- The illustrations shown are based on the projected Crediting Rates. These rates are used purely for illustration
purposes to show the variability of non-guaranteed benefits under the different investment scenarios and are not
guaranteed. They do not represent upper and lower limits on the investment performance and are not the actual
returns earned on the actual premiums paid for the life insurance product. The actual Crediting Rate will fluctuate (ie.
rise or fall) monthly based on the performance of the asset investments. A higher credited return may be credited if
the investments have performed well and conversely, a lower or negative credited return may be credited if the
investments have performed poorly. A negative Crediting Rate will result in a reduction of Account Value.
- The investment is classified under the medium-risk category as it diversifies into equities and fixed income securities.
The returns from the investment are not guaranteed and are subject to, but not limited to, market risk, foreign
currency risk and interest rate risk.
- The actual Crediting Rates are NOT GUARANTEED and the past investment performance of the fund is not an
indication of its future performance. The actual Crediting Rates will be determined at the discretion of the
Company.
- Crediting Rate is credited every month into Account 1 and Account 2 and is determined based on investment return
for each calender month after deducting investment tax and Crediting Spread. The Company may determine and
credit the Crediting Rate more frequently than once a month.

Total distribution cost is the amount received by the intermediary for the sale of this policy and services that the
intermediary will provide to you for the duration of your policy. This amount also includes the amount that the
intermediary may be entitled to for the duration of your policy provided that the intermediary meets the performance
criteria set by the company.

(Public Conveyance).

Upon death, you may only receive benefit from either (E) Total Death Benefit, (H) Total Death Benefit (Accidental

IMPORTANT NOTES
1. You should satisfy yourself that this plan will best serve your needs and that you can afford the premium. If you need clarification,
please contact us or the intermediary.

2. This is for illustration purpose only. The Company at its absolute discretion shall determine the terms of acceptance that may be
offered.

3. If No-Lapse Guarantee is forfeited due to non-payment of premium, your policy will lapse / terminate if there are not enough value in
your Account 1 and Account 2 to pay the charges. Your Account Value may not be enough over the years due to:-
- Poor investment performance.
- Non-payment of premiums over a period of time.

4. Account Value
- Account Value refers to the value the policyholder is entitled to from the investment. The Company may invest the assets of the
account in the manner determined by the Company. Income generated from the investment after deducting investment tax and
crediting spread will be credited into the account as credited return.
- The allocated premium (excluding any rider(s) premium and advance premium paid) will be allocated to Account 1. The
Guaranteed Cash Payment and all charges applicable to this policy, eg. insurance charges and policy fees shall be deducted from
Account 1.
- In the event reinvestment option for Guaranteed Cash Payment is selected, the Guaranteed Cash Payment will be allocated to
Account 2. If value from Account 1 is insufficient to cover the insurance charges and policy fees, those charges will be deducted
from Account 2.
- Account 2 Value, if any, can be withdrawn at anytime without any charge imposed. Only full withdrawal is allowed.

5. Prevailing Sum Assured


Prevailing Sum Assured is the (percentage (%) of total Basic Premium paid) less total Guaranteed Cash Payment paid, that is
payable in the event of death or TPD.

6. Premiums Paid
This is the amount that you pay for this policy, excluding any advance premium paid.
- Basic Plan
Not all of the amount paid for basic plan will be invested into Account 1. Please see the explanation on unallocated and allocated
premium below.
- Riders/Supplementary Benefits
Premiums paid for rider(s) will not be allocated into Account 1.

7. Unallocated Premium
- The unallocated premium is an upfront charge on the premium paid and is used to meet the the Company's expenses and total
distribution cost, including the commissions payable to the intermediary.

8. Allocated Premium
- This is the amount of premium for basic plan (excluding any rider(s) and advance premium paid) that will be allocated into
Account 1:

- The allocation rates for basic plan's extra premium is 100% for all years.

9. Charges
The illustrated insurance and other charges are based on current levels. These charges are non-guaranteed and the Company
reserves the right to revise these charges by giving you at least 3 months' advance notification.

i. Insurance Charge
- Insurance charges will be deducted monthly from Account 1. In the event Account 1 Value is insufficient, the insurance charges
will be deducted from Account 2, provided reinvestment option is selected. The insurance charge is for insurance coverage of
death and TPD due to all causes and varies by the attained age next birthday, gender and smoking status, where applicable.
The insurance charges will increase as you grow older.
- For Death Benefit, when the Prevailing Sum Assured is more than Account 1 Value at the beginning of a policy month (after
deducting the monthly policy fee and adding the Allocated Premium (if any) of the month), the difference will be subject to an
insurance charge.
- For Additional Accidental Death Benefit and Additional Accidental Death Benefit - Public Conveyance, insurance charge will be
based on the respective Prevailing Sum Assured.

ii. Surrender Charge
- Surrender charge applies to Account 1 upon surrender or reduction of premium of the basic plan.
- The Surrender Charge scale is as per below:

iii. Policy Fee


- An administration charge of RM7 will be deducted monthly from Account 1. In the event Account 1 Value is insufficient, the
same will be deducted from Account 2, provided reinvestment option is selected.

iv. Crediting Spread


- Crediting spread is 0.9% per annum. It will be deducted monthly from the investment return prior to crediting the credited return
into Account 1 and Account 2.

10. In the case of a collateral policy where the benefit payout (inclusive of non-guaranteed benefits) at maturity is designated for
settlement of a credit facility, please note that the actual maturity payout may not be sufficient to cover the outstanding loan amount
after taking into account the variability of non-guaranteed benefits.

Note: Please read this Product Disclosure Sheet before you decide to take up Great Flexi Plus. Be sure to also read the general terms
and conditions.

1. What is this product about?


Great Flexi Plus is a limited pay universal life endowment plan with 30-year coverage term that provides annual Guaranteed Cash
Payment starting from the end of the first policy year. This product offers a combination of investment and protection against death or
Total and Permanent Disability (TPD).

2. What are the covers / benefits provided?


The Basic Annualised Premium for this plan is RM2,400.00

· Death Benefit
In the event of death of the life assured or in the event of TPD prior to the policy anniversary on which the life assured attains age
70 years next birthday, the following will be payable:
i. The higher of:
a) Prevailing Sum Assured equivalent to (105% of total Basic Premium paid) less total Guaranteed Cash Payment paid; or
b) Account 1 Value; and
ii. Any Account 2 Value.

Payment of TPD benefit is subject to the following conditions:


- If Account 1 Value exceeds Prevailing Sum Assured, 100% of Account 1 Value is payable in one lump sum and the policy will
be terminated.
- If Account 1 Value is lower than Prevailing Sum Assured, Account 1 Value is payable in one lump sum and the excess of
Prevailing Sum Assured over Account 1 Value (Net Sum Assured) not exceeding RM10,000,000 shall be paid in 3 annual
instalments with the first being a lump sum of the Net Sum Assured or RM2,000,000 whichever is lesser, and the balance of
the Net Sum Assured (if any), will be payable in two equal annual instalments. Any further balance of Net Sum Assured
thereafter, shall be payable upon death of the Life Assured or upon maturity of this policy.
- In addition, the Guaranteed Cash Payment reinvested in Account 2, if any, shall be payable in one lump sum.
- The maximum TPD Benefit payable under this and all policies (including riders) on the same life is RM10,000,000 per life
(excluding group policies).

· Additional Accidental Death Benefit


In the event the death of the life assured is due to accidental causes prior to policy anniversary on which the life assured attains
age 70 years next birthday, the Company will pay an additional Prevailing Sum Assured equivalent to (105% of total Basic
Premium paid) less total Guaranteed Cash Payment paid, on top of the Death Benefit.

· Additional Accidental Death Benefit-Public Conveyance


In the event the death of the life assured is due to accidental causes prior to policy anniversary on which the life assured attains
age 70 years next birthday, resulted:
i. while traveling in a public conveyance (other than a cable car, taxi, hired car or any form of transport chartered for private
travel); or
ii. while riding as a passenger in an elevator or electric lift; or
iii. in consequence of the burning of any theatre, cinema, hotel or other public building in which the life assured shall be present
at the time of commencement of the fire;
the Company will pay an additional Prevailing Sum Assured equivalent to (105% of total Basic Premium paid) less total
Guaranteed Cash Payment paid, on top of the Death Benefit and the Additional Accidental Death Benefit.

· Guaranteed Cash Payment


- Guaranteed Cash Payment is payable from Account 1, regardless of the value of Account 1 provided all premiums due and
overdue premium interests are paid up to date.
- Guaranteed Cash Payment will be payable at the end of each policy year in the following manner:

- Two options are available in relation to Guaranteed Cash Payment:
Cash withdrawal - To receive the Guaranteed Cash Payment when it is payable.
- To leave all Guaranteed Cash Payment with the Company to be reinvested into
Account 2.
- Once the Guaranteed Cash Payment is reinvested in Account 2, it will fluctuate based
on the investment performance whereby the value in Account 2 may increase if the

investments have performed well and conversely, it may drop if the investments have
performed poorly.
- The full amount of Account 2 Value can be withdrawn anytime without any charge
imposed.

· Maturity Benefit
Upon survival of the life assured to the maturity date of the policy, the following will be payable in one lump sum:
i. The higher of:
a) (105% of total Basic Annualised Premium paid) less total Guaranteed Cash Payment paid; or
b) Account 1 Value; and
ii. Any Account 2 Value.

If there is any unpaid premium and overdue premium interest upon maturity, only Account Values shall be payable in one lump
sum.

The above benefits are subject to the terms and conditions stated in the policy contract.

Duration of cover: 30 years or upon termination, whichever occurs first.

3. How much premium do I have to pay?


· The estimated premium for this plan that you have to pay: RM 2,400.00 Yearly.
· Premium duration: 20 years or upon termination, whichever occurs first.

Note: The Company allocates a portion of the premium to Account 1. Any unallocated premium will be used to pay commissions to
agents and other expenses of the Company. You are advised to refer to the allocation rates and other details given in the sales
illustration.

4. What are the fees and charges that I have to pay?


· The Insurance charges are deducted monthly from Account 1. In the event Account 1 Value is insufficient, the insurance charges
will be deducted from Account 2, provided reinvestment option is selected. It will increase as you grow older. The insurance
charges are non-guaranteed and vary by attained age next birthday, gender and smoker status, where applicable at each policy
anniversary.
· The policy fee of RM7 will be deducted monthly from Account 1. In the event Account 1 Value is insufficient, the policy fee will be
deducted from Account 2, provided reinvestment option is selected.
· Crediting spread of 0.9% per annum will be deducted monthly from the investment return prior to crediting the credited return into
Account 1 and Account 2.
· Surrender charge applies to Account 1 upon surrender of basic policy or on the partially surrendered amount from Account 1 upon
reduction of Basic Premium.
· The prevailing government service tax will be added to the premium quoted in question 3 above, where the premium is paid by a
business organisation other than the Company.

Note: Charges are not guaranteed and the company reserves the right to revise these charges by giving 3 months’ advance
notifications. Details of insurance charges and other charges for this plan are given in the sales illustration.

5. What are some of the key terms and conditions that I should be aware of?
· Importance of disclosure - you must disclose all material facts such as medical condition, and state your age correctly.
· Free-look period – you may cancel your policy by returning your insurance policy within 15 days after you have received it. Any
Account Value, any unallocated premium, insurance charges and policy fees that have been deducted will be refunded to you.
· Crediting Rate – Crediting Rate is credited every month into Account 1 and Account 2. It is not guaranteed and is determined at
the discretion of the Company. A higher credited return may be credited if the investments have performed well and conversely, a
lower or negative credited return may be credited if the investments have performed poorly. A negative Crediting Rate will result in
a reduction of Account Value.
· Grace period - A grace period of 30 days from each premium due date is given for you to pay your premiums.
· Basic Premium - This is the regular modal premium paid for the basic plan, excluding riders’ premium, any extra premium for both
basic plan and riders’ and any advance premium paid.
· Basic Annualised Premium - This is the total Basic Premium paid for a policy year.
· Non-payment of premium – any premium paid after the grace period will be charged with overdue premium interest at a rate to be
determined by the Company, the prevailing interest rate is available on the Company’s official website. The payment of overdue
premium and overdue premium interest will not be entitled to the credited return which are determined prior to such payment.
· Surrender - Upon surrender of the policy, the Account 1 Value (after deducting the surrender charge) plus any Account 2 Value
shall be payable in one lump sum. Any rider attached shall then be terminated.
· Policy Lapse - subject to No-Lapse Guarantee, the policy will lapse when Account 1 Value and Account 2 Value are insufficient to
pay for the insurance charges and policy fees.
· No-Lapse Guarantee
- The policy will remain inforce even if Account 1 and Account 2 are insufficient to deduct the policy fees and insurance
charges on the monthly deduction date, provided there is no overdue Basic Premium, overdue extra premium and overdue
premium interest. Any unpaid policy fee and insurance charge incurred will be accumulated and recovered from you. No-
Lapse Guarantee feature will be forfeited if there is any overdue Basic Premium and overdue premium interest after the
grace period.
- During the No-Lapse Guarantee period, all benefits under the policy shall be payable.
- The accumulated unpaid policy fees and insurance charges shall be deducted from the following :
i. when the premium is paid;
ii. upon partial or full surrender of Account 1;
iii. upon full withdrawal of Account 2;
iv. from any benefit or claim proceeds payable under this policy; or
v. upon any other transaction made under this policy as may be determined by the Company.

- No-lapse Guarantee is not applicable to riders attached to the policy. Rider(s) will lapse upon non-payment of premium after
30 days grace period, regardless of the Account Value.

6. What are the major exclusions under this plan?


· Death during the first policy year from the risk commencement date or from the date of any reinstatement of the policy, whichever
is later, as a result of suicide, while sane or insane.
· Accidental Death Benefit and Additional Accidental Death Benefit (Public Conveyance) are not payable if death of the life assured
does not occur within ninety (90) days from the date of accident.
· TPD resulting from self-inflicted injuries, while sane or insane.
· TPD existed prior to or on the Risk Commencement Date of the policy or the date of any reinstatement, whichever is later.

7. Can I cancel my plan?


Yes, you may cancel your policy by giving a written notice to the Company. However, it is not advisable to hold this policy for a short
period of time in view of the high initial costs. Buying a life policy is a long-term financial commitment. If No-Lapse Guarantee is
forfeited, your policy may lapse unless your policy has sufficient Account Value. The cash amount that the Company will pay you
when you cancel the policy before the maturity period may be much less than the total amount of premium that you have paid.

8. What do I need to do if there are changes to my / my nominee(s) contact details?


It is important that you inform us of any change in your / your nominee(s) contact details to ensure all correspondences reach you /
your nominee(s) in a timely manner.

- Kadar Kredit sebenar adalah TIDAK DIJAMIN dan prestasi pelaburan dana yang lepas bukan petunjuk bagi
prestasi pada masa hadapan. Kadar Kredit sebenar akan ditentukan mengikut budi bicara Syarikat.
- Kadar Kredit dikreditkan setiap bulan ke dalam Akaun 1 dan Akaun 2 serta ditentukan berdasarkan pulangan
pelaburan bagi setiap bulan kalendar selepas menolak cukai pelaburan dan Sebaran Kredit. Syarikat mungkin
menentukan dan mengkreditkan Kadar Kredit dengan lebih kerap berbanding hanya sekali sebulan.

Kos agihan keseluruhan adalah amaun yang diterima pengantara untuk jualan polisi ini dan perkhidmatan yang diberi

premium. Jika anda memerlukan penerangan, sila hubungi kami atau pengantara.

2. Untuk tujuan ilustrasi sahaja. Syarikat berhak menentukan terma penerimaan yang mungkin ditawarkan mengikut budi bicara.

3. Jika Jaminan Tidak Luput dibatalkan atas sebab premium tidak dibayar, polisi ini akan luput/tamat jika nilai Akaun 1 dan Akaun 2
tidak mencukupi untuk membayar caj yang dikenakan. Nilai Akaun anda mungkin tidak mencukupi akibat:-
- Pulangan pelaburan yang buruk.
- Premium tidak dibayar untuk suatu tempoh masa.

4. Nilai Akaun
- Nilai Akaun merujuk pada nilai yang berhak diterima oleh pemegang polisi daripada pelaburan. Syarikat mungkin melabur aset
akaun dengan cara yang ditentukan Syarikat. Pendapatan yang diperoleh daripada pelaburan selepas menolak cukai pelaburan
dan sebaran kredit akan dikreditkan ke dalam akaun sebagai pulangan kredit.
- Premium diperuntukkan (tidak termasuk sebarang premium rider dan premium yang dibayar terlebih dahulu) akan diperuntukkan
ke dalam Akaun 1. Pembayaran Tunai Dijamin dan semua caj yang berkenaan bagi polisi ini, contohnya caj insurans dan yuran
polisi akan ditolak daripada Akaun 1.
- Jika memilih pilihan pelaburan semula bagi Pembayaran Tunai Dijamin, Pembayaran Tunai Dijamin akan diperuntukkan pada
Akaun 2. Jika nilai Akaun 1 tidak mencukupi untuk membayar caj insurans dan yuran polisi, caj tersebut akan ditolak daripada
Akaun 2.
- Nilai Akaun 2, jika ada, boleh ditarik balik pada bila-bila masa tanpa sebarang caj dikenakan. Hanya penarikan balik penuh
dibenarkan.

5. Jumlah Asurans Semasa


Jumlah Asurans Semasa adalah (peratusan (%) daripada Premium Asas keseluruhan) ditolak Pembayaran Tunai Dijamin
keseluruhan yang telah dibayar, yang boleh dibayar jika kematian atau TPD berlaku.

6. Premium Telah Dibayar


Ia adalah amaun yang anda bayar bagi polisi ini, tidak termasuk sebarang premium yang telah dibayar terlebih dahulu
- Pelan Asas
Bukan semua amaun yang telah dibayar bagi pelan asas akan dilaburkan ke dalam Akaun 1. Sila rujuk penerangan berkenaan
premium tidak diperuntukkan dan premium diperuntukkan seperti di bawah.
- Rider/Manfaat Tambahan
Premium yang dibayar bagi rider tidak akan diperuntukkan ke dalam Akaun 1.

7. Premium Tidak Diperuntukkan


- Premium tidak diperuntukkan adalah caj yang dikenakan terlebih dahulu ke atas premium yang dibayar dan digunakan untuk
memenuhi perbelanjaan Syarikat dan kos agihan keseluruhan, termasuk komisen yang dibayar kepada pengantara.

8. Premium Diperuntukkan
- Amaun premium bagi pelan asas (tidak termasuk sebarang premium rider dan premium yang telah dibayar terlebih dahulu) yang
akan diperuntukkan dalam Akaun 1:

- Kadar peruntukkan untuk premium tambahan pelan asas ialah 100% untuk semua tahun.

9. Caj
Ilustrasi insurans dan caj lain adalah berdasarkan tahap semasa. Caj-caj ini tidak dijamin. Syarikat berhak menyemak semula caj-caj
ini dengan memberikan anda sekurang-kurangnya notis 3 bulan terlebih dahulu.

i. Caj Insurans
- Caj insurans akan ditolak secara bulanan daripada Akaun 1. Jika Nilai Akaun 1 tidak mencukupi, caj insurans akan ditolak
daripada Akaun 2, dengan syarat pilihan pelaburan semula telah dipilih. Caj insurans dikenakan bagi perlindungan insurans
untuk kematian dan TPD akibat semua sebab, serta berbeza mengikut umur hari jadi berikut yang dicapai, jantina dan status
merokok, mana yang berkenaan. Caj insurans akan meningkat apabila umur anda meningkat.
- Untuk Manfaat Kematian, apabila Jumlah Asurans Semasa melebihi Nilai Akaun 1 pada permulaan suatu bulan polisi (selepas
menolak yuran polisi bulanan dan menambah Premium Diperuntukkan (jika ada) bagi bulan tersebut), perbezaan tersebut akan
tertakluk pada caj insurans.
- Caj insurans bagi Manfaat Kematian akibat Kemalangan Tambahan dan Manfaat Kematian akibat Kemalangan -
Pengangkutan Awam Tambahan bergantung pada Jumlah Asurans Semasa masing-masing.

ii. Caj Serahan
- Caj serahan dikenakan untuk Akaun 1 apabila pelan asas diserahkan atau Premium Asas dikurangkan.

iii. Yuran Polisi


- Suatu caj pentadbiran sebanyak RM7 akan ditolak secara bulanan daripada Akaun 1. Jika Nilai Akaun 1 tidak mencukupi, ia
akan ditolak daripada Akaun 2, dengan syarat pilihan pelaburan semula telah dipilih.

iv. Sebaran Kredit


- Sebaran kredit adalah 0.9% setahun. Ia akan ditolak secara bulanan daripada pulangan pelaburan sebelum pulangan kredit
dikreditkan ke dalam Akaun 1 dan Akaun 2.

10. Dalam kes polisi cagaran apabila pembayaran manfaat (termasuk manfaat tidak dijamin) semasa matang telah ditentukan untuk
membiayai kemudahan kredit, sila ambil perhatian bahawa pembayaran manfaat matang sebenar mungkin tidak mencukupi untuk
menyelesaikan amaun tunggakan pinjaman selepas mengambil kira kebolehubahan manfaat tidak dijamin.

Semua amaun yang ditunjukkan adalah dalam RM (Kesalahan & ketinggalan dikecualikan).

1. Apakah produk ini?


Great Flexi Plus adalah pelan endowmen universal life dengan bayaran terhad berserta tempoh perlindungan selama 30 tahun yang
menyediakan Pembayaran Tunai Dijamin (GCP) tahunan bermula daripada akhir tahun polisi pertama. Produk ini menawarkan
kombinasi pelaburan dan perlindungan terhadap kematian atau Hilang Upaya Total dan Kekal (TPD).

2. Apakah perlindungan / manfaat yang disediakan?


Premium Tahunan Asas bagi pelan ini ialah RM2,400.00.

· Manfaat Kematian
Jika kematian atau TPD berlaku sebelum ulang tahun polisi apabila hayat yang diasuranskan mencapai umur 70 tahun hari jadi
berikut, perkara berikut akan dibayar:
i. Mana-mana yang paling tinggi:
a) Jumlah Asurans Semasa bersamaan dengan (105% daripada Premium Asas keseluruhan yang telah dibayar) ditolak
Pembayaran Tunai Dijamin keseluruhan yang telah dibayar; atau
b) Nilai Akaun 1; dan
ii. Sebarang Nilai Akaun 2.

Pembayaran manfaat TPD tertakluk pada syarat berikut:


- Jika Nilai Akaun 1 melebihi Jumlah Asurans Semasa, 100% Nilai Akaun 1 akan dibayar dalam jumlah sekali gus dan polisi
akan ditamatkan.
- Jika Nilai Akaun 1 lebih rendah daripada Jumlah Asurans Semasa, Nilai Akaun 1 akan dibayar dalam satu jumlah sekali gus
dan Jumlah Asurans Semasa yang melebihi Nilai Akaun 1 (Jumlah Asurans Bersih) serta tidak melebihi RM10,000,000 akan
dibayar secara 3 kali ansuran tahunan iaitu bayaran pertama dibayar secara sekali gus bagi Jumlah Asurans Bersih atau
RM2,000,000, mana yang lebih rendah dan baki Jumlah Asurans Bersih (jika ada) akan dibayar secara dua kali ansuran
tahunan yang sama rata. Sebarang baki Jumlah Asurans Bersih selanjutnya, akan dibayar semasa kematian Hayat yang
Diasuranskan atau apabila polisi matang.
- Sebagai tambahan, Pembayaran Tunai Dijamin yang dilabur semula dalam Akaun 2, jika ada, akan dibayar dalam satu
jumlah sekali gus.
- Manfaat TPD maksimum yang dibayar di bawah ini dan semua polisi (termasuk rider) ke atas hayat yang sama adalah
RM10,000,000 bagi setiap hayat (tidak termasuk polisi berkelompok).

· Manfaat Kematian akibat Kemalangan Tambahan


Jika hayat yang diasuranskan meninggal dunia akibat kemalangan sebelum ulang tahun polisi apabila mencapai umur 70 tahun
hari jadi berikut, Syarikat akan membayar tambahan Jumlah Asurans Semasa bersamaan dengan (105% Premium Asas
keseluruhan yang telah dibayar) ditolak Pembayaran Tunai Dijamin keseluruhan yang telah dibayar, bersama dengan Manfaat
Kematian.

· Manfaat Kematian akibat Kemalangan Tambahan – Pengangkutan Awam


Jika hayat yang diasuranskan meninggal dunia akibat kemalangan sebelum ulang tahun polisi apabila mencapai umur 70 tahun
hari jadi berikut, akibat:
i. berada dalam pengangkutan awam (selain daripada kereta kabel, teksi, kereta sewa atau sebarang jenis pengangkutan sewa
untuk kegunaan peribadi); atau
ii. sebagai penumpang dalam lif atau lif elektrik; atau
iii. akibat kebakaran sebarang teater, pawagam, hotel atau bangunan awam lain yang mana hayat yang diasuranskan berada di
situ semasa kebakaran bermula;
Syarikat akan membayar tambahan Jumlah Asurans Semasa bersamaan dengan (105% Premium Asas keseluruhan yang telah
dibayar) ditolak Pembayaran Tunai Dijamin keseluruhan yang telah dibayar, bersamaan dengan Manfaat Kematian serta Manfaat
Kematian akibat Kemalangan Tambahan.

· Pembayaran Tunai Dijamin


- Pembayaran Tunai Dijamin adalah dibayar daripada Akaun 1, tanpa mengambil kira nilai Akaun 1 dengan syarat semua
premium yang perlu dibayar dan faedah bagi premium lampau tempoh dibayar sehingga terkini.
- Pembayaran Tunai Dijamin akan dibayar pada akhir setiap tahun polisi dengan cara berikut:

- Terdapat dua jenis pilihan berkaitan dengan Pembayaran Tunai Dijamin.
Pengeluaran tunai - Untuk menerima Pembayaran Tunai Dijamin apabila ia boleh dibayar.
- Untuk meninggalkan semua Pembayaran Tunai Dijamin dengan Syarikat untuk
dilaburkan semula dalam Akaun 2.
- Apabila Pembayaran Tunai Dijamin dilaburkan semula dalam Akaun 2, ia akan naik
turun berdasarkan prestasi pelaburan iaitu jumlah dalam Akaun 2 mungkin bertambah
Pelaburan semula
jika prestasi pelaburan adalah baik dan sebaliknya ia mungkin berkurang jika prestasi
pelaburan adalah buruk.
- Keseluruhan nilai Akaun 2 boleh dikeluarkan pada bila-bila masa tanpa dikenakan
sebarang caj.

· Manfaat Matang
Apabila hayat yang diasuranskan masih hidup sehingga tarikh matang polisi, perkara berikut akan dibayar dalam satu jumlah
sekali gus:
i. Mana-mana yang paling tinggi:
a) (105% daripada Premium Asas keseluruhan yang telah dibayar) ditolak Pembayaran Tunai Dijamin keseluruhan yang telah
dibayar; atau
b) Nilai Akaun 1; dan
ii. Sebarang Nilai Akaun 2.

Jika terdapat sebarang premium dan faedah bagi premium lampau tempoh yang belum dibayar semasa matang, hanya Nilai
Akaun akan dibayar dalam satu jumlah sekali gus.

Manfaat di atas tertakluk pada terma dan syarat yang dinyatakan dalam kontrak polisi.

Tempoh perlindungan: 30 tahun atau sehingga penamatan, mana yang berlaku dahulu.

3. Berapakah premium yang saya perlu bayar?


· Anggaran premium yang anda perlu bayar bagi pelan ini ialah: RM 2,400.00 Tahunan.
· Tempoh premium: 20 tahun atau sehingga penamatan, mana yang berlaku dahulu.

Nota: Syarikat memperuntukkan sebahagian premium pada Akaun 1. Sebarang premium tidak diperuntukkan akan digunakan untuk
membayar komisen kepada ejen dan perbelanjaan lain Syarikat. Anda dinasihatkan agar merujuk kadar peruntukan dan butiran lain
yang dinyatakan dalam ilustrasi jualan.

4. Apakah yuran dan caj yang saya perlu bayar?


· Caj insurans ditolak secara bulanan daripada Akaun 1. Jika Nilai Akaun 1 tidak mencukupi, caj insurans akan ditolak daripada
Akaun 2, dengan syarat pilihan pelaburan semula telah dipilih. Ia akan meningkat apabila umur anda meningkat. Caj insurans
adalah tidak dijamin dan berbeza mengikut umur hari jadi berikut yang dicapai, jantina dan status merokok, mana yang
berkenaan, pada setiap ulang tahun polisi.
· Yuran polisi sebanyak RM7 akan ditolak secara bulanan daripada Akaun 1. Jika Nilai Akaun 1 tidak mencukupi, yuran polisi akan
ditolak daripada Akaun 2, dengan syarat pilihan pelaburan semula telah dipilih.
· Sebaran kredit sebanyak 0.9% setahun akan ditolak secara bulanan daripada pulangan pelaburan sebelum pulangan kredit
dikreditkan pada Akaun 1 dan Akaun 2.
· Caj serahan akan dikenakan untuk Akaun 1 apabila polisi asas diserahkan atau ke atas amaun serahan sebahagian apabila
Premium Asas dikurangkan.
· Cukai perkhidmatan semasa yang dikenakan kerajaan akan ditambah pada premium yang dinyatakan dalam soalan 3 di atas,
bagi premium yang dibayar oleh organisasi perniagaan selain daripada Syarikat.

5. Apakah terma dan syarat utama yang saya perlu ketahui?


· Kepentingan untuk membuat pendedahan - anda mestilah mendedahkan semua fakta yang material seperti keadaan perubatan
dan umur dengan betul.
· Tempoh memerhati bebas – anda boleh membatalkan polisi dengan memulangkan polisi insurans dalam tempoh 15 hari selepas
menerimanya. Sebarang Nilai Akaun, sebarang premium tidak diperuntukkan, caj insurans dan yuran polisi yang telah ditolak
akan dibayar balik kepada anda.
· Kadar Kredit – Kadar Kredit dikreditkan setiap bulan ke dalam Akaun 1 dan Akaun 2. Ia tidak dijamin dan ditentukan mengikut
budi bicara Syarikat. Pulangan kredit yang lebih tinggi mungkin dikreditkan jika prestasi pelaburan adalah baik dan sebaliknya,
pulangan kredit lebih rendah atau negatif mungkin dikreditkan jika prestasi pelaburan adalah teruk. Kadar Kredit yang negatif
akan menyebabkan pengurangan Nilai Akaun.
· Tempoh tenggang – Suatu tempoh tenggang selama 30 hari daripada setiap premium yang perlu dibayar diberi kepada anda
untuk membayar premium berikutnya.
· Premium Asas – Ia adalah premium modal tetap yang dibayar bagi pelan asas tidak termasuk premium rider, sebarang premium
tambahan untuk pelan asas dan rider, serta sebarang premium yang telah dibayar terlebih dahulu.
· Premium Tahunan Asas - Ia adalah Premium Asas keseluruhan yang perlu dibayar untuk satu tahun polisi.
· Premium tidak dibayar – sebarang premium yang dibayar selepas tempoh tenggang akan dikenakan faedah premium lampau
tempoh pada kadar yang akan ditentukan Syarikat, kadar faedah semasa boleh didapati dalam laman sesawang Syarikat.
Pembayaran premium lampau tempoh dan faedah premium lampau tempoh tidak berhak menerima pulangan kredit yang telah
ditentukan sebelum pembayaran.

· Serahan - Apabila polisi diserahkan, Nilai Akaun 1 (selepas ditolak caj serahan) termasuk sebarang Nilai Akaun 2 akan dibayar
dalam satu jumlah sekali gus. Sebarang rider yang dilampirkan akan ditamatkan.
· Polisi Luput – tertakluk pada Jaminan Tidak Luput, polisi akan luput apabila Nilai Akaun 1 dan Nilai Akaun 2 tidak mencukupi
untuk membayar caj insurans dan yuran polisi.
· Jaminan Tidak Luput
- Polisi akan kekal berkuat kuasa walaupun Nilai Akaun 1 dan Nilai Akaun 2 tidak mencukupi untuk menolak yuran polisi dan
caj insurans pada tarikh penolakan bulanan, dengan syarat tiada Premium Asas lampau tempoh, premium tambahan
lampau tempoh dan faedah bagi premium lampau tempoh. Sebarang yuran polisi yang belum dibayar dan caj insurans yang
dikenakan akan terkumpul dan anda hendaklah membayarnya. Ciri Jaminan Tidak Luput akan dilucut hak jika terdapat
sebarang Premium Asas lampau tempoh, premium tambahan lampau tempoh dan faedah bagi premium lampau tempoh
selepas tempoh tenggang.
- Semasa tempoh Jaminan Tidak Luput, semua manfaat di bawah polisi adalah boleh dibayar.
- Yuran polisi dan caj insurans terkumpul yang belum dibayar akan ditolak daripada perkara berikut:
i. apabila premium dibayar;
ii. semasa serahan sebahagian atau penuh bagi Akaun 1;
iii. semasa penarikan balik penuh bagi Nilai Akaun 2;
iv. daripada sebarang manfaat atau tuntutan yang boleh dibayar di bawah polisi ini; atau
v. jika sebarang transaksi lain dibuat pada polisi ini seperti yang ditentukan Syarikat.
- Jaminan Tidak Luput tidak berkenaan bagi rider yang dilampirkan pada polisi. Rider akan luput jika terdapat premium yang
tidak dibayar selepas tempoh tenggang 30 hari, tanpa mengambil kira Nilai Akaun.

6. Apakah pengecualian utama di bawah pelan ini?


· Kematian dalam tahun polisi pertama daripada tarikh permulaan risiko atau sebarang tarikh penguatkuasaan semula, mana yang
kemudian, akibat bunuh diri, ketika waras atau tidak waras.
· Manfaat Kematian akibat Kemalangan dan Manfaat Kematian akibat Kemalangan Tambahan (Pengangkutan Awam) tidak
dibayar jika kematian hayat yang diasuranskan tidak berlaku dalam tempoh sembilan puluh (90) hari dari tarikh kemalangan.
· TPD akibat kecederaan yang dilakukan sendiri, ketika waras atau tidak waras.
· TPD sedia ada sebelum atau pada Tarikh Permulaan Risiko bagi polisi atau sebarang tarikh penguatkuasaan semula, mana yang
kemudian.

7. Bolehkah saya membatalkan pelan?


Ya, anda boleh membatalkan polisi dengan memberi notis bertulis kepada Syarikat. Namun, anda tidak dinasihatkan untuk
memegang polisi ini dalam jangka masa pendek memandangkan kos permulaan yang tinggi. Membeli polisi hayat adalah satu
komitmen jangka panjang. Jika Jaminan Tidak Luput dibatalkan, polisi anda mungkin luput melainkan polisi mempunyai Nilai Akaun
yang mencukupi. Amaun tunai yang dibayar oleh Syarikat apabila anda membatalkan polisi sebelum tempoh matang mungkin
berkurangan daripada amaun premium keseluruhan yang anda telah bayar.

8. Apakah yang saya perlu lakukan jika terdapat perubahan butiran perhubungan saya / penama?
Anda hendaklah memaklumkan kepada kami jika terdapat sebarang perubahan butiran perhubungan anda / penama bagi
memastikan semua surat-menyurat dapat diserahkan kepada anda / penama dengan sepatutnya.
