Why Biolase is the way forward?
As a dental implantologist and
orthodontist who strives to be at the
forefront of advances in the dental
profession, I have long been interested
in the use of lasers in dentistry, but until
recently their application has been
somewhat limited.
Rapid advances in their development is
likely to see lasers becoming an
integral, everyday tool in dental
surgeries as laser technology enters a
whole new era. Lasers reduce the need
to use high speed drills on the surface
of teeth and can remove enamel,
dentine and decay as well as cutting
soft tissue around the tooth in the time it
currently takes for anaesthetic to numb
the site.
Despite their many advantages,
dentists are still unable to replace drills
with lasers in their practices because
they cannot as yet cut surfaces such as
metal and porcelain. That said,
research is moving on a pace and it
will not be long before their range of
applications has advanced to an extent
where they can be utilised to undertake
these procedures both efficiently and
cost-effectively.
Originally developed by Theodore
H Maiman using a theory devised by
Einstein, lasers were reportedly first
used in oral surgery in 1977, but the
dental laser revolution did not get
underway until the mid 1980s. The
arrival of the D-Lase 300 – a laser
system invented by Dr. Terry Meyers
and his brother William – led to the
establishment of American Dental Laser
which became part of market leading
laser company Biolase six years ago.
The laser system which involves laser
energy, water and air being combined
to safely remove enamel and dentin,
was cleared for use on both adults and
children in 1988, allowing Biolase to
sell and market its product globally.
The benefits to dentists and patients are
myriad, given the ease with which
decayed matter can be removed and
teeth and soft tissue cut. The analgesic
advantages are also welcome, as
lasers save time in waiting for the local
anaesthetic to take effect and, from the
patient’s perspective, sometimes negate
the need for injections – a procedure
which most people dislike and fear.
To find out exactly what lasers are
now capable of and how their
introduction could benefit my own
practice, I recently attended a two-day
intensive course at dentist Mark
Cronshaw’s practice in Cowes on the
Isle of Wight.
Making the journey truly was a case of
‘trains, boats and automobiles’ and
after a scenic walk from the pier I
arrived at the New Holmwood Hotel -
a comfortable three star Best Western
hotel offering panoramic views across
the Solent.
Mark’s runs a holistic dental practice
and has been using Biolase for the
past 10 years. In that time he has
amassed a wealth of experience.
The course featured an excellent
presentation, practical demonstrations
and slide shows.
Because there were only three of us in
attendance we received one to one
tuition and saw for ourselves how

efficient the laser
was. The laser light
was focused on a
fine beam of water
and the water
molecules expanded 1600 times
to create mini explosion creating a
cavity. This felt totally different from
using a drill and wearing loopes was
essential. However, within a few hours
we had all mastered the technique.
Our practical assignment involved
cutting teeth and soft tissue on a pig’s
head. We also learned how to service
the equipment which is essential if you
are to maintain your laser in good
condition. Additionally, we each
received a folder containing well
presented, user friendly material about
Biolase to take away with us.
There is no doubt that Biolase offers
many advantages including cutting
soft tissue, cleaning and sterilising
canals during endodontics and
periodontal procedures. The new
revolutionary turbo head cuts teeth very
efficiently, leaving a neat cavity.
Implementing laser technology
represents a major investment for
surgeries and the full Biolase package
costs approximately £50,000.
Although a less expensive version for
use in soft tissue cutting and whitening
teeth is available for £8,000.
All in all, the Biolase course proved
worthwhile and I would highly
recommend it to other practitioners who
are keen to learn more about the laser.

How does the Waterlase MD™
Turbo work?
The Waterlase MD™ Turbo is a
significant breakthrough in dental
laser optical technology.
A contra angle handpiece with a
computer designed optical tip
replaces the traditional handpiece
and its fiber tip. The optical tip is
capable of focusing the laser beam
to a 500- or 700-micron diameter
spot size, depending on which of
two available Waterlase MD™ Turbo
tips is used (MX5 - 500 microns,
or MX7 - 700 microns). The beam
maintains this spot diameter for a
distance of approximately 1 - 3 mm
at a distance of about 3 - 5mm from
the tip providing maximum cutting
efficiency across that distance,
for fast and effective Classes I - V
cavity preparations.
What procedures can be
performed using the Waterlase
MD™ Turbo system?
The Waterlase MD™ Turbo system is
indicated for various hard and soft
tissue procedures that do not require
tissue contact or access through
tissue to reach the site, such as
Class I - V cavity preparations,

lengthening. It assists in performing an
externally beveled gingivectomy,
shaping the free gingival margin,
troughing, and recontouring and
smoothing bone.

Lasers in oral implantology
Instrumentation for dental implant
procedures has not changed
significantly over the last few decades.
Most of these procedures share one
parameter in common; they are
primarily resective in nature. When
tissue is traumatised, it goes through an
inflammatory cascade which results in
edema, erythema and discomfort for the
patient. A period of tissue remodelling
then occurs in which the production of
matrix metallo-proteinases
(collagenases, elastases, and
gelatinases) occurs. These enzymes are
the primary inducers of both soft and
hard tissue remodelling. Crestal bone
remodelling around implants is one
result of the action of these compounds.
The loss of peri-implant osseous support
may cause the collapse of the
interdental papilla, resulting in
compromised gingival display in the
aesthetic zone.
A paradigm changing modality has
recently become available for use in
oral implantology. The Er,Cr;YSGG
laser (Waterlase, Biolase Technologies)
has been used to treat peri-implant
tissue for the past 8 years. The
hydrophotonic properties of the
2780nm wavelength targets tissue
along a cell layer rather than in a tissue
zone. Studies indicate a cellular action
zone of only 8-15 microns, leaving
adjacent tissue undisturbed. This results
in pure tissue ablation rather than
coagulation. The absence of tissue
damage eliminates the precursors of the
inflammatory cascade that may affect
tissue as far as 10mm from the surgical
site. Tissues can then go directly to
regrowth and regeneration.
Peri-implant tissue differs considerably
from periodontal tissue. There is no
direct attachment of periodontal fibres
to the implant surface. In addition, there
is a larger volume of collagen and
smaller number of fibroblasts in peri-
implant tissue. This makes the reparative
potential much lower for peri-implant
tissue. When the hemi-desmosomal
attachment to the neck of the implant is
disturbed, it tends to scar down rather
than reattach to the previous position on
the implant surface. Secondary
uncovering of the implant, whether by
flap or tissue punch, removes the
healed attachment apparatus. The
attachment is reestablished more
apically, which ultimately results in
crestal bone remodelling. Healing time
for this pathway may be several
months. If final impressions are taken
prior to full tissue maturity, gingival
recession may occur leading to uneven
gingival margins and loss of papillary
form. The stability and health of this
attachment apparatus is of prime
concern in the aesthetic zone.

“I first learned about the Waterlase
MD™ on the internet and I was intrigued
to see first hand this apparently
amazing machine. The claims made by
the manufacturer seemed scarcely
credible as it was billed as an all tissue
laser with the ability to cut cavities and
treat decay without local anaesthetic in
the majority of instances. I travelled to
the annual meeting of the ADA in Las
Vegas in November 2006 and was
sufficiently impressed to order a unit
which duly arrived just before Christmas
of that year. At that time mine was one
of the very first units in the UK and I had
to rely on distance learning for
instruction on how to use the unit.
The Waterlase MD™ had been is use in
the States since 2002 and at that time
there were around 3,500 users so there
was a wealth of clinical experience I
could draw on. The experience and
enthusiasm of our American colleagues
is tremendous and following my initial
visit to the USA I have been back time
and again. As a mature practitioner of
25 years this year it has helped me to
stay fresh, motivated and very
interested. My clinical experience of the
laser was initially confined to relatively
simple procedures such as gingivectomy
and cavity preparation. I discovered
that I could indeed perform some
procedures without a local anaesthetic
however it was a slow business
compared to a conventional turbine
and there were still occasions when I
had to resort to placing a local.
More recently a new handpiece has
been introduced which cuts as fast as
an airotor and I am better able now to
use the laser rather than a drill for many
more clinical procedures. As my
experience developed and as I took
further training my clinical proficiency
increased and I started to widen the
range of clinical procedures I was
happy to offer. I discovered that
osseous crown lengthening can often
be achieved without the need to raise a
flap and that the healing and tissue
recontouring was so predictable and
reliable that the definitive crowns or
veneers could be placed without having
to wait. As a CEREC® user I took to
both doing the osseous surgery and
placing the definitive crown all on the
same day with excellent results.
Patients reported little other than minimal
discomfort post surgery and for
aesthetic and cosmetic procedures
involving tissue recontouring the laser is
easy to use, reliable and predictable
with outstanding clinical results.
More recently I have used the laser as
an additional step in my endodontics as
there are new special radial firing tips
which help debride and sterilise root
canals. The clinical results have proven
to be outstanding and I frequently see
filled lateral canals on my post treatment
radiographs. In vitro research
demonstrates that the laser disinfects the
interior of a root canal to a depth of
1mm which is ten times more effective
than bleach alone. I have recently
returned from an advanced laser
assisted periodontal surgery course
where I learned tissue grafting
techniques along with bone and
connective tissue regeneration
treatments. Also I have been taught the
Annapolis method of laser assisted
periodontal surgery which is proving a
very successful treatment modality for
the management of moderate to
advanced periodontal disease. As with
all laser treatments post operative
discomfort and sensitivity is minimal in
sharp contrast to the difficulties
associated with the conventional
approach. Again thanks to the lasers
mode of operation these procedures
are clean, simple and reliable and I
have found myself taking on a much
expanded range of effective treatments
many of which are historically the
province of specialised practitioners.
The response from my patients has been
overwhelmingly positive and I have
found myself enjoying my dentistry more
than ever before. In my opinion the
Waterlase MD™ is the biggest single
advance in dentistry I have seen during
the course of my career. My confident
expectation is that within the next
decade the majortity of UK dentists will
wish to integrate the laser into their
clinical practices. Over the next few
issues I intend to present some treated
clinical cases looking at examples of
laser assisted surgical extractions,
crown lengthening surgery,
the Annapolis laser assisted periodontal
surgery regime and frenectomy. In the

Waterlase: One small step for me, one giant
leap for dentistry - week one
I can honestly say my first week alone
with my Waterlase I was terrified - not
of doing the dentistry which was fine
but taking care of the machine itself
initially frightened me to death.
However I faithfully followed the
training I had with Jackie Cooper
(the Henry Schein Minerva Laser
Specialist) and Mark Cronshaw and so

The truth is it is a bit like being given a
formula one racing car to drive when
all you are really used to is a standard
family hatchback. Luckily after a few
crunched gears I have managed to get
around the first lap and in my opinion
with great success. I have already
completed three flapless osseous crown
lengthening procedures with amazing
results and what some may find of most
interest is that one patient was actually
a referral (in my opinion the laser offers
countless possibilities not only to
upgrade you own work but to attract
other dentist’s high fee earning
procedures such as gum lifting and
pre-implant extractions (to name just

I’d like to keep you informed of just
some of the uses I have found for the
Ezlase. Gingival troughing for fixed
prosthodontics (including cerec) has
become significantly easier and faster,
resulting in increased profitability.
The Ezlase is also outstanding for cold
sore treatments. This type of therapy
generally takes me 5 minutes to
complete and patients are generally
happy to pay a decent fee to prevent
the discomfort and social
embarrassment of the virus.
Certainly the greatest impact has been
on our periodontal program. I am
using the Ezlase in conjunction with
root surface debridement therapy, and
to say that we are having outstanding
results is an understatement. We have
seen bleeding scores reduced from
73% to below 10% in record times.
We have also seen mobilities reduced
from grade III to grade I in as little as 3
weeks, as well as pocket depth
reductions. Patients are outstanded as
to the the improvement in gingival
health and the speed at which it
occurs.

carried out a very neat gingivectomy to
align the gingival, and then ran the
laser 3mm into the sulcus under direct
vision with loops.
The final stage was to help the healing
process with a few minutes of laser
“band-aid” and then root desensitation
treatment. The whole procedure took
about an hour and a quarter but will
definitely get quicker with practice.
Every one of the patients could see the
immediate benefits and all three
experienced only mild after pain the
same day of the procedure and one did
not require painkillers at all. Healing in
all the cases has been very rapid in fact
about three times faster than
conventional surgery.
This means of course from the clinicians
point of view that the restorative phase
can be done far quicker. In my
experience patients get impatient
waiting 12 weeks plus for conventional
surgery to heal. The second major
benefit of the laser is that you have total
control of the end result since as the

Cronshaw on the Isle of Wight. The two
day event began with coffee and
croissants and a very friendly
introduction to the other delegates.
The course order was logical, easy to
digest modules interspaced with short
practical hands on sessions. This is
where the course excelled.
With a one to one mentor, Mark was
there to guide my inexperienced hands
(albeit on animal jaws) and deliver that
wow factor when hard dental tissues
were being cut using the
Waterlase MD™.

followed by the opportunity to send in a

I have since invested in the Ezlase

systems but the laser offers quick
efficient cutting in a bloodless field.
