Abstract
The argon beam coagulat or (ABC) delivers radiofrequency elect rical energy t o t issue across a jet of
argon gas, providing noncont act , monopolar, elect rot hermal hemost asis. This st udy compared t he
efficacy of t he ABC wit h convent ional t echniques for cont rol of t raumat ic hepat ic and splenic
hemorrhage. St andardized lacerat ions were made t o t he liver and spleen of 6 heparinized dogs.
Cont rol of bleeding was at t empt ed wit h t he ABC (150 W), t he Nd:YAG laser (90 W, noncont act ),
horizont al mat t ress sut ure, microcryst alline collagen, and regenerat ed cellulose. If bleeding had not
been cont rolled wit hin 3 minut es, coagulat ion was at t empt ed wit h t he ABC in order t o prevent
exsanguinat ion. Tissue from bot h t he liver and spleen was assessed hist ologically for damage induced
by t he ABC. Delayed injury and early healing aft er ABC coagulat ion was st udied in six addit ional dogs
killed 1 and 3 weeks post operat ively. The ABC st opped bleeding from 25/25 hepat ic lacerat ions in 48
+/- 8 seconds (mean +/- SEM) and from 18/18 splenic lacerat ions in 28 +/- 3 seconds. The Nd:YAG
laser, mat t ress sut ures, and t opical hemost at ic agent s failed t o cont rol bleeding in 14 of 15
applicat ions aft er 3 minut es. The ABC successfully salvaged all failures in less t han 1 minut e. The
dept h of splenic and hepat ic t hermal injury wit h t he ABC ranged from 2 t o 7 mm and was proport ional
t o t he durat ion of applicat ion. Post operat ively wound healing progressed normally wit hout bleeding or
infect ion at t he coagulat ion sit e. The ABC appears t o be a excellent inst rument for achieving
hemost asis in solid organ injury, and may be especially valuable in managing pat ient s wit h coagulat ion
deficit s.
