Data Privacy Consent for College Students

I am allowing the Technological Institute of the Philippines (T.I.P.), through its authorized
personnel, to collect, use, and process my personal information, which includes my name,
date of birth, email and physical addresses, demographic information, contact information,
and other relevant information, photograph, audio/voice recording, video, as applicable, for
the following purposes:

1. The processing of my application for admission, registration, enrollment, and graduation

from T.I.P.;
2. The processing of my application for scholarships/grants;
3. The processing of my request for school-issued documents such as, but not limited to,
Transcript of Records, Certificate of Graduation, Certification of Grades, and Certificate of
Attendance;
4. The creation, activation and reactivation of my student's portal login account and login
credentials and my parents'/guardian's account and login credentials on the Academic and
Registration Information System (ARIS) of T.I.P.;
5. The posting of my grades, account balances, scholarships/grants, if any, and class
schedule on my ARIS student's portal account, which may be accessed only using my login
credentials;
6. The disclosure of my grades, account balances, scholarships/grants, if any, and class
schedule to my parents/guardian through the ARIS parents'/guardian's portal, subject to my
prior approval if I am at least eighteen (18) years of age;
7. The inclusion in the T.I.P. Excellence Roll of my full name, academic program, and
General Point Average (GPA) for the semester/s when my academic performance is
considered exemplary based on T.I.P. policy;
8. The publication on the T.I.P. website, T.I.P. circulars or memoranda, the T.I.P. TV, T.I.P.
Voice, social media platforms, international/national TV, radio, and print publications or any
public announcement of my full name, photograph, academic program, and year level, and
the posting of tarpaulins with the aforementioned information, should I participate or win in
any competition or event as a representative of T.I.P.;
9. The publication on the T.I.P. website, the T.I.P. TV, social media platforms,
international/national TV, radio, and print publications or any public announcement of my full
name, photograph, degree, and academic rank, and the posting of tarpaulins with the
aforementioned information, should I top a board examination;
10. The publication on the T.I.P. website, the T.I.P. TV, social media platforms,
international/national TV, radio, and print publications or any public announcement of my full
name, photograph/video, and academic program to show my participation in T.I.P. school
events;
11. The processing of my applications for internship/externship and job placement through
the T.I.P. Career Center;
12. The evaluation of T.I.P.'s services and products for the continuous improvement of the
same and to support its decision-making activities;
13. Counseling, advising, coaching, mentoring, and other similar sessions that may
contribute to the improvement of my academic performance and personal well-being;
14. My membership in the T.I.P. alumni association after graduation and/or updating of my
personal information through graduate tracer studies;
15. The legitimate and lawful disclosure of my personal and other relevant information to the
appropriate government and non-government agencies requiring reports/data on
enrolled/graduated students of T.I.P.;
16. The legitimate and lawful disclosure of my personal and other relevant information to the
appropriate administrative agencies, enforcement authorities, regulatory bodies, or other
government or public authority in connection with any government inquiry or proceeding
requiring the same; and
17. The transfer or disclosure of my personal and other relevant information to T.I.P.'s
industry partners, present and/or prospective employers, which shall use or process my
personal information solely for the purposes mentioned above and as part of their
background checking.

I am also allowing T.I.P. to store my personal and other relevant information as long as
necessary for the fulfillment of the above purposes and in accordance with applicable laws,
including the Data Privacy Act of 2012 and its Implementing Rules and Regulations, and the
T.I.P. privacy policy.
