LEFLORE COUNTY HEALTH CENTER OPENS


-New Center Offers $10 appointments for Leflore County’s Uninsured-

Greenwood, MS – May 16, 2011 – The Leflore County Health Center will celebrate its grand
opening with a ribbon cutting ceremony and brief open house at 9 a.m. on Thursday, June 2, 2011.
This new community-based health center is located at 706 Hwy 82W, Suite A, Greenwood, MS.

The Leflore County Health Center (LCHC) opens to provide high quality medical services to the
uninsured citizens of Leflore County. The new clinic is staffed with certified medical
professionals committed to providing personalized care in a confidential environment. For a $10
administrative fee, patients will receive comprehensive primary care services.

The opening of this health center was made possible with the support of the Greenwood Leflore
Hospital, Viking Corporation, Trustmark Bank, Jones Signs, and many other generous donors.
Support has also been provided through a partnership with Delta Health Alliance (DHA) initiated
in November of 2010. DHA is an organization that is committed to increasing access and
improving the quality of health care throughout the Delta. Through the agreement, the LCHC
agreed to begin transitioning the clinic from a traditional approach to primary care to a more
patient-centered approach. Clinical staff, including a nurse practitioner, nurse, patient navigator
(social worker) and community health worker, provided through the agreement with DHA, will
address the needs of the “whole person”. A registered dietician and clinical pharmacist will
support the LCHC, along with three additional DHA-affiliated clinics in Humphreys, Sunflower
and Washington Counties. Clinic teams are particularly focusing on improving outcomes for
chronic illness, specifically diabetes and hypertension.

The Center is open Mondays and Thursdays from 10 a.m. to 7 p.m., Tuesdays and Wednesdays
from 9 a.m. to 5 p.m. and Fridays from 9 a.m. to 3 p.m. Patients are encouraged, but not required,
to make appointments in advance in an effort to minimize wait-time. Appointments can be made

The Leflore County Health Center is a Mississippi-based non-profit health center dedicated to
improving the health outcomes of the people of Leflore County. LCHC strives to be an affordable
healthcare option that increases the ability of an individual to access high quality care, while
simultaneously educating the citizens of Leflore County on the prevention of disease and illness.
LCHC belongs to the people of Leflore County and is run by a board representative of the
community and healthcare stakeholders.
Delta Health Alliance is a tax-exempt, non-profit organization, head-quartered in Stoneville,
Mississippi. Delta Health Alliance is changing health care in the Mississippi Delta by improving
access to health care and providing education for healthier lifestyles. DHA accomplishes this by
linking the three major universities that serve the Delta (Delta State University, Mississippi State
University, Mississippi Valley State University) with the University of Mississippi Medical
Center, and Delta Council., the region’s economic development organization. The primary
purpose of the DHA is to coordinate and provide oversight for community-based programs that
address critical healthcare and wellness gaps in the Delta. DHA strives to accomplish this by
