Practicum Log

For this practicum, you are required to complete a log of your hours. Please note you are not expected

to have something complete for each week. You are expected to have completed 135 hours to engage

in the direct care experience to satisfactorily complete this course. This plan should be shared with your

mentor and agreed upon prior to submitting at mid-point and final to course faculty.

Acceptable hours include any interactions with your mentor, meetings with the faculty, any course work

(i.e. implementing the QI project, collecting data, working on the Project Charter, writing the final

summary paper, etc.)

to previous months. (8 hours)

technicians for second cycle. (Minimum of five on dayshift and five
on evening shift). Planning for 24 hours for one week (3 hours each
