Together, the NFL Alumni Association and The Schwartzberg Companies have
teamed up to provide NFL alumni and their families a unique program that offers a
full spectrum of Rehabilitation Services and Long Term Care.
OUR PROGRAM
Given the breadth of our organization, we have been able to establish a program that
is tailored to the specific needs of the NFL Alumni Association and its members.
The program includes:
• Specialty care units within our facilities,
• A newly constructed designated facility that will focus on the needs of the NFL Alumni
Association,
• Brain trauma and dementia programs
• Specially designed rehabilitation programs for the unique needs of retired athletes and
• Outreach programs for retired players to make sure they are not slipping through the
cracks.
As part of the program, our team of specialists will work with you to guide you
through the “ins and outs” of the health care and insurance systems.

• Guide you through the Insurance, Medicare and Medicaid process
• Customize services tailored to your individual needs.
SPECIALIZED SERVICES
Orthopedic Rehabilitation
The program seeks to assist each patient in reaching maximum potential
through rapid mobilization and restoration of function. Our goal-directed ap-
proach to orthopedic and musculoskeletal rehabilitation tailors treatment to
help:
• restore physical function and enhance the skills needed to perform daily activities
• build strength, endurance and improve balance and regain mobility
• optimize independence
Brain Injury Services
Our residents suffering from brain trauma benefit from highly specialized
programs that are tailored to the unique physical and/or functional limita-
tions, cognitive impairments, and emotional or behavioral difficulties of the
individual.
Our goal-directed approach helps brain injury residents to:
• maximize responsiveness
• develop new cognitive and behavioral strategies to compensate for any deficits
• improve physical function and mobility to enhance the skills needed to perform
daily activities
Dementia and Alzheimer's Care
Our Dementia and Alzheimer's program provides care for those facing the
complex cognitive, emotional and physical effects of Alzheimer ’s disease
and other dementia related illnesses. Our staff recognizes the complex
nature of these illnesses and works to meet the individual’s medical, clini-
cal and nutritional needs.
