Cognitive function: Rancho Level Clinical Correlate I No Response II Generalized response III Localized response IV Confused-agitated V Confused-inappropriate VI Confused-appropriate VII Automatic-inappropriate VIII Purposeful and appropriate A series of cognitive tests for primary assessment and ongoing evaluation in treatment of brain injured individuals. To identify fall hazards and accessibility issues of the homeowner and family members Evaluate accessibility issues in a clients home Evaluates risks in various rooms of the home as well as general risks such as fire hazards; examines safety issues in self-care and household tasks; examines the potential for wandering and the use of memory aids; and identifies how help could be summoned It helps identify occupational performance issues that are important to the client.

All clients with cognitive ability and insight

Occupational Self-Assessment (OSA)

The OSA is comprised of four domains which almost All clients with cognitive ability completely correspond with the Occupational Therapy and insight Practice Framework: 1. Skills/Occupational Performance 2. Habituation -Habits, Roles 3. Volition -Personal Causation, Values, Interests 4. Environment -Physical Environments: natural and built environments, objects -Social Environments: social groups, occupational forms Descriptive questionnaire that documents the individuals participation in occupations by 30 min. intervals and describes clients perceptions of type of occupation along with sense of competence, value, and enjoyment attributed to each occupation. Designed to collect a broad range of information believed to influence and indicative of a person's occupational performance and to identify areas needing more in- depth evaluation. Identifies risk factors for work-related musculoskeletal disorders related to awkward postures, repetition, force & contact stress A means to assess the amount of fatigue that accumulates in muscles during various work All clients with cognitive ability and insight

For use in ergonomic investigations of workplaces where work related UL disorders are reported. RULA is a screening tool that assesses biomechanical and postural loading on the whole body with particular attention to the neck, trunk and upper limbs. A means to assess posture for risk of work related musculoskeletal disorders OSHA checklist for computer use (Video Display Terminal = computer) Creating a safe and comfortable workstation Ameliorating behaviors that impede learning or quality home life.

Intended to establish the degree of independence of the patient from any help however minor and for whatever purpose. Evaluates a patient's self-care abilities in 10 areas, including bowel and bladder control, feeding, bathing, dressing, and walking. Assess clients ability to perform activities of daily living independently. The Index ranks adequacy of performance in the six functions of bathing, dressing, toileting, transferring, continence, and feeding. Measures ADL independence. Items are divided into six sub-dimensions: Mobility, Emergency Communication, Dressing, Elimination, Bathing/Hygiene, and Eating. Links performance in activities of daily living (ADL) to neurobehavioral impairments

18-item (13 motor & 5 cognitive), 7 level instrument to measure the effectiveness of rehabilitation interventions and to describe outcomes in a uniform way, with respect to functional status (disability) and burden of care (type & amount of assistance) Determines ability to function in 17 basic living skills in the arenas of self-care, safety and health, money management, transportation and telephone, and work and leisure. A functional measure that records the level of cognitive support required by a person Alzheimer's to complete a cooking task successfully. The results allow the clinician to help caregivers understand the level of support the impaired person needs to perform daily living tasks. Evaluates the individuals ability to perform the IADL tasks he or she needs and/or wants to perform. AMPS consists of 80 IADL tasks. Persons are observed performing two to three tasks of their choice.
