MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This Internship Agreement sets forth the terms and conditions between BALIWAG POLYTECHNIC COLLEGE, herein
referred to as the “College”, acting through its Office of the Internship and Placement Services, herein referred to
as the “Department” and BALIWAG TRANSIT INC, herein referred to as the “Organization” and is for the
purpose of providing cooperative arrangement for an industry internship program for students as provided in the
curriculum.

The ORGANIZATION agrees to the following:


1. To provide the selected student for an internship a variety of work-related experience within the scope of
services offered by the organization.
2. To correspond with a CERTIFICATE OF ACCEPTANCE OF INTERN before the commencement of the internship
period (see attached format).
3. The ORGANIZATION reserves the right to modify planned tasks during the internship period as may be
required by changes in the ORGANIZATION’S circumstances; however, the ORGANIZATION will make every
reasonable effort to insure that the intern is employed in a responsible position with exposure to multiple
tasks and situations, as required by the curriculum.
4. To provide quality supervision of the intern and to conduct performance discussions /feedback held on a
systematic basis during the internship program.
5. CERTIFICATE OF COMPLETION. The ORGANIZATION representative must acknowledge the interns’ successful
completion of the internship requirements.
6. To hold the COLLEGE harmless for any actions taken by the student during their participation in the
internship program.
7. To inform the DEPARTMENT for any changes in the work location, schedules and other concerns that may
occur during the internship period.
The COLLEGE agrees to the following:
1. To endorse for placement in the internship program only those students who have earned a satisfactory
record and have met the minimum requirements established by the COLLEGE.
2. To extend the authorized representatives of the ORGANIZATION an open invitation to visit the COLLEGE and
consult with faculty and students.
3. To have a representative of the DEPARTMENT, COLLEGE, available to the ORGANIZATION for assistance and
consultation in administering the internship program.
4. To advise students of their responsibilities regarding participation in the internship program, including
expected professional conduct and the requirement to follow rules and standards set by the ORGANIZATION
and the COLLEGE.
The ORGANIZATION and the COLLEGE jointly agree to the following:
1. That there will be on-going, open communication between the COLLEGE and the ORGANIZATION to insure
understanding of the expectations and roles of both parties in providing the internship experience for
students.
2. That the ORGANIZATION Site Supervisor and the DEPARTMENT of the Internship Program will be responsible
for determining and evaluating the program of experiences for the student assigned to the Organization.
3. That this Agreement does not limit the ORGANIZATION to accepting only students from the COLLEGE into
their internship program.
4. That neither party shall be responsible for costs or expenditures incurred by the other in the conduct of this
program, other than those expenses defined in any separate agreements that may be made between the
parties cooperating in this program.
5. That the student assigned to the ORGANIZATION shall be and remains a student of the COLLEGE and may
be able to receive remuneration as an employee of the participating ORGANIZATION during his/her
Internship Program if prior approval has been secured from both the COLLEGE and ORGANIZATION involved.
6. That either the COLLEGE or the ORGANIZATION may dismiss a student during the internship period, if, in the
opinion of either party, the student is not making satisfactory progress or is not meeting the expectations
set forth by either party. Any student who does not satisfactorily complete the internship or any portion
thereof may repeat the internship at the same ORGANIZATION only with the written approval of both the
ORGANIZATION and the COLLEGE.
7. That this agreement will remain in force for a period of three years from the date of execution. Either party
may terminate the agreement at any time on 30 days written notice to the other party; any internship in
progress at the time of the written notice will be completed under the terms and conditions of this
agreement.
