GUIDELINES FOR ACQUISITION OF CONTROLLED

PRESCRIPTION PADS & CONTROLLED RECORD BOOK

Required Material (1st requisition)

• Letter requesting for Controlled Prescription Pads and Record Book Register A:

ü Use Clinic’s Letter Head addressed to the Drug Control Department


in the Ministry of Health
ü The letter must be dated with the authorized personnel’s
signature and stamped

A. Prescription Pad for Controlled Drugs

• The prescription pad contains three copies:

ü Green copy: Clinic keeps as records

ü Pink copy: Sent to MOH Drug Control Department Private Sector Section-
All doctors are requested to return back the pink copy of the
prescriptions to the Ministry of Health at the end of each
month promptly

ü White copy: Patient to take to the pharmacy

N.B. All doctors need to abide with the federal decree, which states that the limited period
of dispensing controlled drugs is for general practitioner and for psychiatrist.

B. Record Books (Controlled Drug Register A Group 4)

• Each sheet carries the information of one drug with its name/strength/ unit stated on
the far right corner:
ü All information should be provided in case of inspection
ü The information should not be erased or crossed out (in case of a mistake,
use another line and sign it)
• The book should only be used by the assigned personnel who is responsible for
maintaining this book
• The register should be kept for record purposes for five years

* Government services will submit the requests to MOH with Service Charge.
* E-dirhams are the only accepted method of payment to the Ministry of Health.
N.B. If clinic chooses path B, DHCC will not be responsible to follow up with paper work.
