Comparison of Once-Daily Versus Twice-Daily Gentamicin

Dosing Regimens in Infants 2500 g†

Abstract
OBJECTIVE: There is no uniformity in the current

recommendations of dosing regimen of gentamicin for neonates. We

conducted a prospective, randomized, controlled trial to compare a

once-daily dosing regimen to the twice-daily dosing regimen for

neonates 2500 g during the first 7 days after birth.


STUDY DESIGN: Infants 2500 g admitted to the Neonatal

Intensive Care Unit and prescribed gentamicin for suspected

bacterial infection were randomized to receive either 4 mg/kg every

24 hours, study group (n=20), or a standard regimen of 2.5 mg/kg

every 12 hours, control group (n=21). Serum gentamicin

concentrations (SGCs) were followed and gentamicin

pharmacokinetics calculated on all infants.

RESULTS: Peak SGC 30 minutes after the first dose was 8.2± 1.7

g/ml in the study group, compared to 6.4± 1.5 g/ml in the

control group (p=0.001). Ninety-five percent of study group infants,

compared to 81% of the control group, had peak SGCs in

therapeutic range after the first dose. Peak SGC at 48 hours (steady

state) was 8.9± 1.5 in the study group and 6.8± 1.1 in the control

group (p=0.0001). On further analysis, a significantly higher

percentage of infants in the study group, compared to the control

group, had peak SGCs in higher therapeutic ranges of 6 to 12

g/ml as well as 8 to 12 g/ml. None of the study infants, compared

to six control infants, had trough SGCs 2 g/ml at steady


state. Thus, none of the study group infants, versus six of the control

group infants, needed a dosing adjustment at 48 hours (p=0.02,

Fisher's exact test).

CONCLUSION: We found that 4 mg/kg gentamicin given every 24

hours achieved significantly higher peak SGCs and safe trough

concentrations in all infants, compared to the twice-daily regimen of

2.5 mg/kg. We suggest that SGCs may not need to be followed in

term infants prescribed a short course of this once-daily regimen for

suspected early-onset sepsis if renal functions are normal. Journal of

INTRODUCTION

Gentamicin, along with ampicillin, is widely used in full-term neonates

as empirical therapy for presumed early-onset sepsis because the

combination is synergistic against common etiologic agents prevalent in

this age group.1,2 Similar to other aminoglycosides, gentamicin has a

narrow therapeutic index,3 and monitoring of serum concentrations is

essential to ensure optimum therapeutic concentrations and to avoid the

likelihood of nephrotoxicity and ototoxicity. Studies have suggested the

importance of achieving high therapeutic serum gentamicin

concentration (SGC) at the outset of therapy for improving the outcome

of patients with Gram-negative infections.4,5 Grasso et al.,6 in their in


vitro model, showed that a high peak gentamicin concentration is the

main determinant for antibacterial activity. Studies in adult patients have

demonstrated high-dose, extended-interval dosing of aminoglycosides to

be more efficacious and potentially less toxic compared to multiple

daily doses.7,8,9 However, data in neonates are sparse and neonatal

reference books continue to suggest varying dose schedules for full-term

infants.1,10,11

The aim of our study was to establish a uniform once-daily dose

regimen for gentamicin for full-term neonates and compare its

pharmacokinetics with that of traditional twice-daily dose regimen. For

this purpose, we chose to evaluate the new dose schedule of gentamicin,

i.e., 4 mg/kg given at 24-hour intervals, as recommended recently by

Neofax,12 a neonatal formulary book widely used for reference purposes.

We compared this schedule with the traditional dose schedule of

gentamicin, i.e., 2.5 mg/kg given at 12-hour intervals.11,13

METHODS

This study was designed as a prospective, randomized, controlled trial

and was conducted in the Neonatal Intensive Care Unit (NICU) of the

Cook County Children's Hospital, Chicago, IL, from March 1999 to

November 1999. The protocol was approved by the Institutional Review

Board of the Cook County Hospital. Written consent from parents was
required before enrollment of an infant in the study. The decision to

initiate gentamicin therapy was made by the treating attending

neonatologist. Inclusion criteria were: (1) birth weight 2500 g; (2)

age ≤ 7 days; (3) APGAR scores of 5 at 5 minutes; and (4)

suspected systemic or focal bacterial infection. Exclusion criteria were:

(1) history of perinatal asphyxia, shock, or cardiopulmonary arrest; (2)

seizures; (3) anomalies of the kidney or ear; (4) major congenital

anomalies incompatible with life; and (5) presence of neuromuscular

disorder.

Infants were randomly assigned to either study group or control group

by picking up a preassigned sealed envelope containing consecutive

infant numbers. For each number, a corresponding assignment, "study"

or "control," was randomly ordered as per a computer-generated list.

The study group infants were given gentamicin at 4 mg/kg per dose

every 24 hours12 and the control group infants were given gentamicin at

2.5 mg/kg per dose every 12hours.13 Neofax has recommended that

37 weeks13 and 38 weeks 12 be used to define full-term infants.

However, we used birth weight of 2500 g as a convenient and


duplicable characteristic for the term and near-term infants because a

majority of our mothers did not have accurate dates. Gentamicin was

infused over a 30-minute period with a metered syringe pump using a

microbore tubing. The tubing was flushed with routine flush solution

after completion of infusion. All infants were concomitantly treated with

ampicillin.

Data Collection

The following infant demographic profile was collected: gestational age

as determined by maternal history and confirmed by physical and

neurological exam based on Dubowitz gestational age assessment,14

birth weight, length, weight for gestational age, mode of delivery,

APGAR scores at 1 and 5 minutes, and postnatal age at the time of

initiation of therapy. Presence of patent ductus and concurrent use of

any nephrotoxic medications in the infant were also recorded. The

maternal demographic profile included: history of prolonged rupture of

membranes; chorioamnionitis; use of nephrotoxic medications, such as

aminoglycosides, diuretics, and indomethacin; and events, if any, that

may affect the fetal renal functions.

A complete blood count, blood culture, and urine latex antigen for group

B Streptococcus were done as a part of sepsis work up and were

recorded in the data sheet. Serial CBCs, urine culture, and CSF results
were also recorded, if requested by the treating physician, based on

clinical indications. Renal functions were monitored by measuring 24-

hour urine output on days 1, 2, and 3 of therapy. Serum electrolytes,

blood urea nitrogen, and serum creatinine were measured on days 2 and

3 of the study. Urine sodium and urine creatinine were measured on

days 2 and 3 of therapy.

SGCs

Blood for peak SGC was drawn 30 minutes after completion of the

gentamicin infusion and, for trough concentration, 30 minutes prior to

the start of gentamicin infusion. SGCs were measured after the first dose

and before and after the doses at 24 and 48 hours in the study group and

after the first dose and before and after the doses at 12, 24, and 48 hours

in the control group. To compare gentamicin pharmacokinetics, an

additional SGC was measured 11 hours after completion of the first

dose of gentamicin in the study group and was labeled as midpoint. This

SGC corresponded to the trough SGC in the control group taken before

the dose at 12 hours. Trough and peak SGCs drawn with the dose at 48

hours were considered to reflect steady state.

SGCs were measured by fluorescence polarization immunoassay

analyzer (Abbott, Axsym, Abbott Park, IL). Serum levels of <0.3


g/ml were reported as 0.3 g/ml. The coefficient of variation was

9.5% at a concentration of 1.15 g/ml and 7.9% at a concentration of

7.3 g/ml. Serum creatinine, urine sodium, and urine creatinine were

measured by Kinetic Alkaline Picrate methods using Olympus AV600.

Therapeutic SGCs

Peak SGC between 5 and 12 g/ml and trough SGC of <2 g/ml

were considered therapeutic.12,15,16 Dose adjustments were made during

the first 48 hours of study if peak SGC was <5 g/ml andinfant had

abnormal white blood cell count or positive blood culture or definite

evidences of sepsis. Dose adjustments were also made during this time

if gentamicin peak level was >12 g/ml and/or a trough level was

2 g/ml and there were signs of renal insufficiency as indicated

by high serum creatinine (>1.0 mg/dl), or an increase in the serum

creatinine value by >0.5 mg/dl or unexplained polyuria as defined by

urine output of >4 ml/kg per hour or oliguria as defined by urine output

of <1 ml/kg per hour.


Pharmacokinetics of Gentamicin

Individual pharmacokinetics measurements were done using one

compartment open linear model. The elimination rate constant (k) for

gentamicin was calculated by using the following formula:

k=(Cpmax/Cpmin)ln/time interval between two serum concentrations, where

Cpmax is peak SGC, Cpmin is trough SGC, and ln is natural log. The

biological half-life of gentamicin was calculated as 0.693/k. The volume

of distribution (Vd, l/kg) for gentamicin was calculated as Vd=dose per

kg/(Cpmax-Cpmin).

Hearing screen was done in all infants using the Natus ALGO infant

hearing screener. The result is displayed as Pass/Refer for individual

ear. The sensitivity of the result is 98% and specificity 96%. Hearing

screens were performed on all infants prior to discharge from the NICU.

Statistical Analysis

The trial was proposed as a pilot study. We planned to enroll 20 infants

in each arm of the study. Allowing for attrition and exclusion from the

study, we initially planned to enroll 10% more than the requisite

number. Statistical analysis was performed after enrollment of all

infants. All data were entered on an SPSS program. Statistical analysis

was performed using chi-square test and Fisher's exact test if necessary

for discrete variables, and two-way analysis of variance for continuous


variables. p<0.05 was considered significant.

RESULTS

Forty-four infants met the criteria and were enrolled in the study. Three

infants were excluded after enrollment: parents of two infants withdrew

consent, and a third infant was excluded because the initial blood

sample was lost. Thus, there were 41 infants: 20 in the study group and

21 in the control group. All infants were enrolled within the first 24

hours after birth. Table 1 lists the demographic profile of the two

groups. The groups were comparable in regard to birth weight,

gestational age, APGAR scores, postnatal age, and diagnosis of, or

severity of, neonatal illness. Two infants in each group required

inotropes for hypotension.

SGCs

As expected, the peak SGCs at all times were significantly higher in the

study group compared to the control group (Table 2). All but one infant

(4.78 g/ml) in the study group achieved therapeutic concentrations

(5−12 g/ml) immediately after the first dose compared to 17 infants

(81%) in the control group (Figure 1A). Subsequently, none of the

infants in the study group had subtherapeutic SGCs, whereas in the


control group, two infants at 24 hours and one infant at 48 hours still

had subtherapeutic SGCs (Figure 1C).

On further analysis, 93% of all peak SGCs in the study group was in a

higher therapeutic range of 6 to 12 g/ml11 compared to 57% in the

control group (p=0.0001) (Table 2). The difference was not significant

between the two groups after the first dose, but a significantly higher

percentage of infants in the study group compared to the control group

had peak SGCs between 6 and 12 g/ml after the doses at 24 and 48

hours. In addition, a significantly higher percentage of infants in the

study group compared to the control group had peak SGCs between 8

and 12 g/ml range at each time point studied (Figure 2).

None of the infants in either group had toxic peak SGCs of >12

g/ml at any time point (Figure 1, A and C). None of the infants in the

study group had a potentially toxic trough concentration of 2

g/ml before the 24-hour dose or before the dose at 48 hours. In contrast,

nine control infants at 24 hours (43%) (p=0.001) and six infants at 48

hours (29%) (p=0.02) had potentially toxic trough SGCs (Figure 1B).

Thus, six control infants (29%) compared to none of the study infants
required a change in dosing schedule after the dose at 48 hours (p=0.02).

One of these control infants also had subtherapeutic peak SGC at

48hours.

Two infants in the study group prior to the dose at 24 hours and one

infant prior to the dose at 48 hours had trough SGC of <0.5 g/ml

(Figure 1B), whereas none of the infants in the control group had such

extremely low trough SGCs at any time.

Infants <37 weeks of gestation had similar trough and peak SGCs as

their term counterparts, and excluding these infants did not change

results. However, our results should not be extrapolated for preterm

infants because we had only a small number of infants <37 weeks of

gestation (Table 1).

Gentamicin Pharmacokinetics

No differences were found between the two groups for gentamicin half-

life (5.82± 1.7 and 5.46± 1.4 hours for the study and the control groups,

respectively) or elimination rate constant (0.13± 0.03 and 0.14± 0.05

hours; study versus control).

Blood cultures in two control group infants grew group B

Streptococcus. None of the study group infants had positive blood


cultures. There were no differences in urine output, serum creatinine, or

creatinine clearance between the two groups (Table 3). All infants

passed a hearing screen test done prior to the discharge from hospital.

DISCUSSION

Our study demonstrates that once-daily dose of 4 mg/kg gentamicin

achieved significantly higher peak SGCs and safe trough SGCs in all

infants compared to the twice-daily regimen of 2.5 mg/kg. None of the

infants receiving once-daily dose required an adjustment in dosing

compared to nearly 30% of those receiving twice-daily dosing (p=0.02).

Since the original article by McCracken and Jones,17 several gentamicin

dosing regimens have been recommended in an attempt to achieve

optimal therapeutic serum concentrations. It has been shown that

clinical response to aminoglycosides, such as gentamicin, depends on

the peak serum concentrations and the peak-to-MIC ratio.4,18 The clinical

outcome was better and the mortality rate lower in adult patients with

pneumonia or sepsis caused by Gram-negative bacteria when peak

serum aminoglycoside concentrations exceeded 5 g/ml during the

first day of treatment.4,18 Themortality rate was only 2.4% when

gentamicin or tobramycin peak concentrations drawn 1 hour after a 30-

minute infusion exceeded 5 g/ml compared to 21% among patients


with peak SGC of <5 g/ml after the first dose.4 This observation

was supported by Noone et al.5 who reported an 88% cure rate if peak

SGCs were 5 g/ml in patients with bacteremia and 8

g/ml in patients with pneumonia compared to only 12% cure rate if peak

SGCs were below these ranges during first 72 hours of therapy. A

higher-dosage, extended-interval regimen of gentamicin in neonates

2500 g allows higher peak serum concentrations that are now

considered therapeutic and yet avoids cumulation of drug.

The need to achieve serum therapeutic concentrations immediately after

the first dose is not addressed by the currently available dosing

recommendations for neonates.10,11 To achieve high peak SGC in

neonates, several investigators have tried a loading dose of 5 mg/kg in

neonates.16,19 However, what subsequent doses should be used is not

clear. In our study, 95% of infants receiving gentamicin at 4 mg/kg at

24-hour intervals achieved therapeutic SGCs after the first dose

compared to 81% of the infants in the control group. None of the infants

in the study group had SGCs of <5 g/ml at 24 and 48hours,

whereas in the control group, two infants at 24 hours (9.5%) and one
infant at 48 hours (4%) still had SGCs of <5 g/ml.

The bactericidal activity of aminoglycosides, such as gentamicin, not

only depends on peak SGC but also on the ratio of peak SGC to MIC for

a given microbe. Moore et al.,18 in a post hoc analysis of four

randomized, double-blind, controlled trials of aminoglycosides for the

treatment of Gram-negative sepsis, demonstrated a graded

dose−response effect between an increasing peak concentration-to-MIC

ratio and the clinical response. The response rate in their study was

approximately 90% when peak-to-MIC ratio was 8 to 12. None of our

infants suffered from Gram-negative sepsis, but we speculate that the

gentamicin dose of 4 mg/kg given at 24-hour intervals to infants

2500 g would be potentially more efficacious in infants with such

infections sensitive to gentamicin even at a MIC of 1 g/ml because

a significantly higher percentage of infants in the study group, compared

to the control group, had peak SGCs between 8 and 12 g/ml

(Figure 2).

Several studies in the last decade have examined SGCs in neonates;

however, some have used extremely small sample size20 and others are

not prospectively designed in a randomized manner.21,22,23,24 A recent


study by Thureen et al.21 used once-daily versus twice-daily dose in

neonates 34 weeks of gestation. The dosing regimen was similar to

our study. Infants were prospectively temporally allocated to receive

one of the two regimens in this cohort study. However, 50% of the

infants in their study receiving twice-daily dose were <37 weeks of

gestation, whereas only 26% of those receiving once-daily dose were

preterm, suggesting a bias toward administering twice-daily dose to

preterm infants. Similar to our findings, Thureen et al. showed that

traditional regimen of 2.5 mg/kg given at 12-hour intervals achieved

significantly lower peak SGCs compared to the group receiving 4 mg/kg

at 24-hour intervals. Lundergan et al., using a bolus gentamicin dose of

5 mg/kg followed by 4 mg/kg in term infants 2500 g, also showed

significantly higher peak SGCs compared to the historical controls who

were given 2.5 mg/kg at 12-hour intervals.23

A significantly higher percentage of infants in the control group

compared to the study group had potentially toxic trough SGCs. This

finding is particularly important in clinical management. Mean trough

concentrations were also significantly lower in the study group

compared to the control group. Recently, Hayani etal.20 studied

gentamicin pharmacokinetics in term infants. They randomized 11

infants to receive 5 mg/kg every 24 hours and 16 infants to receive 2.5


mg/kg every 12 hours. Infants were given gentamicin either

intramuscularly or intravenously. The authors found similar trough

concentrations between the two groups. However, the difference in

trough concentrations might have been missed in their study because of

the small sample size and further dichotomizing the groups for the route

of administration. Our study confirms the findings of Thureen et al.21

and Lundergan et al.,23 who also found significantly lower trough

concentrations and a lower incidence of potentially toxic trough

concentrations in infants receiving once-daily dose of gentamicin

compared to those receiving twice-daily dose.

Using the currently available guidelines for therapeutic range, we found

that approximately a third of the infants in the control group, compared

to none in the study group, required a modification in the gentamicin

dosing (p=0.02). This finding is similar to findings of Lundergan et al.23

who reported a comparative cohort study recently. Approximately 45%

of term infants who received twice-daily dose of 2.5 mg/kg gentamicin

required a change in dosing regimen in their study compared to none of

the infants who received 5 mg/kg as initial bolus followed by 4 mg/kg at

24-hour intervals. These findings might obviate the need to follow

SGCs in term infants who are given a 2- or 3-day course of gentamicin

to rule out sepsis. We recommend that SGCs should still be followed in

term infants who receive gentamicin for a longer duration or those who
have a predisposition to abnormal renal functions, e.g., infants with

perinatal asphyxia or low APGAR scores.

The significance of the low trough SGCs (<0.5 g/ml) found in a

small percentage of the study infants is not clear at this time. Young and

Mangum12 have recently recommended a trough level of 0.5 to 1.0

g/ml as therapeutic, but there is no support in the literature for this

recommendation. Aminoglycosides, such as gentamicin, induce

postantibiotic effect and continue to inhibit growth of Gram-negative

organisms for up to 7 hours after the exposure to minimal inhibitory

concentrations of aminoglycosides.25 The length of postantibiotic effect

appears to be dose-dependent. The long postantibiotic effect of

aminoglycosides after exposure to high serum drug concentrations

suggests that regimens recommending higher doses of gentamicin given

at longer intervals will potentially be more efficacious in neonates with

Gram-negative infections.

We conclude that 4 mg/kg gentamicin given at 24-hour intervals to

neonates 2500 g at birth in the immediate neonatal period achieves

early therapeutic peak serum concentration and safe trough

concentrations in all infants compared with the conventional twice-daily

regimen of 2.5 mg/kg given at 12-hour intervals. Whether this once-


daily regimen of 4 mg/kg in term infants will improve clinical outcome

in proven Gram-negative infections requires further studies with an

extremely large sample size as only a very small percentage of infants

have such infections. However, extrapolating data from adults and

animal studies, this regimen may be most appropriate. Additionally, it

will reduce hospital expenses by cutting the cost of material, labor, and

processing.

Abstrak

TUJUAN: Tidak ada keseragaman dalam rekomendasi saat rejimen dosis gentamisin untuk

PENDAHULUAN

Gentamisin, bersama dengan ampisilin, banyak digunakan pada neonatus penuh panjang sebagai
terapi empiris untuk sepsis awal-awal diduga karena kombinasi yang sinergis terhadap agen

METODE

Penelitian ini dirancang sebagai percobaan, prospektif acak, terkontrol dan dilakukan di Neonatal
Intensive Care Unit (NICU) Rumah Sakit, Chicago, IL Cook County Anak-anak, sejak Maret
1999 sampai November 1999. protokol ini disetujui oleh Institutional Review Board dari Cook

Bayi secara acak ditunjuk untuk kelompok studi atau kelompok kontrol dengan mengambil
amplop tertutup berisi angka bayi preassigned berturut-turut. Untuk setiap nomor, tugas yang
sesuai "belajar", atau "kontrol," secara acak diperintahkan per daftar komputer. Kelompok Studi
bayi diberikan gentamisin pada 4 mg / kg per dosis setiap 24 hours12 dan bayi kelompok kontrol
diberi gentamisin sebesar 2,5 mg / kg per dosis setiap Neofax 12hours.13 telah

dan duplicable untuk bayi panjang dan jangka pendek karena sebagian besar ibu kami tidak
memiliki tanggal yang akurat. Gentamisin adalah diinfuskan selama jangka waktu 30 menit

Pengumpulan Data

Bayi berikut profil demografis dikumpulkan: usia kehamilan yang ditentukan oleh sejarah ibu
dan dikonfirmasi dengan pemeriksaan fisik dan neurologis berdasarkan penilaian usia kehamilan
Dubowitz, 14 berat lahir, panjang, berat untuk usia kehamilan, cara persalinan, skor Apgar pada
1 dan 5 menit, dan usia postnatal pada saat memulai terapi. Kehadiran ductus paten dan

Hitung darah lengkap, kultur darah, dan urin antigen lateks untuk kelompok B Streptococcus
dilakukan sebagai bagian dari pekerjaan sepsis dan dicatat dalam lembar data. CBCs Serial,
kultur urin, dan hasil CSF juga dicatat, jika diminta oleh dokter yang merawat, berdasarkan
indikasi klinis. fungsi ginjal dipantau dengan mengukur keluaran urin 24 jam pada hari 1, 2, dan
3 terapi. Serum elektrolit, nitrogen urea darah, dan kreatinin serum diukur pada hari ke 2 dan 3

SGCs

Darah untuk puncak SGC diambil 30 menit setelah penyelesaian infus gentamisin dan, untuk

pertama dan sebelum dan setelah dosis pada jam-jam 24 dan 48 dalam kelompok studi dan

SGCs diukur dengan immunoassay analyzer fluoresensi polarisasi (Abbott, Axsym, Abbott Park,

9,5% pada konsentrasi 1,15 mug / ml dan 7,9% pada konsentrasi 7,3 mug / ml. Serum kreatinin,
natrium urin, dan kreatinin urin diukur dengan Kinetic Picrate metode Alkali menggunakan
AV600 Olympus.

Terapi SGCs

Puncak SGC antara 5 dan 12 mug / ml dan SGC palung <2 mug / ml dianggap therapeutic.12,
15,16 Dosis dilakukan penyesuaian selama 48 jam pertama studi jika puncak SGC adalah <5
mug / ml andinfant telah abnormal jumlah sel darah putih atau kultur darah positif atau bukti
yang pasti sepsis. Dosis penyesuaian juga dilakukan selama waktu ini jika gentamisin tingkat
puncak adalah> 12 mug / ml dan / atau tingkat palung adalah 2 mug / ml dan ada tanda-tanda

Farmakokinetik Gentamisin

Individu farmakokinetik pengukuran dilakukan dengan menggunakan satu model kompartemen


linear terbuka. Tetapan laju eliminasi (k) untuk gentamisin dihitung dengan menggunakan rumus
berikut: k = (Cpmax / Cpmin) ln / interval waktu antara dua konsentrasi serum, dimana Cpmax

dihitung sebagai dosis = Vd per kg / (Cpmax-Cpmin).

Mendengar layar dilakukan pada semua bayi menggunakan algoritma Natus pendengaran bayi
screener. Hasilnya adalah ditampilkan sebagai Pass / Lihat untuk telinga individu. Sensitivitas
hasilnya adalah 98% dan spesifisitas 96%. Mendengar layar dilakukan pada semua bayi sebelum
pulang dari NICU.

Analisis Statistik

Sidang ini diajukan sebagai studi percontohan. Kami merencanakan untuk mendaftarkan 20 bayi
di setiap kelompok penelitian. Memungkinkan untuk putus sekolah dan dikeluarkan dari
penelitian, kami awalnya direncanakan untuk mendaftar 10% lebih dari jumlah yang diperlukan.
Analisis statistik dilakukan setelah pendaftaran dari semua bayi. Semua data yang dimasukkan
pada program SPSS. Analisis statistik dilakukan dengan menggunakan uji chi-kuadrat dan Fisher
exact test jika perlu untuk variabel diskrit, dan analisis dua arah varians untuk variabel kontinu. p
<0,05 dianggap signifikan.

HASIL

Empat puluh empat bayi memenuhi kriteria dan terdaftar dalam penelitian. Tiga bayi dikeluarkan
setelah pendaftaran: orangtua dari dua bayi menarik persetujuan, dan bayi ketiga dikeluarkan
karena sampel darah awal hilang. Dengan demikian, ada 41 bayi: 20 pada kelompok studi dan 21
pada kelompok kontrol. Semua bayi yang terdaftar dalam 24 jam pertama setelah lahir. Tabel 1
daftar profil demografis dari dua kelompok. Kelompok-kelompok sebanding dalam hal berat
badan lahir, usia kehamilan, skor Apgar, usia postnatal, dan diagnosis, atau keparahan, penyakit
neonatal. Dua bayi dalam setiap kelompok yang diperlukan inotropes untuk hipotensi.

SGCs

Seperti yang diharapkan, SGCs puncak sepanjang waktu secara signifikan lebih tinggi pada
kelompok studi dibandingkan dengan kelompok kontrol (Tabel 2). Semua kecuali satu bayi (4,78
mug / ml) pada kelompok studi mencapai konsentrasi terapi (5-12 mug / ml) segera setelah dosis
pertama dibandingkan dengan 17 bayi (81%) pada kelompok kontrol (Gambar 1A). Selanjutnya,
tidak ada bayi dalam kelompok studi telah SGCs subterapeutik, sedangkan pada kelompok

Pada analisis lebih lanjut, 93% dari semua SGCs puncak pada kelompok studi dalam berbagai

kontrol (p = 0,0001) (Tabel 2). Perbedaan itu tidak signifikan antara kedua kelompok setelah

dibandingkan dengan kelompok kontrol mempunyai SGCs puncak antara 6 dan 12 mug / ml

Tak satu pun dari bayi dalam kelompok bisa memiliki SGCs puncak beracun dari> 12 mug / ml
pada suatu titik waktu (Gambar 1, A dan C). Tak satu pun dari bayi dalam kelompok penelitian
memiliki konsentrasi melalui berpotensi toksik dari 2 cangkir / ml sebelum dosis 24-jam atau
sebelum dosis pada 48 jam. Sebaliknya, bayi kontrol sembilan pada 24 jam (43%) (p = 0,001)
dan enam bayi pada 48 jam (29%) (p = 0,02) telah SGCs melalui berpotensi beracun (Gambar
1B). Dengan demikian, bayi kontrol enam (29%) dibandingkan dengan tidak ada bayi diperlukan
studi perubahan dalam jadwal dosis setelah dosis pada 48 jam (p = 0,02). Salah satu bayi kontrol
juga memiliki SGC puncak subterapeutik di 48hours.

Dua bayi dalam kelompok studi sebelum dosis 24 jam dan satu bayi sebelum dosis pada 48 jam
telah melalui SGC dari <0,5 mug / ml (Gambar 1B), sedangkan tidak ada bayi dalam kelompok
kontrol sudah seperti sangat rendah SGCs melalui setiap saat.

Bayi <37 minggu kehamilan telah melalui serupa dan SGCs puncak sebagai istilah rekan-rekan
mereka, dan tidak termasuk bayi-bayi ini tidak mengubah hasil. Namun, hasil kita tidak boleh
diekstrapolasikan untuk bayi prematur karena kami hanya memiliki sejumlah kecil bayi <37
minggu kehamilan (Tabel 1).

Gentamisin Farmakokinetik

Tidak ada perbedaan yang ditemukan antara dua kelompok untuk gentamisin-setengah hidup
(5,82 ± 1,7 dan 5,46 ± 1,4 jam untuk studi dan kelompok kontrol, masing-masing) atau tingkat
penghapusan konstan (0,13 ± 0,03 dan 0,14 ± 0,05 jam; studi versus kontrol) .

DISKUSI

Studi kami menunjukkan bahwa dosis sekali sehari 4 mg / kg gentamisin dicapai SGCs puncak
secara signifikan lebih tinggi dan aman di SGCs melalui semua bayi dibandingkan dengan
rejimen dua kali sehari 2,5 mg / kg. Tak satu pun dari bayi yang menerima dosis sekali sehari
diperlukan penyesuaian dalam takaran dibandingkan dengan hampir 30% dari mereka yang
menerima dosis dua kali sehari (p = 0,02). Karena artikel asli oleh McCracken dan Jones, 17
gentamisin beberapa regimen dosis telah direkomendasikan dalam upaya untuk mencapai

Persentase yang signifikan lebih tinggi pada bayi pada kelompok kontrol dibandingkan dengan
kelompok studi telah SGCs melalui berpotensi racun. Temuan ini sangat penting dalam
pengelolaan klinis. Konsentrasi melalui Mean juga secara signifikan lebih rendah pada kelompok
studi dibandingkan dengan kelompok kontrol. Baru-baru ini, Hayani etal.20 belajar
farmakokinetik gentamisin pada bayi panjang. Mereka secara acak 11 bayi menerima 5 mg / kg
setiap 24 jam dan 16 bayi untuk menerima 2,5 mg / kg setiap 12 jam. Bayi diberikan gentamisin
baik secara intramuskular atau intravena. Para penulis menemukan konsentrasi melalui serupa di
antara kedua kelompok. Namun demikian, perbedaan dalam konsentrasi melalui mungkin telah
terjawab dalam studi mereka karena ukuran sampel yang kecil dan selanjutnya dichotomizing
kelompok untuk rute administrasi. Penelitian kami menegaskan temuan Thureen et al.21 dan
Lundergan et al., 23 yang juga ditemukan melalui konsentrasi jauh lebih rendah dan lebih rendah
insiden melalui konsentrasi yang berbahaya pada bayi yang menerima dosis sekali sehari
gentamisin dibandingkan dengan mereka yang menerima dua kali sehari dosis.

Menggunakan pedoman saat ini tersedia untuk berbagai terapeutik, kami menemukan bahwa
sekitar sepertiga dari bayi pada kelompok kontrol, dibandingkan dengan tidak ada dalam
kelompok studi, membutuhkan modifikasi di gentamisin dengan dosis (p = 0,02). Temuan ini
mirip dengan temuan Lundergan et al.23 yang melaporkan penelitian kohort komparatif baru-
baru ini. Sekitar 45% bayi panjang yang menerima dosis dua kali sehari 2,5 mg / kg gentamisin
membutuhkan perubahan rejimen dosis dalam studi mereka dibandingkan dengan tidak ada bayi
yang menerima 5 mg / kg bolus awal diikuti dengan 4 mg / kg pada 24 -jam interval. Temuan ini
mungkin meniadakan kebutuhan untuk mengikuti jangka SGCs pada bayi yang diberi 2 - atau
kursus 3-hari gentamisin untuk menyingkirkan sepsis. Kami menyarankan SGCs masih harus
diikuti dalam jangka bayi yang menerima gentamisin untuk durasi yang lebih lama atau mereka
yang memiliki kecenderungan untuk fungsi ginjal normal, misalnya, bayi dengan asfiksia
perinatal atau skor Apgar rendah.

Arti penting dari SGCs melalui rendah (<0,5 mug / ml) ditemukan dalam persentase kecil dari
bayi belajar yang tidak jelas saat ini. Young dan Mangum12 baru-baru ini merekomendasikan
tingkat palung 0,5-1,0 mug / ml sebagai terapi, tetapi tidak ada dukungan dalam literatur untuk
rekomendasi ini. Aminoglikosida seperti gentamisin, menimbulkan efek postantibiotic dan terus
menghambat pertumbuhan organisme Gram-negatif hingga 7 jam setelah terpapar untuk
konsentrasi penghambatan minimal aminoglycosides.25 Panjang efek postantibiotic tampaknya
tergantung dosis. Pengaruh postantibiotic panjang aminoglikosida setelah terpapar obat
konsentrasi serum yang tinggi menunjukkan bahwa rejimen merekomendasikan dosis tinggi
Gentamisin diberikan pada interval yang lebih panjang akan berpotensi menjadi lebih mujarab
pada neonatus dengan infeksi Gram-negatif.

Kami menyimpulkan bahwa 4 mg / kg gentamisin diberikan dengan interval 24-jam untuk


neonatus 2500 g pada kelahiran pada periode neonatal dini segera mencapai puncak konsentrasi
serum terapeutik dan konsentrasi melalui aman di semua bayi dibandingkan dengan rejimen dua
kali sehari konvensional 2,5 mg / kg diberikan dengan interval 12-jam. Apakah ini rejimen sekali
sehari 4 mg / kg pada bayi panjang akan memperbaiki hasil klinis pada infeksi Gram-negatif
yang telah terbukti memerlukan penelitian lebih lanjut dengan ukuran sampel yang sangat besar
karena hanya persentase yang sangat kecil dari bayi mengalami infeksi tersebut. Namun,
ekstrapolasi data dari orang dewasa dan studi hewan, rejimen ini mungkin paling sesuai. Selain

pengolahan.
