Service Agreement Options Charges: To obtain website maintenance services from InforME, you must have a maintenance agreement in place with the minimum fees pre-paid. The minimum fee is $1800 for 12 months or $900 for 6 months, which covers management of the agreement and includes the first 2 hours of maintenance work per month. This fee is non-refundable and the work-hours may not be carried over to other months. Additional maintenance work in a given month beyond the pre-paid agreement hours will be billed on a monthly basis, at the rate of $75 per hour. URL: Terms of Service By signing this agreement, you understand, agree to, and authorize the following: 1. Services available under this agreement: Updates and changes to content on existing pages Creation of new web pages within existing site Formatting and posting of content images Conversion of documents to HTML Creation of basic online forms using AutoForms tool (does not include complex forms or payment transactions) Whats New tool setup for news, calendar of events, etc. Repair of accessibility problems 2. Services NOT available under this agreement: Site design, graphic design, navigation design, header/icon/logo design, and other design services beyond routine maintenance. Those services may be requested separately (at additional charge) and must be placed into InforMEs design department project queue. Programming, database development, application development. Programming services may be requested separately (at additional charge) and must be placed into InforMEs programming project queue. Copywriting, typing, research, and document scanning/OCR services. InforME does not offer these services. 3. InforME will maintain a record of the number of hours used and will invoice the Agency on a monthly basis at the rate of $75 per hour for any work hours beyond the pre-paid agreement. 4. Minimum Charges: A hour minimum charge applies to all requests with the following exceptions: Rush Requests (made less than 24 hours in advance) will incur a 1 hour minimum charge. Forms and Whats New tool setup will incur a 1 hour minimum charge. 5. Scheduling Requests: Requests for routine services under this agreement should be made 48 hours in advance whenever possible. Requests made less than 24 hours in advance will be considered Rush Requests and will incur a 1 hour minimum charge; in addition, InforME cannot guarantee that staff will be available to provide services at the time requested. Requests requiring more than 2 hours of time should be schedule at least one week in advance. 6. Approval of Changes: When possible, changes to the agencys site will be posted under a test filename or directory and a URL will be sent to the agency to review the changes. If necessary, changes will be made directly to the live site, and the agency will be asked to review the changes immediately. It is the responsibility of the agency to review changes in a timely manner and ensure that the changes are correct. 7. Providing Site Content: All web page content must be provided by the agency in a usable electronic format (Word, Rich Text Format, Excel, CSV), with a clear indication of where it should be placed on the site/page. Content should be in final format when sent. Content may be sent as attachments to email (preferred), or on CD.

8. Submission of Requests: We will only accept requests from the contact person designated on the maintenance agreement unless otherwise instructed. Please let us know if you would like to permit submission of requests by additional contacts at your agency. With each request, please provide the desired completion date. 9. Billable Time/Tasks: Billable time is any InforME time spent on tasks required to complete a maintenance request, including but not limited to: discussion or clarification with the agency by email or telephone, execution of requested changes, testing, revisions, and corrections. If a meeting is necessary, the meeting time will be billed at $50 per hour for each InforME staff person attending the meeting. 10. Efficiency Tips: Since minimum charges apply, we recommend that you batch your work whenever possible to get the most work accomplished in the fewest charged hours. A recommended schedule for regular maintenance updates is once or twice a month. Rush requests are performed as needed, with minimum charges applied. 11.
