2 19. 20. 21. 22. 23. Use of vehicles such as cars, trucks, dumpers, cranes, etc. to be preferably avoided in hazardous areas. In unavoidable case, use with proper type of spark arrestor. Loose electric connections should be avoided. Road cuttings should be preplanned and special permission obtained from clients. While excavation, special care is to be taken to avoid damages to cables. While carrying out hot work in hazardous areas, valves, flanges, pipelines to be covered with wet asbestos cloth to prevent accidental fire / sparking and running water to be maintained nearby. Catch basins, manholes and other sewer connections are points of possible gas accumulation. Contractors and their men should exercise special care to see that no sparks or flame occur at these locations unless the openings are properly covered. The use of light distillates (gasoline, etc.) for cleaning purpose is restricted. Carbon tetrachloride is toxic and should not be used. No work should be carried out in the plant areas without valid cold/hot permit. This permit should be renewed in each shift and carried out on supervisors person to be produced on demand. Cold/hot work on running lines or equipment should be carried out in presence of the clients operating personnel only. Do not tamper with valves in a running plant. For construction works, separate water/air to be used and use of water/air from the running plant is prohibited. Removal of blinds/spades. Etc. to be done in presence of clients operating personnel only. Gas tests should be done as per operational personnel requirement. Look out for oil spillage, gas leaks, etc. particularly from safety valves. Special gas specks should be done in and around workspot particularly at flange joints, valve glands, drain valves and threaded joints before commencement of any hot/cold work. Works of flushing, cleaning, purging of operational lines, equipment shall be done by client only. 3

Do not operate tags to be provided on blinds, valves, etc. at isolation points. Following precautions should be taken before commencing the work inside vessels, columns. a) b) c) d) e) f) g) h) i) j)

The vessel has been properly prepared and the temperature should be as near ambient as possible.
Vessel should be isolated or blinds installed close to the vessel. The vessel has been satisfactory gas free and tested. Provide adequate ventilation by keeping all possible manholes open. Limit the number of men working inside. Wear safety hats while working inside vessel. Ensure that no material can fall down from people working above. Only approved reduced voltage extension lights (24 volts) will be allowed for use inside vessel. A standby should be located outside the vessel at the manhole. Provide one flashlight for men working inside each location. Prepare a specific permit for buttoning up of vessels after work is completed. This is to ensure no one is left inside vessel and no material is left behind. One responsible person must inspect vessel prior to buttoning up and stay at the manhole till the manhole is buttoned up at least on four bolts. All the electrical wires for construction lighting should be hanged on wooden pole with proper clamps. All the wires should be connected with two pin or three pin plugs with distribution board. Electrical wires connection should not be loose anywhere. Provide appropriate fuse wire and earthing, so in case of overloading and current leakage there will not be a risk to men or instruments. All the fuse boards and distribution boards should be locked properly and handled by only electrician. At construction site, during rainy season water will be filled up in excavated pits. At that time barricade should be kept to avoid falling injury accident.
