Explanatory Notes
This Logbook is intended to be a record of all operations you perform, or participate in, as part of your training in Neurosurgery. On
the following pages you will find a Training Record, Operation Log, Consolidation Sheets, and a Final Consolidated List of
Operations Performed.
In the Training Record enter your Name, Training Number and a list of the posts you have held in Neurosurgery in chronological
order.
In the Operation Log enter the date, hospital number, patient name, age and the nature of the operation. The last column should be
used to designate your role in the operation. Please enter the page number and ask your consultant to sign the sheet.
After fairly extensive discussions, the Joint Committee on Higher Surgical Training has now decreed that the categories of supervision
described in the trainees logbooks would be standardised for all specialties. These would be amended as follows:
P Performed by yourself. The consultant is available but not in the operating theatre
SU Performed with a consultant on standby in the operating theatre
SS Performed with the consultant present, scrubbed
T Training a more junior trainee
A Assisting a consultant
NB Although the term “consultant” is used above, the categories SU, SS and A also apply when a more senior trainee is present.
The trainee is expected to show a good spread across the surgical groups and many cases formerly classified as P would become SU or
SS.
Use the Consolidated Sheets to keep a continuous record of the numbers of the operations you have performed and present these
sheets at your regular training reviews. The tick boxes are grouped according to the codes P.SU/T and SS and A. Use a different mark
as recommended to differentiate between the three, mark the boxes as you fill out the Operation Log. Complete the final Consolidated
List when you submit your logbook to the SAC for recommendation of completion of specialist training.
For purposes of the logbook paediatric patients are under 16 years old.
