APPENDIX A - Medical
1. INTRODUCTION
The required tasks consists of establishing the design, specifications, list of medical
equipment, instruments and furniture of Tyr general Hospital.
The above feasibility study will be the guideline and the basis of this scope

2.1.1.2 Furniture
Establish the master plan, the specifications, the quantities and cost estimate for the
medical and non medical furniture.

~nsultant shall indicate the position and the power needed in every outlet.
The electrical consumption of every piece of equipment shall be specified by
hour, as well as the utilization pattern over 24 hours with a clear indication of
peak utilization. The consultant shall also indicate any special arrangements if
the need arises.

Mechanical works:
~ant shall indicate the type of fluids and the position of outlets needed
for every equipment, as well as the nature, quantity and the mode of waste-water
disposal suggested for every piece of equipment
• The consultant shall indicate the heat dissipation of every piece of equipment, the
type of ventilation and the room temperature needed in the room and per
equipment, if need be.
The consultant shall make site visits to the hospital. For all civil, electrical and
mechanical works, he shall compare what is required for the installation and functioning
of each equipment to the works already executed in the hospital and shall integrate in
the tender documents all missing works to be completed or corrected.
2.1.2.1 Phase I:
2.1.2.2 Program
Illustrate the recommended program of requirement according to the feasibility study.
Propose a final program in close coordination with the Client and get his approval.
2.1.2.3Ust of equipment
Define the list of equipment in coordination with the Client and upon his approval.
2.1.2.5 Phase II :
• Establish the list, the specifications, the quantities, room by room and the cost
estimate related to medical equipments, instrumentation for surgery, laundry,
kitchen equipments and the medical and non medical furniture.
• Provide a room data sheet for each room in the hospital and the health centers..
• Provide a drawing showing the utilities needed for the medical equipment.
After the approval of Phase II, the tender documents shall be prepared. These
documents shall include:
• The loaded drawings,' (scale 1/100)
• The electromechanical drawing showing the modifications due to the medical
equipment '
• Room by room, Data sheet
• List of medical equipment,
• List of surgery instrumentation,
• List of medical and non medical furniture,
• Laundry and kitchen equipment,
• Specification,
• Bill of quantity,

TIME FRAME:
The time frame needed to implement the above work is 8 months from the date of
notification, subdivided as follows:
• Phase 1 : Phase 4 (Construction) +2 months
• Phase 2 : Phase 4 (Construction) +3 months
• Phase 3 : Phase 4 (Construction) +5 months
4. NUMBER OF COPIES:
The documents related to phases 1, 2 and 3 will be submitted in two hard copies and
one soft copy, and in 6 copies after the approval of phase 3.
APPENDIX B
Consultant's Personnel
The consultant shall employ such staff as may be necessary to fulfill his obligations
under the agreement. The consultant shall make his own assessment of the staff
needed for carrying out the work but must comprise at least the minimum specified
here:
Project Manager
Senior Biomedical engineer
Electro-mechanical engineer
Additional staff or specialists (Biomedical engineers, technicians,... ) as needed.
a. Project manger:
Principal duties: represents the consultant to the client. Holds overall responsibility
for the direction uf the consultant's team. Maintains contact with the client and the
consultant main office. Liaises with all parties concerned in the implementation of the
project. Maintains control of programming and implementation and ensures that
works are proceeding according to schedule.
Principal duties: ensures that all the equipment are confirm with the hospital needs,
check the architecture drawing to insure that areas can receive. the equipments
properly and to prepare the documents mentioned in the TOR.
Principal duties: In addition with design task he should Coordinates between the -----
--- and biomedical to ensure that all needed connections required for the adequate
installation of the equipment are in place. This shall be planned before the end of the
phase 4 period and shall ensure the needed step for the execution of works.
The consultant shall express his staff list supporting the project team in management of
the mission.
The consultant should have a joint venture with a biomedical consultant (approved by
the client) or at least have in his team design a biomedical engineer (approved by the
client) with at least 10 years experience in hospital design and the following staff:
d. Client authorization
Any staff allocate to the project should have the client (CDR) authorization, the client
have the right to refuse any staff, and the consultant should replace it within 15 days.
APPENDIX C
A - Construction
The consultant shall present a work plan for each task of the services.

Any taxes, fees, stamp duties "Order des lnqenieurs" charges or taxes, or any
other municipality or governmental authorities taxes shall be paid directly by the
Client and are not considered within the fees amount of the Consultant.

WHEREAS, (hereinafter called "the Bidder") has submitted his bid dated
for the supply/construction* of
(1) If the Bidder withdraws his Bid during the period of Bid validity;
or
(2) If the Bidder having been notified of the acceptance of his Bid by the CDR during
the period of Bid validity:
(a) fails or refuses to execute the Contract in accordance with the Instructions
to Bidders, if required; or
(b) fails or refuses to furnish the Bank Guarantee for Good Performance of the
Contract, in accordance with the Instructions to Bidders,
we undertake to pay to the CDR up to the above amount upon receipt of its first written demand,
without the CDR having to substantiate its demand, provided that in its demand the CDR will note
that the amount claimed by it is due owing to the occurrence of one or both of the two conditions
specifying the occurred condition or conditions.
This guarantee will remain in force up to and including the date 148 days after the end of the
deadline for submission of Bids, as it may be extended by the CDR, notice of which extension to
the Bank is hereby waived. Any demand in respect of this Guarantee should reach the Bank not
later than the above date. '

BANK GUARANTEE FOR GOOD PERFORMANCE OF THE CONTRACT

The failure of the Consultants to comply with their contractual obligations shall be advised to us in
writing with a copy to the Consultants.
In case of any claim under this guarantee, payments shall be made to the Council for
Development & Reconstruction - Beirut.
This guarantee shall enter into effect on the date of entry into force of the contract and shall
remain in full force and effect until (inserl date), or until Final Acceptance of the
Services, whichever comes later.

We hereby unconditionally and irrevocably guarantee jointly and severally with the Consultants
and as primary obligator and not as surety merely, to pay the CDR upon its first demand and
without cavil or argument any amounts up to the maximum of , in
the event according to the binding opinion of the CDR, the Consultants would fail to comply with
their contractual obligations.
The failure of the Consultants to comply with their contractual obligations shall be advised to us in
writing with a copy to the Consultants.
In case of any claim under this guarantee, payments shall be made to the Council for
Development & Reconstruction - Beirut.
This guarantee shall enter into effect on the date of payment to the Consultants of the down
payment and shall remain valid until (insert date) or until the payments to the
Consultant reach 60% of the total value of the Contract.
