unless the entire bill is paid.

43. NOTE: If the proposed transaction is based upon a POWER OF ATTORNEY, the
power of attorney and affidavit of attorney in fact must be furnished for review PRIOR
TO CLOSING. The Company must be furnished acceptable proof that the power is in
effect and is exercised while the principal is alive, that the power remains in effect and
that the power of attorney has not been revoked at the time of execution, delivery and
recording of the deed. At that time, the Company may make additional requirements or
exceptions.

44. NOTE: If a Mortgage is to be recorded in the proposed transaction, under certain


circumstances, the City of New York aggregates the principal amounts secured by
separate mortgages to determine the applicable rate of mortgage recording tax. The
Company must be advised if the mortgagor in this transaction or a "related"
mortgagor (including the person or entity who will execute the mortgage) has within the

PROPERTIES LOCATED IN THE COUNTIES OF NEW YORK, BRONX, KINGS
AND QUEENS.
1) NYC Real Property Transfer Tax [RPTT] Form
2) Affidavit of Compliance with Smoke Detector Requirement
3) Property Owners' Registration Form
4) Customers Registration Form for Water and Sewer Billing
5) NYS Real Estate Transfer Report [RP-5217NYC]
Unless the sellers and purchasers provide this Company authorization to prepare,
complete and submit such E-Forms to the City Register (as set forth in the attached
Authorization and Release form), Company will have no liability and the title policy
will not cover any harm, loss or damage which the parties may incur because of or
arising from the City's rejection of, or refusal to record, the transfer documents.
NO CLOSING WILL BE COMPLETE AND NO CLOSER IS TO LEAVE A
CLOSING WITHOUT FINAL SIGNED TRANSFER FORMS.

As of September 23, 2013, ACRIS will support the transfer of eTax and Cover Page
transactions from one ACRIS Customer Profile to another.
