Complete the move-in section of this form and return it to within two weeks of moving in. All items are
presumed to be in good condition unless noted otherwise. Test all locks, window latches, smoke detectors,
and equipment. This form is not a repair request. Submit all requests for repairs separately in accordance with
your lease. You will use this form upon move-out. Keep a copy for your records. Note any defects in the items
listed below.

The above list is not all-inclusive and there may be other charges for cleaning, repairs, or replacement
of items. In addition, the above amounts are merely estimates of the minimum charges. Actual charges
may be significantly more. After cleaning, it is recommended that the management conduct a
walk through inspection with tenant to insure that all is cleaned satisfactorily and to
determine what if any costs may be assessed. FAILURE BY TENANT TO HAVE THE MANAGEMENT
WALK THROUGH MAY RESULT IN SUCH ADDITIONAL CHARGES AS DETERMINED BY MANAGEMENT AT SUCH
DATE AS THE PREMISES ARE VACATED.

______ Unit must be completely free of trash and all dust including closets, baseboards and cabinets.

______ Check all appliances and HVAC and then leave at 60 degrees in winter and 80 degrees in summer.
______ Furnace filters must be replaced for your HVAC system.
______ All floors need to be mopped, and cleaned, including underneath cabinets.
______ Sinks to be cleaned (bleached if necessary), and garbage disposal tested and free from blockage.
