Part I: Introduction It was September 6, 2011, a sunny Tuesday morning with a cool breeze complimenting the blue sky, green grass, and the birds who paint some sounds on a rural view, the interaction was done beside a fruit bearing tree, I sat in front of the patient to able to see his gestures, some students from other schools are observing our activity. My colleagues are a little bit anxious regarding the tnpr because its a new experience for them, although I knew that they know we are equipped with some theories and foundation of psych nursing, we have no choice but to immerse with our patients and do our thing which is, to be therapeutic for the patient. Patient X, my patient, is a 44 year old male client of pavilion twenty two, upon seeing him, I greeted and introduce myself, afterwards, we did some hygiene activity, I instructed him to brush his teeth and wash his face with soap and water as his upper arm and lower extremities, as he cleanse his leg, I noticed that he has a fresh wound on his right knee, so I called the attention of my clinical instructor to get a antibiotic cream from her to be applied over the wound of patient X. He was aware of the 3 spheres: person, place, and time, he is a little bit timid and shy, but cooperative and well behaved especially when asked some question to him he respectfully responds to it he can, but minimal only. Part II: Goals(short term) After 30 minutes of nursing intervention the student nurse will be able to: Introduce self and establish rapport with the client. Gain trust from the client. Let the client express his thought and feelings.

Uncomfortable, because im in doubt of his answer. Uncomfortable, knowing the client stayed there for so long feels like disappointing. Uncomfortable, patient has a flat affect and seemed callused about his situation. Comfortable, client is sociable and can blend with others. Uncomfortable, its kind of distressing to know that he is being captured by the police. Uncomfortable, his brother abandon him, it must be difficult on his part.

After 30 minutes of tnpr goal was partially met, I was able to gained trust and establish rapport from my client and let him express his feelings and thoughts, although he is still a little bit passive yet he enjoy my company. As a student nurse, this was the best NPI I experienced, because it has a special kind of communication technique which you need to apply. My plan for the following interaction is to let the patient be more talkative and expressive of his thoughts and feelings.
