Please read all the instructions

below before you begin.

Step 1: Visit the BOSA REDCap survey. The reusable link is provided at the end of the instructions.

Step 2: The Clinician Information survey - please enter basic information about you, the clinician. After
you submit this form, you will be automatically redirected to the form where you may enter patient data.

Step 3: The Patient Information survey - please enter the patient's de-identified information. The required
sections of this form include basic demographics, diagnoses, and scores from the BOSA DSM checklist.
The additional sections, which are not required, include cognitive functioning, ADI-R, ADOS, adaptive
functioning, CARS-2, SRS-2, SCQ, and SDQ. Repeat this step for each patient.

Step 4: At the bottom of the Patient Information entry form, you will see that there are 3 submission
options available: a) “Take this survey again,” b) “Save and return later,” and c) “Submit.”

a) "Take this survey again" - You will be able to repeat the BOSA Patient Information survey for
more than one patient. In order to do this, please click "Take this survey again." This form can be
completed as many times as needed in one sitting.

b) “Save and return later” - If you would like to return and enter more patients at a later date,
please click "Save and return later." Copy or write down the Return Code. Without it, you will not
be able to return and continue this survey.

You may bookmark the survey page to return to it, OR you can have the survey link emailed to
you by providing your email address when prompted. For security purposes, the Return Code will
NOT be included in the email. If you do not receive the email soon afterward, please check your
Junk Email folder.

c) "Submit" - Once you click "Submit" you will not be able to return to the Patient Information
survey. If you click "Submit" before entering data for all your patients, you will need to use the
original survey link (see link below) to access the Clinician Information survey again. You will
need to enter the clinician information again before you will be redirected to the Patient
Information survey.
