Get the template from /oracle/PROD/apps/apps_st/appl/au/12.0.0/forms/US

Put the path of the resources you get Resources 12 folder in the form path in oracle home Start Run regedit

When you open the template form in form builder do the following a. Rename the form start the name with XX and save the new one in your PC b. From the Data Blocks Delete BLOCKNAME and DETAILBLOCK c. From the Canvases Delete BLOCKNAME d. From Windows Delete BLOCKNAME e. Create new block and make the form as you want f. For each component in the form choose Subclass Information from the Property Palette of the item

Put form.fmb in au folder Compile the form In Putty cd /oracle/PROD/apps/apps_st/appl . APPSTEST_testsrv.env cd /oracle/PROD/apps/apps_st/appl/au/12.0.0/forms/US frmcmp_batch module=XX_TEST.fmb userid=apps/apps output_file=/oracle/PROD/apps/apps_st/appl/fnd/12.0.0/forms/US/XX_T EST.fmx compile_all=special batch=yes/ Change the form name .fmb and .fmx Change the output _file to the path you want to create the fmx file

Register form into the application g. Login to the application h. Open Application Developer Responsibility if it is not in your user , login with sysadmin and add it to your user

Open Application Function

k. Press Save or Ctr+s l. Search for the menu to put your form on it (for example we want to

add this form in the menu of responsibility System Administrator ) i. Open ACROW_SYSADMIN Responsibility ii. Open Security Responsibility Define iii. Search for the responsibility you want to add you form in its menu

iv. Copy the Menu Name v. Open Application Developer Responsibility vi. Open Application Menu vii. Search for the menu you get in step 6.6.4.

viii. Add new line and choose your form
