PLAN OF ACTIVITY

(TERMINATION PHASE)

OBJECTIVES:
At the end of 4 hours hospital shift, the student nurses should be able to:

General:
Provide/ Render effective, quality and patient-centered nursing care to the
patients of Adriano B. dela Cerna Hospital Incorporated (ABDCHI) with the
competent application of the nursing process to help clients restore their
holistic well-being and optimum level of functioning through the collaborative
effort of the staff, clinical instructor, student nurses and the patients.

Specific:

• Work toward termination of relationship


• Listen attentively to any instructions given by the clinical instructors
• Gain awareness on the nursing care needs of psychiatric patients
• Apply various techniques in therapeutic communication in dealing with the
patient.
• Observe proper time management and apply universal precautions
• Perform ADL with minimal assistance
• Obtain and monitor vital signs and document accurately and completely.
• Participate in the planned activities
• Commend client’s strong points and improve weak points to continually
make improvement in planning care.
• Develop a realistic plan of action
• Express feelings of both patient and nurse.
• Develop the skills, knowledge, and attitude of good human relationship in
care of psychiatric patients.
• Provide holistic care to the patient

TIME ACTIVITY RATIONALE


05:30am Pre-conference at Mary Pre-conference are given to organize the group.
Mediatrix Inn This activity is important in order to organize and
delineate tasks in the group, give those important
reminders and run through the important details
with regards to the duty. This is also to facilitate
coordination and efficiency of tasks in the area..

• Checking of To check the presence of each member of the


Attendance group and brief them of their duties and
responsibilities and make sure that the work are
properly disseminated.

• Checking of uniforms/ The uniform is an important symbol of the nursing


paraphernalia, and profession. It signifies the person as a health care
materials giver and it should be check by the clinical
instructor including the uniform to be identified
and be respected by clients and other health care
givers. Thus it should be complete, clean and
tidy. Its completeness indicates preparedness of
the group member. To check the completeness of
nursing paraphernalia so as to be used for the
continuity of nursing care to the patients.
Ensuring completeness of materials, supplies and
things is needed to carry out duty effectively and
to avoid delay in carrying out activities and task.
• Patient assignment This is done to enable the student nurse to have
general information about the type of psychiatric
client he/she would be taking care of and to
develop the appropriate interventions in handling
the client. Also, to establish and gain trust and
rapport of the patients so as to minimize their
anxiety and will give way in rendering care.

• Objective-setting Discussion of objectives/goals is necessary so


that student nurses are able to be guided
accordingly and would serve as a basis for
evaluation of student’s performance.

• Orientation to the Student nurses should be oriented to the do’s and

hospital setting don’ts in the hospital they are having their duty.
This is part of the professionalism of becoming a
nurse and also to prevent from having mistakes
by not following the hospital rules and regulations.

This is for the Clinical Instructor to give specific

06:00am Departure from the dorm arrive in the area.

06:30am Arrival at Adriano B. dela Punctuality should be an innate characteristic of a


Cerna Hospital Incorporated student nurse. As student nurses, we are
(ABDCHI) expected to arrive at the assigned area 30
(wake-up time for patients in minutes prior to start time. In this way, student
ABDCHI) nurses are able to prepare themselves physically
emotionally, intellectually and are able to organize
plans towards the things/ activities they intend to
do.

06:45am Morning Prayer To ask for Almighty God’s guidance for the day’s
duty, as his instrument of healing and loving
touch and to thank God for another day of
opportunity and blessing. His presence is needed
for the group to perform better and to avoid
committing errors that may harm the patient’s
health status

06:50am Introduction to the client Indicating the remaining days of encounter is very
important most especially to the psychiatric
clients because of the possibility of depression of
the client during the termination of relationship.
Also, this is to present the reality to the client.

07:00am Assist with the ADL This is for the patient to suffice their need for
• Bathing the patient adequate and proper nutrition (Physiologic need).
• Grooming the patient This is also an opportune time for the SN to
observe behavior of the client and gather
assessment data. This would not only help the
SN but also would improve
relationship/interaction to the client.

07:30am Breakfast of the patients This establishes baseline data necessary for
health management and monitoring of patient’s
condition. Some psychiatric patients are given
with medications aside from therapies to promote
recovery from mental illness.

08:00am Vital Signs Taking and Giving This establishes baseline data necessary for
of Medications health management and monitoring of patient’s

08:20am Celling out Ceiling out of patient is part of the ADL before the
activity starts in each phase. Patients are sent to
the activity area for the activities with the student
nurses.

Activity for each phase This is the meat of everyday encounter. Activities
(see separate paper) are especially designed for the patients in order to
promote self-worth and bring them back their
confidence. With these activities, SNs are able to
build trust with the client. It is the nurse’s
responsibility to establish a therapeutic
environment that fosters trust and understanding
(Videbeck, 2004). Activities are designed to
directly involve the patient and for them to know
more about their care providers as well as other
patients.
09:45am Nurse- Patient Interaction This part is very necessary to fully understand the
(NPI) condition and the story of the patient. The ability
to establish therapeutic relationships with client is
one of the most important skills a nurse can
develop. This is crucial for the success of
interventions requiring psychiatric care because
of the therapeutic relationship and communication
within it (Videbeck, 2004).

10:30am Vital Signs Taking & This gives significant data necessary for health

11:00am Celling in This is part of the usual routine in the hospital.

activities.

11:30am Post Conference and The very purpose is to evaluate if objectives are

Objectives
To gather pictures from the day 1 of patient’s interaction
To compile pictures in a large picture frame
Procedure/Mechanics
The student nurses will take pictures from day 1 of patient’s interaction until the
termination phase
The student nurses together with the patient will paste the different pictures in a
large frame
The student nurses will express their feelings about their experiences in their
patient.
The patient will also try to reflect and expresses their experiences with the
student nurses.
