Life Care Center of Medina, owned and operated by Life Care Centers of America, is committed to being a premier provider of long-term health care. In cooperation with Fresenius Medical Care and their 25 years of experience in providing treatment to individuals with kidney disease, Life Care Center of Medina is proud to be serving the community of those who need dialysis and therapy treatment.

Our Services
Dialysis patients in need of short term rehabilitation, post-hospitalization care, complex medical service or other health programs can receive the specialized services they need and the quality care they deserve. Whether an individual is new to hemodialysis or an established dialysis patient, skilled nursing, rehabilitation and dialysis services can be met at one location: Life Care Center of Medina. Case management and social services are available to assist individuals with discharge planning and set up hemodialysis services in the community post discharge.

The dialysis unit is a cooperative effort between Life Care Center of Medina and Fresenius Medical Care. Fresenius Medical Care North America is widely recognized for its leadership and innovation in kidney dialysis treatment and the development of innovative products for people with kidney failure. Operating a network of more than 1,000 dialysis clinics in the United States, they provide dialysis treatment to more than 75,000 patients with kidney failure.

Rehabilitation is often a critical component of a residents care plan. A skilled team of therapists evaluates the rehabilitation needs of each resident and designs an outcome-oriented treatment program. Physical therapy helps restore movement and function while minimizing pain. Speech and language pathology addresses the needs of residents who are suffering from speech, hearing and swallowing disorders. Occupational therapy helps residents achieve greater independence in the activities of daily living such as dressing, bathing and eating, as well as more complex endeavors such as household management, meal planning and community interaction. Respiratory therapy helps residents reach optimal physical functioning by promoting pulmonary care through education, medication, oxygen administration and pulmonary exercises.

We recognize that every resident has unique needs, goals and abilities. By working closely with the resident and his or her family, the staff develops an individualized care plan for each resident. The plan is periodically reevaluated to ensure that it meets a residents changing care needs. The building is constructed from fireproof materials to ensure adequate fire protection. Exterior doors are connected to a security alarm system, and a nurse call light system is located at each bedside and in all bathroom facilities.

Respite, or short-term care, is provided for individuals who need assistance while their primary caregiver is on vacation or temporarily unable to provide home care. Respite participants receive the same services as the facilitys full-time residents, including quality nursing care and supervision, as well as group and individual activities.

Residents are encouraged to participate in a wide range of activities planned by a certified recreation therapist. A monthly calendar keeps residents and guests informed of upcoming group events such as parties, movies, exercise classes, musical programs, religious services and craft classes. The activities department also schedules excursions to allow residents to stay in touch with the community. Residents ride safely and comfortably in a 15-passenger, wheelchairaccessible bus. Additionally, the recreation therapy department develops, with the assistance of the resident and their family, an individualized activity plan based on the preference, desire and ability of each resident.

Through this rehabilitation program, individuals are often able to learn alternate methods to complete their activities of daily living. Physical therapists teach dementia rehab patients the techniques needed to stay fit, gain strength and maintain flexibility and balance. Occupational therapists work with patients on their fine motor skills, and they will also help individuals with cognitive impairments learn to schedule their time, balance their checkbook and perform selfcare tasks. Speech/language pathologists focus on cognition. They design programs that will encourage cognitive stimulation. Routines are continually updated, so each patients mind is constantly stimulated.

Dementia rehab is a new approach to rehabilitation that focuses on the individual as a whole person. A team of therapists, specially trained to work with individuals with cognitive impairments, will evaluate the residents cognitive function and physical abilities. A unique care plan will then be created for each patient, based on specific individual needs. This comprehensive approach is what makes Garden Terrace Alzheimers Center of Excellences dementia rehab different from traditional rehabilitation programs. The ultimate goal of this program is to delay the progression of the disease, optimize independence and encourage brain health and wellness. A short screening process will determine if dementia rehab is right for your loved one.

BEFORE REHAB

AT THE END OF YOUR STAY


D We will coordinate your discharge. You will be discharged from this facility only when your facility doctor releases you. We will recommend any equipment and postdischarge rehab needs at that time. D Your doctor or a nurse may recommend an extension beyond your last day of therapy if your medical status requires further observation. D You will then transition in to our Level II program and receive further exercises and mobility with a specially trained Restorative Aide. During this time, you will be observed to determine your ability to retain the strength, endurance, safety and mobility that you gained from therapy. D Your therapist may reevaluate you if we feel you are not yet ready to be discharged to your home environment.

D We have a flexible schedule. We can work around your preference for split treatments, or getting all your therapy completed during a certain time of day. We want you to be at your best during treatment sessions to get the most from your rehab. D We have several rehab areas in the building. Our large gym is at the rear of the Campbell Rehab Center. We have a smaller occupational therapy and speech room in the hallway between the activity room and the larger gym. Our therapy track, where patients can practice their outdoor skills, is located in the open area in the middle of the Campbell unit. D We have many wonderful options we offer you during your stay here, including our Ready Set Go! program and home evaluations. We use these to assist us in determining your readiness to return home independently and to assure your safety in the home environment. Our Wii gaming system can be utilized for many therapy goals and helps you have fun while doing therapy. D We welcome you to return to our outpatient program to continue your progress after being discharged. Please feel free to visit our outpatient facilities and meet our therapists.

DURING YOUR STAY


D You will be seen 5 to 7 days a week, depending on your medical diagnosis and ability to tolerate therapy. The frequency of your therapy may increase or decrease, depending on your progress. We make every effort possible to offer the patients continuity of care with the same therapist, so please inform the rehab manager if there is a certain therapist you prefer. Outpatient Rehabilitation
e continue to develop our outpatient program with the goal of moving the increased volume of business off-site before the end of the year. A new team of outpatient therapists has come on board recently, due to the demand in the Leeward and Central Oahu service areas for rehabilitation. These services continue to be available as part of the continuum of care for our inpatients upon discharge. Our facility is in the process of establishing a separate offsite satellite rehab clinic that will be located on the second floor of the Kapolei Medical Park, adjacent to Ka Punawai Ola.

ur facility completed a 26 bed minirenovation unit for sub acute rehabilitation services in April 2009. We are currently creating an improved ambience with some minor renovations to accommodate the different needs of the short-term rehab patients.

We have therapists specializing in speech, occupational and physical therapy. The program includes wound care treatment, neurological assessment and training, as well as occupational hand therapy. The facility goal for 2009-2010 continues to work toward partnerships with orthopedic physicians and surgeons. This will allow our therapy and nursing associates the opportunity to be trained with them and by doing so, thus allowing the facility to meet each physicians specific care protocols.

Ka Punawai Ola admits patients with a higher acuity level than the current comparison for the State of Hawaii nursing facilities as reflected in the CMS gathered data.

Ka Punawai Ola is redesigning and renovating current facility office spaces into new rehabilitation areas, creating a functional living center. This area will include a training bedroom, assistive technology center and an area for training in activities of daily living. The facilitys homelike transitional living center allows our patients to practice skills to assure safe discharges to their home environment.

A highly trained team of therapists evaluates the rehabilitation needs of each resident and designs an outcome-oriented treatment plan. Enjoy an award-winning fine dining experience at Life Care Center of Sullivan. At each meal, residents may order one of the daily specials or other meal options that are available every day.

Activities
Residents are encouraged to participate in the wide range of activities planned by our certified activity coordinator. Parties, movies, exercises, musical programs, arts and crafts and religious services are offered, as well as outings. A monthly calendar is posted to keep residents and guests informed of what is scheduled. Volunteers provide residents with another link to the community.

Alzheimers Special Care Unit


Life Care Center of Sullivans special care unit was designed to meet the special needs of individuals with Alzheimers and dementia disorders. Associates are specially trained to work with individuals suffering from Alzheimers. Associates consistently care for the same residents; this helps create a comfortable atmosphere for residents. An activities director designs activities specifically for residents with Alzheimers and dementia disorders. A secure, enclosed courtyard provides a place for residents to spend time outdoors, and meals are served family style in the units private dining room.

Menu options include salmon, steak, hamburgers and salads. Physical Therapy Physical therapy can foster independence and enable functional mobility skills following, or in spite of, neurological and/or orthopedic conditions or injuries. Occupational Therapy Occupational therapy helps patients restore and relearn functioning skills that may have been lost due to an illness or injury.

Accommodations
Comfortable private and semi-private resident rooms are available. To maintain a personal, comfortable atmosphere, residents are encouraged to add personal touches to their rooms by bringing mementos from home. Rooms include telephone and cable television connections. Residents are able to select their choice of wake-up, meal and shower times. The facility also offers spa appointments in which the resident can schedule an appointment for a whirlpool bath or shower in one of the facilitys spa areas. Each spa area features a relaxing fountain, soft music and tropical scents.

Skilled Care Professional Accreditation


Life Care Center of Sullivan is accredited by The Joint Commission, an independent, not-for-profit organization that evaluates the quality and safety of care of more than 15,000 health care organizations. Our associates provide 24-hour skilled nursing care for all residents; this may include assistance with dressing, bathing, walking, eating and the administration of medication. After discharge, one of our associates will follow up with the resident and his or her family. We want to ensure that the resident has made the transition home smoothly and offer additional assistance, as needed.

Butterflies Are Free


Life Care Center of Sullivan has implemented a special program entitled Butterflies Are Free for residents at or nearing the end of life. Our sole purpose is to provide comfort and dignity to these residents so that their experience is as comfortable and pain-free as possible. Honoring the wishes of the residents and their families is paramount.

Camellia Gardens of Life Care is proud to be accredited by The Joint Commission. The Joint Commission, an independent, not-for-profit organization, is a world leader in evaluating the safety and quality of the services provided by more than 15,000 health care organizations across the United States.The organizations primary focus is on evaluating facility performance in areas that most affect patient health and safety.The Joint Commission is governed by a board of both health care professionals and consumers. To earn accreditation, a nursing home or other facility must undergo an extensive on-site review by a Joint Commission team, made up of professionals such as physicians, nurses and administrators. The team assesses the facilitys processes, interviews residents and observes associate performance. The survey must be completed at least once every three years for the facility to maintain accreditation.

Camellia Gardens of Life Care offers rehab patients the Ready Set Go! program to help them return home safely. In this program, caregivers work together with each rehab patient and his or her family to establish recovery goals and discharge plans. The mission of the program is for the resident to meet one of the following criteria before discharge: v Live alone and be responsible for all personal self care and home care. v Live at home with someone else but be responsible for all his or her home care. v Be discharged to an assisted living facility that requires him or her to be independent in areas of home care and some meals. Residents participate in clinical testing to evaluate their level of independence in the following areas: v Completing self-care tasks, including gathering and transporting all personal items to and from the shower, using adaptive equipment if needed, showering, dressing, grooming and cleaning up. v Making their own beds. v Planning and preparing meals. v Setting the table and cleaning up after meals. v Money management (if needed). v Shopping (if needed). v Possible restaurant outing. v Requesting medications. v Transferring in and out of car safely. v Moving without the use of adaptive devices such as a wheelchair, walker, cane, etc. v Communication skills (phone and computer). We are proud of our rehab patients in this Ready Set Go! program for working hard to achieve these goals. We see amazing stories of recovery and are happy to be a part of restoring our residents and patients to their highest functioning abilities. Sara Stover, RN, Staff Development Coordinator I started at Villa Manor in 2007. I am originally from Iowa, and I received my degree from Luther College in Decorah, Iowa. I am responsible for mandatory educational items for all associates. I also help with human resources at Villa Manor. My passion is teaching, and I enjoy adding humor to my lessons to help associates remember the information. grocery store, such as canned goods, produce, soap, shampoo, coffee and baked goods. All items were designed to have the approximate size and weight of the actual item. As part of our discharge planning process, patients are expected to complete the Ready Set Go! program. This includes meal planning and shopping for the necessary items. Once they have made their grocery lists, the patients go through the grocery store, pick out and pay for their items, and return to the Ready Set Go! apartment to prepare their chosen meal. For the therapists, this is a wonderful opportunity to provide patients with cognitive exercises involving meal planning and money management as well as numerous balance, strengthening and reaching activities they need to master prior to returning home independently. For the patients, they are able to complete function-based activities in a fun, safe and structured environment. As one of the patients put it: This is one of the many innovative activities in rehab that I look forward to because I know it brings me a step closer to my ultimate goal of returning home.

Life Care extends compassion to earthquake victims


On Jan. 12, 2010, a 7.0 magnitude earthquake struck Haiti, with a suspected death toll of at least 217,000.The tragedy deeply affected all of us, whether directly or indirectly. Our residents and associates sprang into action to support and care for the victims of the quake. The Heritage Center in Morristown, Tenn. Associates raised almost $100 for the American Red Cross by selling dress-down days. Executive Director Robert Breeden said,I was very proud of our associates for participating in this way.They always have a big heart when it comes to needs like that. Life Care Center of Leominster, Mass. The facility raised $200 for Forward in Health, a non-profit organization that strives to improve the dignity and health conditions in Haiti. Case Manager Kim Jollimore also visited Haiti to treat earthquake victims. I had always heard how much the people of Haiti needed help, Jollimore said.But when I saw it firsthand, it was more than I expected. Cherry Hill Manor, Johnston, R.I. When an associate lost family members in the earthquake, the entire facility family hurt. Associates coordinated a bake sale and sold chances to win gift baskets, raising more than $1,000 to donate to the Red Cross. Life Care Center of Crossville, Tenn. The facility served as a drop-off point for the American Red Cross Haiti relief efforts, daily accepting supplies such as over-the-counter medications, bandages, flashlights, batteries, etc. 6 Life Care Center of Valparaiso, Ind. Associates showed love to the victims of the earthquake by combining their fundraising efforts with Valentines Day celebrations.They sold chances to win eight themed gift baskets, along with a bake sale. In all, they raised $489. Not only did it raise money for Haiti relief, but it created a positive spirit in the facility, said Amber Janeczko, executive director. I am lucky to have such a caring group here. Life Care Center of Estero, Fla. Associate Carline Honore, a Haiti native, was compelled to return to her homeland with Organisation ROME Haiti to administer medical aid. Honore hopes to return to Haiti to help as much as possible as the country recovers. Honore said of her visit,I remember that the little children were coming and said,Im hungry. And its the little children that stay in my mind and my heart. Most of them lost their house, and they have to sleep on the street. Life Care Center of Farmington, N.M. Almost two weeks after the earthquake, Danny Begay, licensed practical nurse, headed to Haiti with a medical team from Relief Ministries based out of Sarasota, Fla. According to an article in the Farmington Daily Times, he worked out of a makeshift hospital, helping wherever needed and even streamlining some of the processes being used by the medical staff. conditions.Fourteen days after the earthquake, people were coming in with infections because they hadnt changed their bandages since then, he said.We were seeing people with broken bones who hadnt been seen yet. The Highlands, Fitchburg, Mass. The Resident Council and activities department banded together to raise $1,000 for the non-profit organization Haitian Outreach by hosting three fundraisers: an afternoon cocktail party, a baked potato bar and a drawing for two Italian gift baskets. Evergreen House Health Center, East Providence, R.I. Proceeds from the facilitys fourth annual winter carnival, $413, were donated to the American Red Cross for Haiti relief. Life Care Centers of America, Corporate Offices, Cleveland, Tenn. Dana Williams from the Public Relations department traveled to Haiti March 20 26, with SCORE International, a non-profit organization, to offer humanitarian aid to the devastated country. While there, her group prepared an orphanage for the upcoming school year and removed rubble from a church just miles from the epicenter of the earthquake. During the visit, Williams was most impacted by her visit to a mass burial site: A blatant contrast of life and death came into focus. Endless blue waters and a beautiful horizon flaunted abundant life just across the road from a stretch of desert-like land covering hundreds of thousands of bodies buried in mounds just a few feet beneath the earth bodies of wives, children, parents and friends.
