UNDERTAKING

I, JANETH B. ALVAREZ, of legal age,single and a resident of San Migue;

Street, Libon, Albay, after having been sworn to in accordance with law, depose
and state:

1. That I am appointed and assigned as Public Attorney I in Public Attorney's

Office Pandan, Catanduanes District Office, PandN, Catanduanes;

2. That upon admission to the BAR, I was employed with the PAo from
September 03, 2018 up to present;

3. That I undertake to remain in the same office for atleast one (1) year from
admission to the BAR, as required by the Rules on Community Legal Aid
Service (Sec. 8 (b), which I am required to render as a new BAR passer;

4. That I am executing this affidavit to attest the foregoing facts and

circumstances.
