MEMORANDUM OF AGREEMENT

This Agreement made and entered into this 14th day of September 2020 at Taft National
High School, Taft, Eastern Samar, Philippines by and between:
TAFT NATIONAL HIGH SCHOOL, an Agency of the national government of the
Republic of the Philippines opened on August 9, 1946, created by virtue of Batas Pambansa
Blg.605 with the principal address at Veteranos St. Barangay 05, Taft, Eastern Samar
represented by its school head, MR. EMIL E. ADENA, herein referred to as “TAFT
NATIONAL HIGH SCHOOL”.

The Stakeholders, and the School Parent-Teacher Association (SPTA) an organization
agency duly organized and existing under the laws of Republic of the Philippines, with office
address at Taft National High School and the stakeholder.
WITNESSETH:
WHEREAS, Republic Act No. 9155 known as Governance of basic Education Act of
2001 is a policy that aims to strengthen School-based Management (SBM) empowering school
and involving the participation of community in delivering education services to the learners in a
more responsive, efficient and effective method through an enhanced school communication
process.
WHEREAS, With the formulation of the School Improvement Plan (SIP) as the roadmap
to measures and interventions of the existing needs of school relative to the pursuit for quality
education.
WHEREAS, The school and the community shall work hand in hand for the continuous
improvement of the instruction, facilities, thrust and programs of the department which is
evidenced-based responsive, systematic approach to planning.
WHEREAS, The partnership of DepEd, Stakeholders and SPTA ensure the formulation
of an organizational structure corresponding to the defined roles, duties and responsibilities of
the member of the School Planning Team.
WHEREAS, herein stipulated are the individual members of the School Improvement
Team and the organizational structure with the corresponding individual assignment and
responsibility and roles in agreement with all the members.
WHEREAS, The stakeholders and other concerned persons together with all school
authorities mentioned in the organizational structure bind themselves to this agreement for and
behalf of the School Improvement Plan.
NOW THEREFORE, for and in considering of the foregoing premises and the terms and
conditions herein set forth, the parties herby agree as follows.
Section 1 Description of the school project and program
The School Improvement Plan (SIP) is formulated in collaboration with
the community, the SIP is a roadmap that lays down the school’s specific
solutions to corresponding identified Priority Improvement Areas (PIAs) covering
a period of three years. It aims to improve the three key result areas in basic
education: access, quality and governance. It is evidence-based, results- based and
child or learner centered.
Section 2 Formulation of the Organizational Structure
The School together with the community and their representatives shall
convene on an agreed time and place for the crafting of a School Planning Team
Organizational Structure vis-à-vis with the individual responsibility and work
description of each member.
Section 3 Duties and Obligations of each member of the team.
a.) School Head as Chairman
a.1.) identify and organize members of the School Planning Team in relation
to the formulation of the school Improvement Plan.
a.2.) Conduct capacity building for every member of the group.
a.3.) Coordinate with appropriate offices and other agencies for the
formulation of the program and other related matters.
b.) SIP Coordinator
b.1.) Spearhead in the organizing the School Planning Team to serve as core
group in the formulation of the program.
b.2.) Schedule meeting and brainstorming
b.3.) Create a feedback mechanism for check and balance as to the
effectiveness of the project.
b.4.) Establish linkages with stakeholders and members of the planning team.
c.) Stakeholders as member of the Team
c.1.) Attend regular meeting of the School Planning Team in the crafting,
revision implementation and evaluation of the project.
c.2.) Participate on deliberations, monitor and evaluation of the project and its
implementation.
c.3) Submit necessary report and data needed in the crafting, implementation
and evaluation of the project.
c.4) Serve as evaluator and feedback-mechanism team to assess the
effectiveness and functionality of the project.
c.5) Report diagnosis and assessment to the project team based on the results
of the implementation of the project.

Section 4. Scope of the Program or Package

The making of School Improvement Plan applies to Taft National High


School as a learning institution. This includes the making of guidelines and policies,
objectives data/information required and the procedure for the preparations,
implementation, reporting, monitoring of SIP and the roles and responsibilities of the
different persons involved; the students, faculty and staff, Local Government Unit, the
parents and stakeholders.

Section 5 Term

The School Improvement Plan (SIP) shall start on April 2019and ends on May of 2022

Section 6 Capacity and Authorization


The parties herein warrant that they have the capacity, power and requisite
authorization to enter into this agreement.
Section 7 Separability Clause
Should any part of this agreement be judicially declared null and void, such
nullity shall not affect the validity of the remaining provision hereof.
Section 8 Breach of Contract
The violation or omission of any part of the provisions of this Agreement shall be
ground for the cancellation or rescission of the same without need for legal or court action.
