Membership Application

When you arrive at INIQUITY for the first time, you must sign a copy of this
application and show a valid driver's license or government-issued ID.
You may print this agreement and fill it out ahead of time
to save time at the door.
INIQUITY (A Private Club) MEMBERSHIP APPLICATION
ALL INFORMATION PROVIDED IS KEPT STRICTLY CONFIDENTIAL
This is an Application to become a member of the private club
INIQUITY. Although sexual activity is not allowed at INIQUITY, INIQUITY
is a unique private club for couples and single women involved in the
"lifestyle". If any type of nudity or sexual activity offends or may
potentially offend you, you should not become a member of
INIQUITY.

In addition to the membership fee, there will also be a user fee (door
charge) for every time that a member uses the Facilities. The
membership fee schedule along with the user fee schedule is posted
online at http://www.iniquityclub.com, and is subject to change without
notice. MEMBERSHIP AND USER FEES ARE NON-REFUNDABLE
except as provided below.

By becoming a member you agree that you will follow all of the rules
and regulations of the club. Should you desire to become a member of
INIQUITY, you should first read the rules and regulations below and
initial if you agree to abide by them. Further, you acknowledge that
there are members of staff available at INIQUITY who you may ask
questions of in order to clarify and questions, thoughts or curiosities you
may have. In addition, before becoming a member, you also have the
option of having one of the staff give you a full orientation as to the
nature, purpose, rules and regulations of INIQUITY.

You also understand that by signing this application, by paying your

membership fees, you are not becoming a member until the
membership committee has formally approved you. Therefore, you will
be allowed into INIQUITY upon signing the application and paying the
appropriate fees as a temporary member for that night only. During the
next week the membership committee of INIQUITY will meet to discuss
your becoming a permanent member. Should INIQUITY decide that you
are inappropriate to become a member, then INIQUITY will refund the
unused portion of all membership fees paid to INIQUITY if you request
it in writing, and you agree that INIQUITY shall retain all user fees paid
in exchange for your temporary admittance and for the costs of
processing your application.

RULES AND REGULATIONS

1. "No" means "No". This is a private club for enjoyment of both men
and women alike. No one has the right to disregard any other person's
wishes. Therefore, should anyone decline your invitation or overtures,
you shall respect that "No" and you shall not bother, annoy, harass, or
persist.
2. No Prostitution. No Illegal Drugs. No Guns or Weapons of Any
Kind.
3. No Cameras, Video or Audio Equipment without permission of the
management.
4. No Inappropriate Conduct. As with "No means No" as a member of
the private club INIQUITY you agree to respect other members wishes
for privacy and enjoyment. Therefore, you shall not engage in any
inappropriate touching or non-consensual touching, nor will you behave
in a manner that is disruptive to others enjoyment. Loud, obnoxious,
and inconsiderate behavior will not be tolerated. Should you act in an
inappropriate manner as determined by management or should
management receive complaints about your behavior that is verified by
management, which management deems to be inappropriate, then you
will be immediately asked to leave the premises. Should this occur more
than one time then your membership will be revoked.
5. You must be over the age of 21 to become a member of INIQUITY.
6. Your membership is Solely for Yourself. Anyone caught allowing
another person to use his or her membership or to pose as him or her
for the sake of entry without being a member will have his or her
membership revoked.
7. Orientation. Should you have any concerns as to what might take
place or what might be viewed within the private areas inside INIQUITY,
then before becoming a member, you should ask management any
questions or concerns you might have. Should you not take the step in
asking any questions you might have as to the nature of this club, then
it is assumed that you know the nature of the club and approve of the
same.
8. Limited Space. Should INIQUITY upon your arrival be filled to
capacity, you understand that regardless of the fact that you are a
member you will not be allowed to enter and said inability to enter shall
not be grounds for any refund of membership fees.
9. Membership. No one shall be allowed entry without a membership,
as verified by our computer.
10. Alcohol. INIQUITY is a BYOB type of private club. No alcohol will
be sold on the premises. Rather, should you desire to drink alcohol
you may bring said alcohol with you to the premises, but no coolers are
allowed.
11. Sexual activity is not permitted on the premises.

The undersigned swear(s) or affirm(s) that:

1) All identifying information provided by the undersigned on the

attached form is accurate; 2) The undersigned applicant(s) do/does not
serve in a capacity as a law enforcement officer, investigator, inspector,
agent employee or informant of any municipal, county, state or federal
governmental agency or department; In the alternative the undersigned
affirm(s) that such status has been duly and fully disclosed in writing on
this form; 3) The undersigned applicant(s) is/are not a reporter,
employee, investigator, independent contractor or informant associate
with, assisting or employed by any broadcast, print, Internet or other
news or entertainment media nor any organization which disseminates
information in any way to the general public; in the alternative the
undersigned affirm(s) that such status has been duly and fully disclosed
in writing on this form; 4) The application of the undersigned for
membership is made in good faith for the sole purpose of obtaining
Membership at INIQUITY and participating in events hosted or
supported by INIQUITY and not for any other purpose.

Under penalty of Perjury, I hereby state that I wish to become a

member of the private club INIQUITY. I further acknowledge that I have
read this application form in its entirety along with the rules and
regulations above. I hereby agree to abide by the terms of the
application and the regulations of INIQUITY and unequivocally state that
no form of nudity or sexual activity offends me or will offend me and
that it is my desire to become a member of INIQUITY.
