HEALTH PE RCEPTION AND HEALTH MANAGEMENT PATTERN Before confinement, the client rated his health, 1 out of 10 , by 10 being the highest and 1 being the lowest. And he stated ung baso di ko mahawakan ngaun ko lang naramdaman un, ayoko na nang maramdaman un e. He do physical check up annually. His personal hygienic practices are taking a bath everyday, changing of clothes after taking a bath, doing hand washing before and after eating, after 3 to 4 days trimming of nails, brushing of teeth 3 times a day. He drinks alcohol occasionally and prefers to drink beer. He usually drinks 8 bottles of 320 ml of beer. He can consume 1 pack of cigarette in 1 days. During confinement he rate his self 8 out of 10 and stated okay na kasi ako pero anytime pwede pang magkasakit. His health beliefs are to eat fruits and have a balance diet. E. NUTRITIONAL AND METABOLIC PATTERN Before confinement, hes typical food intake was more on fish than pork and beef. He drinks approximately 10 15 glasses of water a day, he dont drink soft drinks. He likes fish and hates oily foods. The client eats together with his family at home. They dont do food budgeting instead, they buy anything they want to eat. No eating discomforts. Wound heals fast. During confinement, hes eating foods same as before the confinement. He eats bread or porridge before eating rice meal. F. ELIMINATION PATTERN Before confinement, the client defecates once a day every morning. Its characteristics are hard, brown in color and with discomfort/pain because of the mass. With regards to urinating, the client urinates 5 times a day with pale yellow in color, clear and no discomfort/pain. During confinement, the client reduces pressure when he defecates because of his fistula. When he unintentionally forced to defecate, it is painful. On the other hand, he doesnt have any difficulty in urinating. He drinks a lot of water to achieve normal elimination pattern.

G. ACTIVITY-EXERCISE PATTERN When the client is not yet confined and still working on the ship, he wakes up at 6AM. He works for the whole day until 5PM. He doesnt have any leisure time except for break time. Whenever he had nothing to do, he jogs for 1 hour three times or less in a week. He plays basketball for two hours when he is at home.

During confinement, he still wakes up at 6AM. When hes at home, hes fond of watching cock fight in the afternoon. He spends three hours there.

H. SLEEP-REST PATTERN Before confinement, the client was able to have a normal number of sleep of 6-8 hours. He usually sleeps at 9PM and wakes up at 6AM. He always have a straight sleep at night. He feels good and refreshed upon waking up because he doesnt have any problems on his sleeping environment. During confinement, the client experienced an intermittent sleep. He cant sleep well in the hospital so that he doesnt feel refreshed in the morning. He drinks milk to help him fall asleep. I.COGNITIVE-PERCEPTUAL PATTERN The client is able to read and write without any difficulty. He doesnt use hearing aids but uses eyeglasses. Eyeglasses grade was not specified. He learns easily when instructions are carefully explained on him. Like for example, having a wound care, he prefers to explain it well to him and demonstrate each step so that he will learn the proper way of cleaning his wound. J. SELF-PERCEPTION AND SELF-CONCEPT PATTERN Before and during confinement, the client feels good and calm most of the time. He doesnt think much about problems in life. He takes life easy; he doesnt want to stress himself out. Except for the fistula on the upper part of his anus, there are no significant changes on his body. Whenever there is something that bothers him, hes not used in sharing it to other people instead he keeps it on his own and try to find some solutions about it. K. ROLE-RELATIONSHIP PATTERN Every time the client has a contract abroad, hes not able to be with his family. He is a jolly person and they have a happy family. He always assures that his wife and child are in good condition, meaning they have their necessities and everything since he is the provider for the family. During confinement, since he went back home for his operation, he had a quality time for his wife and child. He feels closer to his child because he always makes sure that hell show to his child that he is loved. And he knows that a fathers love is one of the most important things to a growing person. L. SEXUALITY-REPRODUCTIVE PATTERN

He only has one child. He doesnt want to have more than one because he is not always beside his wife to take care of their child since he works abroad. According to him, they are concerned about family planning. He states that nowadays, it is very difficult to raise a child so they decided to have only one. And he also added that he wants to focus on their child unlike other family, they have more than three yet they cant sustain the needs of every individual. M. COPING-STRESS TOLERANCE PATTERN The client feels calm most of the time so the word stress is not applicable on his vocabulary. But according to him, he gets mad when there is something wrong done. Like when his child didnt do good in school, he wants his child to have a good foundation in education because he will use it in the future. There are no stressful event was mentioned other than that. N. VALUE-BELIEF PATTERN Before confinement, a person is healthy if he balances life well. Like him, he doesnt stress himself with the thing that goes around him. Hes just relaxed and find ways to resolve the problem. Hes a Roman Catholic so religion is important to him. When hes at the ship, he never forgets to pray before going to sleep to thank the Lord for the good things that comes to their way, and asks to take care of his family since hes not there to guide them every minute of the day.

HISTORY OF PRESENT ILLNESS 1 year prior to admission, the patient experience anorectal mass about 0.2 * 0.2 cm associated signs and symptoms noted. He took amoxicillin but no consultation done until; 5 months prior to admission, the size of the mass increased associated with difficulty in defecating with anorectal pain. There are no associated signs and symptoms until; 1 week prior to admission, still with above condition. This time, he sought consult in OPD and was scheduled for OR.
