Ma’am:

In connection with the ALLEGED SHOOTING INCIDENT that transpired in #123

PUROK 3 BARANGAY GIBRALTAR, BAGUIO CITY on NOVEMBER 10, 2018 at
OOA 9:00 AM, with suspect(s). ERICK PAGAS alias “RIK RIK” herein submitted are
the following ballistics evidence:

1) A suspected Caliber .45 Pistol Firearm with Serial No.1945136 , recovered 6ft
away from the door, marked as:
a) GRC-1-11/10/18.
2) Two (2) pieces of fired cartridges from a suspected cal. 45 pistol recovered
inside the house 3ft from the door marked on the inside near the mouth as the
following
a) GRC-2-11/10/18
b) GRC-3-11/10/18.
3) One (1) piece of fired bullet head from a suspected Caliber .45 Pistol firearm
recovered from the wall of the kitchen 4ft from where the victim was allegedly shot
and 3’5” from the floor marked as:
a) GRC-4-11/10/18.
Attached are:

1. Fifteen (15) photographs; three (3) general views, Four (4) medium range view,
six (6) close up views and five (5) extreme close up views.
2. Finished and initial sketch of the crime scene and the locations of the evidences.

The above pieces of evidence were received and undersigned by PSUPT GERARDO K
TUMBAGA on November 11, 2018 at Baguio City Police Office (Main Office).

Purpose of examination is to conduct scientific examination and analysis on the

submitted specimens whether or not the suspected fired cartridges, marked as: GRC-2-
11/10/18, GRC-3-11/10/18; and fired bullet, marked as GRC-4-11/10/18 was fired from
the suspected Caliber .45 Pistol Firearm with Serial No. 1945136 marked as GRC-1-
11/10/18.

We therefore request for a furnish copy and acknowledgement receipt of your findings
for further investigation and filing of complaint in our office.
