Safety and Quality Nursing

Example a. Healthy Individual: Nurse should be able to enlighten the mind of the individual about his or her concern about health and identifying health needs of patient. b. At risk Individual: Nurse should be accountable for the safety of the patient; avoiding errors from medication and preventing patient from accidental falls, explaining certain procedures to patients before carrying them out. c. Sick Mother: Attending to the urgent needs of the patient; nurse should be attentive on the signs and symptoms of a pregnant mother in order to give the proper intervention. d. Sick Child: Nurse assumes responsibility of a surrogate mother to the sick child in the hospital setting. Close supervision to the sick child is important. The nurse should also explain to the parents or relatives of the child the medications and procedure to be carried out. e. Family: Nurse also acts as caregivers to one member of the family and takes responsibility on providing care specially when the rest of the family is not around. Safety of the client id ensured specially on giving medications; nurse should also assist the patient on his or her activities of daily living and explain to the other family members the how to also support the patient in hi/her ADL. f. Community: Nurse should provide sound decision making care of individual/groups considering their beliefs, values. This is attained through identifying the problem of the community, gathering

Healthy Individual: Nurse should inform the healthy individual on maintain a clean environment for prevention of diseases and the healthy individual must also be knowledgeable on being resourceful specially when in the home setting, there are limited resources compared to the hospital setting. At risk Individual: Nurse should be able to prioritize tasks and attend to the needs of the patient. She should be aware of putting the patients condition at priority. Nurse should plan her performance of tasks based on priority. Sick Mother: Nurse should be able to prioritize patient according to the severity of the illness. In this case, if a sick pregnant\ mother seeks help, the nurse needs to attend on her because there are two lives at risk: the mothers and the baby. Sick Child: Nurse should equip the parents information on how to utilize and maximize resources available especially in the home setting. O the othe hand, in the hospital setting, sick children are more likely to undergo certain treatment and procedures also ensuring the proper functioning and performance of equipments to be used is a must to be able to refer malfunction of equipments to decrease the possibility of putting the childs life at risk. Family: Nurse should be

knowledgeable on maximizing available resources and utilize them efficiently to give appropriate intervention. Community: Nurse should act as one of the leaders to implement and maintain safe environment in the community. This can be achieved through observing proper waste disposal, coordinate with the barangay officials on procedures like infection control, and provide information about what to do in case of fire, earthquake, and other emergencies. Healthy Individual: Nurse should provide information to a healthy individual about promoting health and preventing illness. Nurse should encourage the individual to practice a healthy behavior. At risk Individual: Nurse should assume the responsibility of formulating a plan of interventions which will enhance the condition of the patient. Sick Mother: Nurse should develop a plan based on anticipated needs of the sick mother. Involvement of the family is also important because the mother has a vital role in the family. Sick Child: Nurse should establish a health education plan with the family members of the sick child in order for the other members of the family to be able to help for the betterment of the childs condition. Family: Nurse assesses learning needs of the family regarding promoting health and prevention of illness through obtaining information by interview, observation and validation of data. Healthy Individual: Nurse should always bear in mind that with dealing with patients, she should always act as professional. She should protect the positive image of the Nursing profession. At risk Individual: In dealing with at risk patient, nurse should adhere in accordance with the policies and procedures both local and international: completes information given by the doctor for an informed consent if the patient will undergo procedure such as surgery, or on the other hand, if the patient refuses to undergo certain procedure or treatment, nurse should secure a waiver of responsibility for refusal. Sick mother: If mother was given a medication for pain as indicated by the doctor, the nurses legal responsibility is to document all matters concerning patient care. Sick Child: If the nurse had given a medication as prescribed by the physician to a child client, she needs to document it as part of her nursing care. Documentation should be always accurate. Family: If a family member is to undergo a certain surgical procedure, the nurse should present an informed consent to the family members in order to proceed with the surgery. Community: If a nurse is part of an organization or works in an institution in the community, she should be knowledgeable of the

Healthy Individual: Nurse should respect the individual and not to do unethical behavior such as malpractices (e.g. medication error) At risk Individual: Nurse should observe her actions specially on dealing with at- risk patients avoiding malpractice such as further injury from an accidental fall because the nurse was not able to raise the side rails of patients bed. Sick Mother: Nurse should know boundaries between her and her client. Confidentiality and privacy is important. Patients information should not be publicized. Sick Child: Nurse should be tactful with dealing with patients. Nurse should protect the patients confidentiality of information because it is included in the patients bill of rights. Family: Nurse should accept responsibility and accountability for own actions and decisions. In dealing with the family, the nurse should coordinate with all family members not only following her own decisions because family act as one. Community: Nurse should respect the rights of a certain group of people. If a nurse is dealing with the Muslim community, she should respect the difference in belief between her own belief and of the Muslims. Healthy Individual: Nurse should always present herself as a professional with good manners and positive behavior towards patients. If an individual asks about the factors which may

At risk Individual: If the individual demands too much attention, nurse should always demonstrate good manners and right conduct. Sick mother: If a mother asks you what you are doing to her that requires you to prick her finger, nurse should explain to the mother that the procedure is called a Hemo Gluco Test which will determine the amount of her blood sugar in the body. Nurse should have congruence with her words and actions. Sick Child: If a nurse has a hard time making a sick child drink his medicine, nurse should try another approach which would let the child cooperate with the regimen (e.g. taking the medicine with juice) Family: Nurse should be able to make an effort to try new strategies on helping the family to enhance the sense of responsibility on each family member. Community: Nurse may support activities in her barangay specially which concern health and nursing. Through this, nurse will be able to broaden her knowledge and may be able to apply her learning in improving health care. Healthy Individual: Nurse should encourage patient to speak about what is relevant to his condition. This is important in order to make clear to the individual what bothers him about his health. able to detect variations on the vital signs of the patient from day to day in order to modify interventions and to be able to report deviations to the physician. Sick mother: Nurse may share with the team current information regarding particular patients condition. If mother resist to comply with her medical regimen, nurse may seek advice from other health team members for recommendations. Sick Child: Nurse should participate during nursing rounds in order to check daily patients records and conditions. Family: Nurse may request feedback from patient or other member of the family regarding care rendered for quality improvement and personal development as well. Community: If a problem was identified in the community setting, nurse communicates the problem with the rest of the group in order to gives appropriate suggestions on corrective and preventive measures. Healthy Individual, Family and Community: Nurse has the responsibility of disseminating information about a certain research finding and communicate it with individual, family and community. At risk individual, sick mother and sick child: Nurse may apply her research finding in improving the nursing care to patients. Healthy Individual: Nurse should be responsible for confidentiality of the individual seeking assistance of a health care provider. At-risk Individual: If nurse rendered patient care,

Sick Mother: If nurse has given a medication to the mother as prescribed by doctor, she needs to record it to keep track on the progress of patients care. d. Sick Child: If the child has hyperthermia and the nurse rendered a tepid sponge bath and recorded it for updating patients care. e. Family: Nurse observes legal imperatives in record keeping refraining from releasing records and other information without proper authority. Patients records are confidential that even family members have limitations when it comes to seeing the records. f. Community: Nurse maintains an organized system of filing and keeping patients records in a designated area. a. Healthy Individual: Nurse should establish rapport in order listen from the heart of individual while he expresses his concern about his health. b. At-risk Individual: Nurse should listen attentively to the patients request and queries. At-risk patients have high demands when it comes to health care. Nurse should be patient and reserved while attending to these needs. c. Sick Mother: Nurse should identify verbal and non-verbal cues to interpret patients body language and facial expressions which may indicate a need or a request. d. Sick Child: Nurse identifies a significant other so that follow up care can be obtained specially if the patient is a child. Identify also noverbal cues of the patient to attend

Healthy Individual: If patient had any concern with his health but needs to seek an advice from a physician, then the nurse may refer the patient with other members of the health team specifically to the doctors. b. At risk Individual, Sick Mother and Sick Child: For this kinds of patients, collaboration is essential in order for other health team members contribute to decision making regarding patients needs and concerns. c. Family and community: Nurse should collaborate plan of care with other members of the health team in carrying out interventions to a bigger population like the family and the community. The team may organize medical mission for families of the community to benefit to imporove health.
