obtaining Work Permit

3.3.1 Load materials properly by placing loads towards the middle
of dump bed
3.4.1 Maintain a safe distance from swinging radius
3.4.2 Operator to always look out for people and equipment within
work area.
3.4.3 Swing bucket slowly
3.4.4 When swinging bucket engage the reverse alarm and use a
Banks man
3.4.1 Use appropriate PPE.
4.1.1 Use trained / experienced drivers
4.2.1 Observe site speed limit (10km/hr.)
4.3.1 Follow safe driving techniques and be alert for other
equipment, personnel and passers-by.
4.4.1 Approach blind spots at slower speed and follow directions of
a banks man.
4.5.1 Work only during favorable weather conditions.
5.1.1 Inspect unloading site prior to dump.
5.2.2 Follow banks man’s instructions when in reverse motion.
5.3.1 Wear appropriate PPE
6.1.1 Be alert for other equipment, personnel and passers-by.

when in reverse motion.
