Your physical application for the graduate course has been received. The outcome of your application will be made known
to you by post before end of May 2011.
1. Please submit 1 Copy of :
IMPORTANT: (ORIGINAL/CERTIFIED TRUE COPY) University exam results (from 1st year to
final/honours year) / full academic transcript in an official SEALED envelope with its flap bearing the
security seal of the university and the signature of the Registrar or representative sent DIRECTLY
from the issueing university to NUS
If the original is not available, please let us have a photocopy of your University exam results (from 1st
DO year to final/honours year) / full academic transcript for processing in the meantime
Documentary evidence of financial support (eg. bank statement of at least S$15000).
NOT S’pore NRIC / Citizenship Certificate / Passport
/ evidence of Singapore Permanent residence status e.g. entry permit / Employment Pass

Documentary proof of change in name (if you had name changed and your name in NRIC /
