DIAGNOSIS OF THE WEEK

STOMATITIS
Inflammation of the mucous lining of any of the structures in the mouth, which
may involve the cheeks, gums, tongue, lips, and roof or floor of the mouth.

It is a very common sign of one or more of the following cause-


• Poor oral hygiene
• Dietary protein deficiency
• Poorly fitted dentures
• From mouth burns from hot food or drinks
• Toxic plans
• Conditions that affect the entire body, such as medications, allergic
reactions, radiation therapy, or infections.
• Severe iron deficiency anemia can lead to Stomatitis.
• Deficiency of vitamin B2 (Riboflavin), B3 (Niacin), B6 (Pyridoxine), B9
(folic acid) or B12 (Cyanocobalamine).
• Gastro intestinal disturbance

Prognosis
The prognosis for the resolution of stomatitis is based on the cause of the
problem. Many local factors can be modified, treated, or avoided. Infectious
causes of stomatitis can usually be managed with medication, or, if the problem
is being caused by a certain drug, by changing the offending agent.
Prevention
Stomatitis caused by local irritants can be prevented by good oral hygiene,
regular dental checkups, and good dietary habits. Problems with stomatitis
caused by systemic disease can be minimized by good oral hygiene and closely
following the medical therapy prescribed by the patient's health care provider.

HOMOEOPATHIC APPOACH

Borax
This is the great remedy in mouth affections. It has aphthous inflammation of the
mouth; the mouth is very hot and dry, mucous membrane bleeds easily; the
thrush is more often located on the inner surface of the cheeks, although also on
tongue and fauces. Offensive breath, puffy, indented tongue.

Nitric acid
Acrid saliva, pricking, splinter-like pains, aphthae and whitish gums, raw places in
the mouth, corners of the mouth excoriated, the gums are unhealthy, bleed easily
and have a foul odor. This remedy has deep bluish ulcers in the mouth having
dark edges; the mucous membrane is denuded. The salivary glands are swollen
and tender.

MERCURIUS SOLUBILIS
Sweetish metallic taste. Salivary secretions greatly increased; bloody and viscid.
Saliva fetid, coppery. Speech difficult on account of trembling tongue. Gums
spongy, recede, bleed easily. Sore pain on touch and from chewing. Whole
mouth moist. Crown of teeth decay. Teeth loose, feel tender and elongated.
Furrow in upper surface of tongue lengthwise. Tongue heavy, thick; moist
coating; yellow, flabby, teeth-intended, feels as if burnt, with ulcers, Fetid odor
from mouth, can smell it all over room. Alveolar abscess, worse at night. Great
thirst, with moist mouth.

ARSENICUM ALBUM
Unhealthy, easily-bleeding gums. Ulceration of mouth with dryness and burning
heat. Epithelioma of lips. Tongue dry, clean, and red; stitching and burning pain
in tongue, ulcerated with blue colour. Bloody saliva. Neuralgia of teeth; feel long
and very sore; worse after midnight; better warmth. Metallic taste. Gulping up of
burning water.
REMEDY OF THE WEEK

POTHOS FOETIDUS

• For Asthmatic complaints, worse from inhaling any dust. Millar’s Asthma.
• Hysterical patients. “Will- O’- the wisp”* like character of subjective
symptoms.
• Has a marked action on Glabella. Patient has a sensation of outward
drawing from Glabella. There is red swelling across the bridge of the nose.
• Headache in Single spots, with violent pulsation of temporal arteries.
• Inflation and tension in Abdomen.
• Absent minded, Irritable.
• Sneezing with pain in throat. Troublesome respiration, with sudden feeling
of anguish and sweat.

NOTE – “Will-O’-the-wisp” is a term used for-


• Ghostly light sometimes seen at night or twilight over marshes or forest
like areas. It resembles flickering lamp which recedes on approaching.
• Also considered to a person or thing that is elusive/ alluring/ misleading}

CASE OF THE WEEK

Allergic Rhinitis with a long CONCENTRATED GRIEF

A patient aged 48 years came to my clinic on 7th Jan. 2011 with complaint of
body ache & running and watery nose since past 3 days. His complaint started
after exposure to cold air while he was riding pillion on bike. After a short case
taking I prescribed him HEPAR SULPH 30/ TDS/ 3Days. & asked him to report
me after 3 days. He came on 4th day to inform me that he felt only a bit better,&
that his body ache was better but discharge from nose wasn’t improved much.
I gave him ACONITE 30/TDS/3Days, after which he reported me that agin his
discharge wasn’t ameliorated. He continued such short visits to my clinic & I
considering his case to be another simple case of Coryza gave him all the acute
& specific medicines and never did the proper case taking.

This continued for approximately 12-15 days, during the course of which his
condition stood the same & showed no improvement. Meanwhile he also visited
an ENT Physician who diagnosed his case to be of Allergic Rhinitis. After 4-5
visits when his condition didn’t improved I refreshed my mind & started with
proper case taking.
Patient told me that this complaint of running & watery discharge from nose was
a regular complaint of his in every winter. He had occasional spell of headaches
also. Nothing significant was found in past & family history. His appetite was
good, stool clear & he had a good thirst. When I asked him about any kind of
mental stress he at 1st hesitated & tried to avoid by saying that everyone has
stress these days, but when I asked him again he told me that his wife was
mentally challenged & is of a very aggressive nature, she often beats their
children & abuses them. He said, he just can’t do anything to help her & is too
much depressed with the condition at his home. He hasn’t shared this with
anyone yet & feels that there is no point in sharing this with anyone as no one is
going to help him.

Taking his life situation in consideration, I prescribed him a dose of IGNATIA 200,
followed by SL 30 for 7 days. On the 8th day he entered my clinic with a big smile
telling me that his discharge was much better now & he had no spell of
headache. I repeated PLACEBO for another 7 days.

Till a week back patient came to me for medication, his complaint was almost
over & he felt much better.

BONE MARROW SCAN—DIAGNOSTICS

Normal: Even concentration of the radionuclide throughout the reticuloendothelial


system, red blood cells and bone marrow.

Usage: Assists in diagnosis of defects of bone marrow or bone marrow


depression after chemotherapy or radiation in the differential determination of
myeloproliferative disorders. Differentiates acute from chronic haemolysis and
bone infarction from osteomyelitis in sickle cell disease. Aids in the selection of
bone marrow biopsy site and in the staging of Hodgkin’s disease, lymphomas
and metastatic disease of the bone marrow. Assists in the evaluation of the
hyperplasia of the bone marrow associated with the chronic haemolytic anemia
and polycythemia vera.
Description: bone marrow scan is a nuclear medicine study in which the
radionuclide indium chloride is administered intravenously and followed by
radiographic imaging of the entire body. This scan may be nonspecific in
conditions of diffuse disease such as osteomyelitis and tumor.

Risk: Hematoma at the injection site

Contraindication: During pregnancy & breast feeding

MAGIC OF LAUGHTER

ANATOMY AND PHYSIOLOGY OF LAUGHTER


Laughter is an expression of human behavior, regulated by the brain. The limbic
system is a primitive part of brain that is involved in emotions and help us with
the basic functions necessary for survival. Two structures in the limbic system
are involved in producing laughter and these are amygdale and hippocampus.
If we analyze the anatomy and physiology of laughter, it has been well
documented that our feelings either negative or positive, converted into
neuropeptides or messenger molecules, which influence every cell in our body.
When we laugh, then our positive feelings are converted into chemicals into
chemicals such as endorphins, which are neurotransmitters that attach to the
same receptors in our brain as the opiates. Opiate drugs not only cause us to
lose touch with reality, but they also numb pain. When we laugh, it releases the
endorphins in our brain, which has healing influence on our system.
Laughter like any exercise has 2 phases, in the arousal phase - the heart beat
increases and the resolution phase-the heart rate decreases.
