Background and Instructions:

As per the Reserve Bank of India guidelines on Know Your Customer (KYC) policy, banks have to update customer
documents for all their existing customers at a regular frequency. Regulation may also make this mandatory for you to
continue using your credit card.
Please provide the documents as stated below for your credit card as well as for additional card member(s), if any
One set of documents would suffice if you hold more than one Citibank credit card.
· Recent passport size photograph (To be affixed in the slot provided below)
· Self attested copy of PAN card or any valid photo identity document
(e.g. Driving License, Passport, Voter’s Card or Identity card issued by Govt. Org., PSU, Armed Forces, Statutory/ Regulatory Authority)
· Self attested copy of Valid Visa for India (for expatriates only)
The copy of the photo ID document should be signed by the respective card members along with the related card number
mentioned therein. Please print a copy of this document (selecting A4 size) and use the below response format
mentioning your unique reference number as in the correspondence sent to you. You may send your responses to
