Please complete this from if you are proposing to transfer a license for an InfoVista branded product from
one licensee to another or one machine to another.

If you are the Transferor (the entity wishing to transfer the product license), please complete Part I.
If you are the Transferee (the entity to whom the product license is proposed to be transferred), please
complete Part II.

Part I: Transferor and Current Licensee Information

I/We am/are the Transferor and current licensee of the licensed product(s) identified below and certify that the
information provided below is correct. I/We am/are proposing to transfer all right, title and interest in the license(s)
I/we possess with respect to the licensed product(s) (including the right to use any prior versions or upgrades) to the
Transferee identified in Part II below. We represent that the Transferee has agreed to be bound by the terms of the
licensed product’s current End User License Agreement. I/We understand that, by signing and submitting this
Software License Transfer Form, we represent that we no longer have any copies of the licensed product in our
possession and control and that all copies have been removed from our computers. Effective as of our signature
below, I/we am/are relinquishing all rights to the licensed product(s) and you are authorized to remove my/our name
from InfoVista’s customer records in connection with the licensed product. If this transfer is being completed on behalf
of a company or other organization, I/we represent and warrant that I/we have the authority to sign this transfer on
behalf of the Transferor.

documentation) and certify that the information in Part 1 and provided below is correct. By signing, I/we confirm that
I/we have read the End User License Agreement included with the licensed product proposed to be transferred and
that I/we agree to be bound by its terms and conditions.
