1.0 INTRODUCTION

This Corporate Safe Work Instruction shall be complied with by all project members of EPC
Contractors’ and their Subcontractors for their safety. It is to prevent safety accidents caused
by construction equipment by regulating and establishing safety standards in the use of
construction equipment.

2.0 SCOPE

This procedure applies to Jawa#1 CCPP Project. In the event that there is any conflict arising
between this procedure and local statutory and customer requirements, superior standards are
above the others.

3.0 ROLES & RESPONSIBILITIES

3.1 PROJECT MANAGER


Project Manager is responsible for the approval on the operation of rigging work, and
relative activities at the site, and overall health and safety management of the work.
3.2 PROJECT CONSTRUCTION TEAM MEMBER
Project Construction Team Member shall review the adequacy of the plan for rigging
work, and she/he shall have responsibility for inspecting rigging slings and relative
components (lifting support device) on the delivery to the site and on a monthly basis.
He/she shall also manage and oversee training and inspections to ensure that workers
comply with safety standards.
3.3 PROJECT EHS MANAGER
Project EHS Manager is responsible for reviewing the safety of work plan and checking
the adequacy of the rigging slings through a delivery inspection and conducting a daily
inspection and monthly inspection (Colour code management) He/she shall stop the work
when the safety standard is not complied with. He/she shall check the implementation of
the inspection process prior to work.
3.4 SUBCONTRACTOR
Subcontractors shall establish a work plan for the rigging work, select appropriate rigging
slings and components, and comply with safety standards during the operation, and
ensure that the rigging worker conducts the daily inspection and take corrective actions
if necessary.
3.5 WORKER
Workers shall comply with safety standards during the work and conduct an inspection
prior to the work.

4.2 TERMS & DEFINITIONS

Rigging Work
A series of work activities that connect the cargo to the hook using a lifting device, and
transporting to the desired destination, and separating the cargo from the hook.

Sling
A device that is able to lift a cargo, such as a wire rope sling, chain sling, belt sling, etc.
Lifting Support Device
An auxiliary device that helps the cargo remain fastened tightly in order to carry the cargo
safely
Loop (Eye)
A ring-shaped part that makes it easier to fit the end of the sling into the hook

Safe Load
A load that is within the appropriate safety factor and is considered within the range that
is able to operate the sling and lifting support device without damaging its function or
quality
Safety Factor
A numerical value that refers to the safe load by providing the safety of the product use
based on breaking load, and it can be calculated by dividing the breaking load by the
working load

5.0 QUALIFICATION & COMPETENCY MANAGEMENT

5.1 QUALIFICATION
5.1.1 The rigging work in designated hazardous work shall be conducted by a qualified
rigging worker in accordance with the instruction.
5.1.2 A ringgingworker shall have an appropriater training and qualification in
accordance with national legislation.
5.2 COMPETENCY MANAGEMENT
5.2.1 Copies of evidential documents for rigging work qualification/license shall be
kept at the EHS Team office.
5.2.2 A rigging worker’s clothing shall be designed according to the country or site
standards and shall be identifiable with the signalman and banksman.
5.2.3 A rigging worker shall be trained more than 2 hours per month on a regular
basis, and the following shall be included.
 How to check the sling and lifting support devices
 Handling of cargo and its working methods
 Signaling method and team work
 Other requirements for safety and health management
5.2.4 Lifting operations shall be supervised and carried out by competent persons.
Competent requirements are set out below;
 Lifting operations shall be supervised by a competent lifting supervisor who
shall have relevant experience and hold a valid 3rd party certificate of training
in lifting operations.
 Rigging of lifting equipment, slinging of loads, signaling of cranes will be
undertaken only by trained and competent persons. All riggers, slingers and
signalers shall hold a valid 3rd party training certificate in the relevant discipline.
Slinger / signalers shall wear an orange construction safety helmet and an
orange high visibility vest for the purpose of easy identification on the worksite.
 Lifting equipment operators shall be trained and shall hold a valid 3rd party
training certificate and shall be competent in the use of the specific lifting
equipment on site.
 Where relevant operators of mobile lifting equipment shall hold a national
driving license for the class of mobile equipment.
 General users of lifting equipment and lifting accessories shall be trained in the
safe operation of the specific make and model of equipment and associated
accessories.
 Operator or specialist responsible for the assembly and dismantling of lifting
equipment (e.g. tower cranes / crawler cranes) shall be competent and trained
in the safe system of work for erection / dismantlement for the specific make
and model of equipment.
 The Project Management Team shall establish documented training records for
lifting equipment operators, riggers, slingers and signalers.

6.0 PROCEDURAL REQUIREMENT

6.1 WORK PLAN PREPARATION


6.1.1 Prior to any rigging work, a work plan shall be created, reviewed, and approved. -
Work plan preparation by subcontractor→ reviewed by Project Construction Team
Member → reviewed by Project EHS Manager → approved by Project Manager.
6.1.2 Rigging work (Cargo handling) plan regarding lifting cargo shall include a work
plan containing the conditions of use of the construction equipment, and it shall
add the required section corresponding to the rigging work plan.
 General lifting work shall be carried out by a crane. For lifting a light load, it shall
only be performed by the equipment that it is designed for and has been
evaluated and inspected by the manufacturer, third party and approved by
DISNAKER, such as a forklift .
 When planning for rigging work, the safety measurer for critical risk factors and
plan for handling of heavy cargo shall be taken.
6.1.3 Rigging slings and lifting devices shall be checked by the safety team every month
and color-coding management as shown in the table below shall be implemented
for easy visibility. (3 days before the last day of each month).

However, the color can be changed according to client’s request and site
standards.
6.1.4 When applying a man basket and lifting container, the units shall only be used
after having a structural review by the relevant division of each business unit or by
the designated technical personnel in the country, and all documentation shall be
kept with the safety team.

6.2 DELIVERY OF RIGGING SLING


6.2.1 Rigging slings shall be a standard product made by a professional manufacturer
and shall be brought into the site after planning reviews, and shall not be used
arbitrarily for construction and repairs at the site.
6.2.2 Rigging slings shall have a marking (tag, label, etc.) indicating the maximum safety
working load, and the documents need to be considered when bringing into the
site are as follows:
• A certificate of safety (Country or international standard), and a certificate of
manufacture or warranty
• A test report of rigging sling, etc. and other document (if necessary)

• A safety factor shall be over 6 for Samsung C&T standard, over 4 for the
support, and over 10 for a man basket shall be applied.
6.2.3 The safety factor (cutting load value divided by maximum value of load) of the
lifting support device, such as a sling with a hook or shackle fastened together,
shall be equal to or higher than the safety coefficient of the rigging sling.
6.2.4 Delivery condition of the wire rope sling
• A test report of the wire rope sling (Including certification, size, breaking load)
• A marking of the maximum allowable load on the sling
• The section that has been cut and processed is done by mechanical methods
instead of heating methods, such as gas torch.
6.2.5 Delivery condition of chain sling
• A chain test report (including size and breaking load)
• A marking of nominal diameter, grade, or code of chain
• It shall be over grade G80 for lifting purposes.
• Only one welding section shall be permitted in one link, and the pitch shall be
within 3 times of the diameter.
• Confirming usage standard that is suggested by the manufacturer
6.2.6 Delivery condition of belt sling
• The same material shall be used (polyamide, polyester, polypropylene, etc.).
• A marking of type, model, safe load, manufacturer, manufacturing date or code
• Confirming the safety of certified products in Indonesia, or other equivalent
and internationally recognized standard, including Europe, USA, Korea, etc.
• Confirming the usage standard that is suggested by the manufacturer

6.2.7 Delivery condition of shackle


• A marking of the manufacturer or registered trademarks, safe working load,
diameter of body
• Standardized parts of body, pin, or bolt
• Confirming usage standard that is suggested by the manufacturer
6.2.8 Delivery condition of clamp
• A marking of manufacturer, grade, model, usage (vertical, horizontal, or
combined type)
• A marking of working load and safety ring attached
• Standardized parts of body, pin, or bolt
• Confirming the usage standard that is suggested by the manufacturer
6.2.9 Delivery condition of other lifting support devices (eye bolt, master link, hook,
turnbuckle)
• Providing necessary documents, such as drawings that can be checked for
the standards and original dimensions
• Confirming the usage standard that is suggested by the manufacturer

6.2.10 Delivery condition of lifting container
• A review of the structure (at first), non-destructive inspection (once every 6
months)
• Using wire rope slings and chain slings (belt sling is prohibited)
• Using 4 lines for hooking at 4 edge points for all products
• Fastening method (semi-permanent fastening of curved shackle pin bolts, and
thimble installed inside the loop of wire rope)
• Attaching all related documents, such as safety signs displaying the loading
capacity, test report, etc.
6.2.11 Delivery condition of man cage
• A review of structure (at first), non-destructive inspection (once every 6 months)
• Using wire rope sling and chain sling (belt sling is prohibited)
• Using 4 lines for hooking at 4 edge points for all products
• Fastening method (semi-permanent fastening of curved shackle pin bolts, and
thimble installed inside the loop of wire rope)
• Attaching all related documents, such as safety signs displaying the loading
capacity, test report, etc.
• Installation with safety standards for doors, safety rails, and toe boards, etc.

6.3 CHECK OF RIGGING SLING


6.3.1 When the rigging sling and lifting support device are brought into the site, it is
necessary to use the standard product complied with the certification procedures
in accordance with each country regulations and international standards, and the
inspection shall be carried out according to the inspection system of the country.
6.3.2 A copy of the supporting documents for manufacture, authentication, and
inspection shall be kept in the safety team’s office.
6.3.3 Slings and lifting support devices shall follow the disposal standard that is
proposed by the manufacturer, and they shall be inspected and disposed
according to the following criteria:
6.3.4 Inspection and disposal standard of wire rope sling
• It shall be inspected before the start of operation, and the inspection items are
as follows;
- Degree of abrasion: Measure the diameter of the most worn out and
loaded area
- Presence of disconnection: Number of disconnection and its distribution
status
- Degree of corrosion: Corrosion and rusting condition
- Oiling condition: Surface and internal oiling condition
- Combined status: Condition of loosening and tightening between the joint
and the inserted end
- Others: Maintaining the normal status of twist and evidence in a tangle

• The criteria for disposal are as follows;
- When damage on a single strand in one twist section is 10% or more
- When the wire's nominal diameter has been reduced by 7% or more
- When it is severely deformed, corroded, and/or tangled up
- When the number of disconnected strands in non-rotating wire is either 4
or more within 6 times of wire rope's nominal diameter or 8 or more within
30 times of wire rope's nominal diameter
- When it is damaged by heat and electric shock
6.3.5 Inspection and disposal standard of chain sling
• Chain slings shall be inspected before the operation starts, and the inspecting
items are as follows:
- Degree of abrasion: Measure the diameter of the most worn out and the
place where the ring contacts
- Deformation status: Deformation and crack of the link
- Degree of corrosion: Corrosion and rusting condition
- Others: Damage due to external impact
• The criteria for disposal are as follows:
- When length is measured 5% longer than manufactured date
- When ring diameter has been reduced by more than 10%
- When there is a crack or deformation
6.3.6 Inspection and disposal standard of belt sling
• Belt slings shall be inspected before the operation starts, and the inspecting
items are as follows;
- Degree of abrasion: Core wires are exposed due to damage of the cover
of the loop and body sections
- Exposure status: Exposure of core wires due to loose threads on binding
and joint sections
- External reaction: Severe discoloration and chemical reaction due to heat
and chemicals
- Deformation status: Uneven thickness due to partial accumulation of core
wires
- Others: Normal condition of twisted state and damage by external impact
• The criteria for disposal are as follows:
- When there is fluff, damage, or stitch over the entire width of the belt
- When defect area by cut or scratched is equivalent to 10% of width or 20%
of thickness
- When loosened length of the sewing line is larger than the width of the belt

- When the sewing thread is cut in the sewing lines and loop section, or
even if belt separation is recognized
- When it is severely damaged or corroded, or exposed by chemicals
- When the period of use exceeds 3 years
• The criteria for disposal of round sling are as follows:
- When core wires are exposed due to damage of the cover of the loop and
body sections
- When core wires are exposed due to loose threads on binding and joint
sections
- When uneven thickness is recognized due to partial accumulation of core
wires
- When the period of use exceeds 2 years
6.3.7 Inspection and disposal standards of a shackle
• Shackles shall be inspected before the operation starts, and the inspecting
items are as follows:
- Deformation is recognized clearly by visual inspection
- Using standardized parts and checking if the fastening pin and bolt is
unavailable due to body deformation
- Checking for missing pins
- Confirming non-destructive inspection (once every 6 months)
• The criteria for disposal are as follows:
- When screw threads are damaged
- When the shackle is deformed permanently or grooved by external impact
- When the abrasions to the body diameter, pin and bolt exceed 5% of the
original dimension
- When there is no marking of safe working load, manufacturer, and body
diameter

6.3.8 Inspection and disposal standard of clamps


• Clamps shall be inspected before the operation starts, and the inspection
items are as follows;
- Any damage, abrasion, or deformation
- Installation and normal working of safety ring
- Clearly displaying grade, allowable load, etc.
- Standardized parts of body, bolt, nuts, and pin
• The criteria for disposal are as follows:
- When safety ring is missing or does not work

- When the abrasion of the part (Jaw, cam) which contacts with the material
is worse than the manufacturer’s standard
- When the amount of abrasion of the pin and pin hole, and deformation to
the opening exceed 5% of the original dimension
- When clamps are modified arbitrarily, or welded, or affected by the heat
6.3.9 Inspection and disposal standard of a turnbuckle
• Turnbuckles shall be inspected before the operation starts, and the inspection
items are as follows.
- Damage, abrasion or deformation of the ring part and hook
- Damage, abrasion or deformation of the bolt, nut, and rod
6.3.10 The criteria for disposal are as follows:
- When the deformation of the hook opening exceeds 5% of the original
dimension
- When the body is bent or deformed
- When screw threads are damaged, or rod is worn by more than 5% of the
original dimension
6.3.11 Inspection and disposal standard of master ring and hook
• The master ring and hook shall be inspected before the operation starts, and
the inspection items are as follows.
- Damage or deformation of ring or hook
- Smooth rotation of the shank part on the hook or abrasion of the screw on
the shank part
- Normal working of hook safety latch (if installed)
• The criteria for disposal are as follows:
- When the deformation exceeds 5% of the original dimension
- When the abrasion exceeds 10% of the original dimension
- When there is severe cracking, thermal deformation, or corrosion

6.3.12 Inspection and disposal standard of a lifting container


• Lifting Containers shall be inspected before the operation starts, and the
inspection items are as follows.
- Welding condition of lugs and material components (Non-destructive
inspection conducted if crack is suspected during visual inspection)
- Abrasion, damage, or fastening condition of the rigging sling
- Following inspection procedure in accordance with Lifting Container

• The criteria for disposal are as follows:
- When material part is damaged, or welding defect is found

- When it is subject to the disposal criteria of each rigging sling

6.3.13 Inspection and disposal standard of a man cage


• Man Cages shall be inspected before the operation starts, and the inspection
items are as follows.
- Welding condition of lugs and material components (Non-destructive
inspection conducted if crack is suspected during visual inspection)
- Abrasion, damage, or fastening condition of rigging sling
- Condition of the safety rail, toe board, and gate
- Wearing a life jacket and installing a life buoy in case of marine operation
- Following inspection procedure in accordance with Man Cage Safety
Checklist
• The criteria for disposal are as follows:
- When a material component is damaged, or welding defect is found
- When the Man Cage is subject to the disposal criteria of each rigging sling
6.4 CHECKING DOCUMENTS
6.3.14 Documentation to be checked in operation
Project EHS Manager shall frequently check the documentation for the following:
• Records of daily and monthly inspections (Color management).
• Records of test reports for slings and lifting support devices
• Records of structural review for a man cage and lifting containers
• Records of non-destructive inspection reports and expiration dates
6.3.15 Lifting Plans & Permit to Work
• All kind of lifts will require a lifting procedure in form of Lifting Plan/Permit
depending on the load and lift type. Lifting criteria has been set as “General
Lifts”, Intermediate Lifts,” Heavy Lift” & Critical Lifts. Any Load with 75% of the
rated crane capacity will be considered as “Critical lift” regardless of load
weight. Contractors while applying the Lifting Plan/permit have to take the
actual configuration of the crane .1.e Crane SWL as per current counter weight
and line parts. Similarly attached and take the information from the load chart
as per the actual configuration of the crane.
6.3.16 General lift (Requires only Lifting Plan)
• Any Load under 10 tons that will be lifted or lowered using Single Crane shall
require General lift plan (1-10 Tons). The General Lift Plan shall be valid for
unlimited time until unless changes in the crane configuration or and lifting
related personnel for a specific area. General Lift Plan needs to be submitted
24 hours before the lift date. If general lifting involve erection or and installation
of the load shall also require lifting permit.

Plan shall prepare and approved by Lifting Supervisor. Intermediate Lift Plan shall
be valid only until up to 7 days. Intermediate lift plan needs to be submitted
minimum 24 hours before the lift date. Intermediate plan also require PTW.

6.3.18 Heavy Lift

Any load above 20 tons that will be lifted or lowered using crane shall be
considered as “Heavy Lift” and shall require Heavy Lifting Plan. Heavy Lift Plan
shall be prepared and approved by Lifting Supervisor. Heavy lift plans shall require
approval from each EPC lifting specialist at Head office. Heavy Lift Plan shall be
valid only until that specific lifting task is completed. The Heavy lift Plan needs to
be submitted minimum 7 days before the lift date. Heavy lift plan also require PTW.

6.3.19 Critical or tandem Lift
Any Load > 40 Tons, lift (1) that exceeds 75 percent of the rated capacity of the
crane or (2) requires the use of more than one crane (Tandem Lifting) (3) Lifting
personnel in man-basket shall be considered as “Critical Lift” and shall require
Critical lifting plan designed and approved by Lifting supervisor. Critical lifting plan
shall also require a specific Method Statement and Risk Assessment describing
step-by-step rigging methods. Critical Lift Plan shall be valid only until that specific
lifting task is completed. The Critical lift Plan needs to be submitted minimum 24
hours before the lift date.

7.0 METHOD

7.1 GENERAL
7.1.1 During the use of rigging slings and lifting support devices, the following shall be
complied with:
• Rigging slings shall not be used by exceeding its working load, the lifting angle
shall be maintained within 60 degrees, and it shall not be dragged or dropped.
• Wire ropes shall not be used for lifting operations if there are significant cut,
snags and / or holes. The competent supervisor shall determine when to
withdraw a web sling from use.
• Lifting accessories shall display the SWL (safe working load).

• An operation with a man cage by a mobile crane shall be prohibited in the case
of overseas projects, a man cage can be used according to the standards set
by the country regulations. The man cage shall not be operated with exceeding
wind speed of 7 m/s and lifting speed of 30 m/min, and the load including the
passengers shall be less than 50% of the crane capacity and lifting gear
capacity.
• Wire ropes shall not be used for lifting operations if kinked, significantly rusted
or individual wire strands are broken. The competent supervisor shall
determine when to withdraw a wire rope from use.

• When cutting wire rope or chain, it shall be cut by mechanical methods. Wire
rope shall not be cut by heating methods, such as a gas torch, and the sling
which is affected by the heat, such as an arc, flame, or high temperature
contact, etc. shall not be used.
• The wire rope termination method, such as the direction and number of clips,
and fastening method, and strand arrangement, etc. shall be used according
to the standards of manufacturers and country.
• The basket hanger shall not be used to prevent the cargo from falling down,
and the pallet and prop shall be managed firmly in order to avoid breaking and
collapse, and the cargo with sharp edges and angles shall be covered with
proper protection.
• Rigging with only one line shall be prohibited, but if it is unavoidable, measures
shall be taken to prevent the cargo from slipping, dropping, and rotating.
• If more than 2 lines are used, a curved shackle shall be used. If a belt sling is
used, the designated shackle only for the belt shall be used.
• If more than 3 lines are used, the tension of the slings shall be uniform, and
impact loads shall not be imposed.
• When a strangle knot method is used, a shackle shall be used to prevent
damage to the loop section, and the direction shall be checked to prevent the
shackle pin and bolt from loosening. The diameter of the shackle body shall
be larger than the wire rope diameter, and it shall not be used from the side.
• Lifting support devices, such as hook, shackle, clamp, or ring, etc. that have
deformation or cracks shall not be used as a hooking device for the crane, and
any obstacle within lifting operation range shall be checked in advance, and
the sling shall not be released due to collision with nearby structures.
• Coupling links and master links in the chain sling shall be selected based on
the diameter of the chain.
• A belt sling shall not be heavily twisted or tied, and the loop part shall not be
overly tightened when it is hung on the hook.

• Use of web belt slings shall be avoided, and round slings shall be used instead.
Life expectancy shall be checked for the round sling and replaced when
exceeded.
• A 1-ton bag and fiber rope sling shall not be used for heavy load lifting
purposes, and if the 1-ton bag is lifted by a crane, it shall be operated only
after putting it in the lifting container.
• When working near high-voltage lines, the separation distance shall be
ensured, and a foreman shall be deployed.
• Only main hook of the crane shall be used for lifting activities. If any contractor
need to use secondary crane hook shall require approval from Samsung C&T
HSE Manager
• All loads shall be secured and balanced before lifting. Ratchet straps, banding
and or cargo netting may be used.
• Exclusion zones shall be implemented for loading / unloading zones. Exclusion
zones shall be clearly signed and demarked with warning tape or hard barriers
to prevent inadvertent access. Slinger / signalers shall ensure that general
workers are moved from the path of slung loads where practically possible. All
slinger / signalers shall be issued a whistle for warning general workers of
lifting operations in progress.
• All mobile cranes shall extend outriggers before commencing lifting operations.
All crane warning devices / lights shall be in full working order prior to
commencing lifting operations.
• Crane operators shall not exceed the crane capacity and shall immediately
cease operations should automatic warning devices activate.
• Grillages (crane mats) shall be used on mobile crane outriggers to spread the
load. The outriggers pads shall be double of the size of outrigger base and
made from a solid wood or manufactured pads.

• Outrigger pads size > X2 of Outrigger base and to reduce the point of contact
between an outrigger and the ground, the steel plates are required to install
after the outrigger pads.
• The dimension of the steel plates are based on the equipment and ground
condition.
• For a crawler crane, the steel plates shall be 2m x 6m x 25mm
• For a mobile crane, truck crane and pump car the steel plates shall be
1.5m x 1.5m x 25m with the ground surface has been confirmed by civil team
for the soil endurance and stability

• If the ground surface is not confirmed by civil team for the soil stability, 2m x
6m x 25mm shall be applied.

• A copy of 3rd party inspection and test certificates shall be maintained for all
lifting equipment and lifting accessories (lifting gear).
• Lifting equipment and accessories (lifting gear) shall be color coded on a
monthly or quarterly basis to ensure that only certified equipment is in use on
the site. A color tag shall only be attached to equipment / accessories that
have a valid test / inspection certificate.
7.1.2 After a rigging worker completes the preliminary work for lifting operations, such
as binding of cargo, etc., and the cargo shall initially be lifted only 10~20cm for
checking the center of gravity. Then, the worker shall maintain a safe distance
from the lifting operation range. When the cargo is landing, the worker shall wait
in a safe position and take the cargo by holding the guide rope in order to lower
the cargo safely.
7.1.3 When an excavator is used for lifting operations, the model that has been reviewed
for its lifting capacity shall be selected, and the lifting operation plan shall be
prepared according to the lifting load table presented by the manufacturer. The
installation standards of safeguards (overload prevention device, etc.) for local
statutory requirements shall be observed.
7.1.4 When a crane is used for lifting construction equipment, the equipment shall be
fastened to the designated lifting hooks with proper rigging methods, and shall be
followed the equipment manual requirements. In the case of large or high-priced
equipment, the specially manufactured and examined devices, such as spreader
beams, shall be used.
7.1.5 A sling used for pulling or towing shall have a safety factor of 4 or more, and the
area shall be restricted from the workers, where there is a risk that the sling and
lifting support device can be broken and dispersed.
7.1.6 Any action which removes the safeguard functions or disturbs normal operation
shall not be taken during the operation. In the case when the abnormality of
safeguard is identified, the operation shall be immediately stopped, and the
findings reported to the Project Construction Team Member and Project EHS
Manager.

8.0 Recommended Slinging Methods

8.1.1 The slinging methods detailed below are recommended. The guide below contains
only basic information. Exact slinging techniques should be determined by the
rigger under the supervision of the competent lifting supervisor. Project HSE staff
should check that slinging techniques are broadly in line with this SWP and that
the overuse of fabric straps is minimized particularly were chain slings or wire rope
slings are recommended.

Man Baskets

Man baskets may be lifted by crane. A safety wire rope


shall be attached as shown in the diagram.

The operative shall wear a restraint harness without


shock absorber and shall tie on as shown in the
diagram.

Man baskets shall be certified every 6 months.

Cranes and accessories used for man lifting shall be


certified every 6 months.

Specific method statements shall be required from


contractors prior to commencement of man lifting
operations.

Scaffold Tubes / Rebar Lengths

When lifting bundles of tubes, bars or other loose


materials (whether banded or not) slings should be
double wrapped.

Note that a choke hitch will reduce the SWL by 20%

Do not use fabric straps as individual tubes / bars are


likely to slip free. Fabric straps will also be significantly
damaged by rebar and may fail.

Concrete Bucket

Concrete buckets should not be lifted directly by the


crane hook.

A single-leg sling (commonly known as a drop or skip


chain) should be used, as moving a heavy crane hook
precisely into place can be difficult.

Stillages

Wrap wire rope or chain slings round corner posts. Do


not use fabric straps. Do not attempt to lift double
stacked stillages.

Mesh / Pre-formed Rebar cages

Welded Rebar Mesh


Pass each hook of a four-legged chain sling through
the mesh and return to form a choke hitch. Lifting points
shall be positioned evenly to prevent undue bending /
twisting.
Prefabricated Rebar Assemblies
These assemblies rely on tying wire to hold them
together. Lifting points MUST be determined by the
Lifting Supervisor.

Palletized Loads

Pallet forks with safety netting should be used for


blocks and other stacked materials than is delivered on 
a pallet.

Note that the netting size should be smaller than the


smallest item to be lifted.

Pipes

For short pipes, pass wire rope or chain sling through


pipe.

For long pipes, use two wire rope or chain slings in a


double wrapped basket or choke hitch if slings are likely
to slide together.

If available, use pipe hooks / clamps for the end fittings


of the lifting chain. Alternatively, a web sling can be
used.

Fabric straps may be used if centered or 2 straps are


used.

Steel Sections / Beams

Use double wrapped wire rope or chain slings when


slinglegs can slide together.

Lifting Points / Spreader Bars

Many items of plant or prefabricated equipment (e.g.


porta- cabins, compressors, skips, pre-built scaffold
etc.) are provided with a certified, built in lifting point.
Lifting points should be used with 2 or 4 leg chains.
Attachment points MUST be determined by the
competent lifting supervisor.

Where items may bend or twist, a spreader bar or lifting


beam shall be used.

Fabric straps can be used if shackles are fitted to the


lifting points.

Form Work Lifting

Formwork tables and shutters shall be lifted with chain


slings where lifting points are available or specialist
lifting devices such as the C Hook (shown).

This is a specialist activity and shall require a detailed


method statement from the supply partner prior to
commencement of work.

Tag Lines

Double tag lines should be used where possible to


improve landing control.

Centre of Gravity

Where uneven or unbalanced loads are lifted, the


slinging method shall account for the centre of gravity.

9.0 Safety Incident Prevention

9.1.1 Fall Prevention for Man Cage


• The working route shall be checked in advance in order to prevent collision.
• The condition of the hook safety latch and rigging slings shall be inspected.
• The safe load shall not be exceeded, and lifeline shall be installed and
connected to the anchorage safety facility.
9.1.2 Falling Object Prevention
• The working route for equipment shall be planned, assigned, and restricted.
• Only authorized lifting support devices shall be used at the site and inspected
before the operation starts.
• A sling shall not be used in excess of the safe working load suggested by the
manufacturer.
• Guide ropes (tag lines) shall be used for lifting operations, and the loading
material and rigging work shall not interfere with surrounding objects.
• Restrict the access within the working radius, and a signalman shall be
deployed.
9.1.3 Caught-in Prevention
• When fastening a lifting support device, attention shall be paid to prevent
fingers or body parts from being caught in the components.
• Holding or supporting the rigging slings by hand shall be prohibited.
• The loading material shall not be dragged or pushed by the worker, and a
guide rope (tag line) with sufficient length shall be used.
• A fixed object shall not be separated or disassembled by using a rigging sling.
9.1.4 End of Operation
• The rigging worker shall send the pre-arranged signals clearly to the
signalman and operator.
• The man cage used for emergency evacuation and rescue operations shall be
stored in a designated place that can be accessed and deployed quickly.

shall be stored in a designated place.
• Lifting containers and man cages shall be stored in a designated place.
• Unqualified slings and lifting support devices according to the manufacturer’s
standard shall not be stored or repaired arbitrarily, and they shall be disposed
of immediately.
